LIFTING THE TEMPORARY SUSPENSION OF THE ISSUANCE OF TRAVEL


AUTHORITY/ PERMIT IN THE MUNICIPALITY OF CALASIAO

Whereas, a temporary suspension on the issuance of travel authority/permit was issued


by the Local Government Unit of Calasiao as a precautionary and preventive measure to
prevent the transmission of the COVID- 19 virus;

Whereas, contact tracing and other health protocols were strictly implemented in areas
where some of our residents had registered positive result for the COVID-19 virus;

Whereas, it is no longer necessary to suspend the issuance of travel authority/permit in


order to normalize the movement of our residents within and outside the Municipality
subject to strict health and safety protocols recommended by our the municipal health
officer as follows:

1. All applicants should present a valid ID to the barangay officials prior to the grant
of a Barangay Clearance and to the Health Office personnel as an initial
requirement for Medical Clearance Certification. The ID should bear the address of
the applicant;

2. The Barangay Clearance and Medical Clearance Certificate should state the validity
period;

3. Applicants should pay the appropriate fees allowed by law as stated in the DILG
Memorandum Circular No. 2020-097 for the Barangay Clearance and Medical
Clearance Certificate;

4. For the Medical Clearance Certification, applicants shall submit their Complete
Blood Count (CBC) result. The CBC result shall not be more than 24 hours
upon application;

5. The applicant should be reminded that a new Medical Clearance Certificate will
only be issued again to them after 14 days to ensure that they will adhere to the
14 day home quarantine period upon return to the municipality. Once Travel
Authority issuance is resumed, there will be a list of those with approved
Medical Clearance Certifications to countercheck the date during their
reapplication for Medical Clearance Certification;
6. Authorized Persons Outside Residence (APOR) will not be issued Travel
Authority but if so required, the APOR must go through all the processes related
above and to show proof of their being an APOR such as company, business
permit and/or employer's letter of employment;

7. The schedule of the issuance of Medical Certificate at the Rural Health Unit-1
will be from 8:00 AM to 11:00 AM, Monday to Friday to allow the Health
Office staff to attend to other health programs, data recording and reporting in
the afternoon

Now, therefore by the power vested in me, I, Joseph Arman C. Bauzon, do hereby issue
this Executive Order effective today, July 13, 2020.
