Narrative information

On 08/08/2020 at 1251 hours i was dispatched to a missing adult report for Jacob W. Cefolia at E The compiamant was an ex-wife
(KristEne M. Cefolia), who stated she has not heard from Jacob since 08/06/2020. She advised he lives aione at

I arrived on scene to speak with Kristine, while Officer Pacewic responded to 454 S. Rex. Kristine stated her son was the last person to see and
speak with Jacob, which was on 08/06/2020. He went there for dinner and left between 2000 hours and 2030 hours. He (son) advised KristEne
that Jacob appeared to be extremely drunk. After Jacob did not answer phone cal!s and texts, Kristine went to his house on 08/08/2020 around
1130 hours. Kristine stated his vehicle was not parked outside or in the garage. She also noticed a note on the front door which was from
Jacob's co-worker (Joe Tibb!e). At this point she spoke with Joe, who advised he also has not spoke with Jacob since 08/06/2020. Kristine then
called family members; no one saw him after 08/06/2020. Kristine advised Jacob does not take any medication and does not have any mental
illnesses to her knowledge. However, she advised he might have a drinking problem. Knstine also advised that Jacob has "fantasized about
going off the grid" in the past; meaning no eiectronics or communication with people. Kristine couid not think of anyone that Jacob might be
staying with.

At this time Officer Pacewic advised Jacob's vehicle was not at the address and there was no answer at the door. See his supplemental report for
further.

-then called Jacob's coworker from United Airlines, Joe Tibbie f^BW^^B. Joe spoke with Jacob on 08/06/2020 around 1200 hours for work
purposes and was later at his house around 1830 to 1900 hours. Joe believed that Jacob broke up with his girlfriend in San Francisco; Jacob
mentioned "it's all Wendy's fault" Joe was not certain what that was in reference to. Joe was then notified that Jacob was not working on
Friday, which was unusual. Joe advised they had been primariiy working from home, but Joe did not report online. Joe checked the United
Airlines employee fiight logs, which showed Jacob's last flight on 08/05/2020 which was inbound from San Francisco, Caiifomia. Joe did not •
have any further information.

At this point I called his girifriend from San Francisco, Wendy Rodriguez^8®g^^^p!f Wendy stated she broke up with Jacob a few weeks
ago, but Jacob wanted to get back together with her. Jacob flew out to San Francisco for her birthday on 08/04/2020 and stayed the night. She

stated nothing seemed out of the ordinary; he returned home on 08/05/2020 because he was watching his kids on 08/06/2020. Wendy stated
she knows Jacob has been to therapy, but does not know where or who the doctor is. She also advised Jacob mentioned something about
wanting to go into the mountains to be alone. She knows Jacob has been stressed out over work and is drinking aicohol more than usual, but
he did not make any suspicious/conceming statements.

then spoke with Jacob's sister, Genny: who lives in Bioomington. Genny stated Jacob came to her house with his kids on
08/02/2020. He seemed to be in good spirits; nothing seemed out of the ordinary. Genny made me aware of another possible girlfriend, Dina.

spoke with Dina via teiephone Dina last spoke with Jacob on 08/06/2020 around 1430 hours through text messaging. She
sent several more messages throughout the day and again on 08/07/2020 but received no response. Dina then became worried and spoke with
Genny. Dina said Jacob did not make any unusual statements and could not think of anywhere he might be staying. Dina advised Jacob was
supposed to go to her house for dinner today, 08/08/2020.

I was made aware that Jacob's vehicle (2019 Land Rovei|§^g||g| was seen driving westbound on ButterfEeld and Commonwealth on
08/07/2020 at 0923 hours. DuComm also advised that DuPage County Sheriff's ran his license plate on 08/08/2020 at 0113 hours and 0407
hours, They did not know the situation or which Sheriff ran the plate.

I attempted to call Eilinois Tollway in order to track Jacob's !-Pass; the offices were closed. See Officer Pacewic's supplemental report for further.

Jacob and his vehicle were entered in LEADS by Service Officer Raga.

On 08/08/2020 Elmhurst Officers searched Jacob's residence with negative results; see Officer Schmidt's supplemental report for further.

On 08/09/2020, I was made aware that DuPage County Forest Preserve officers located Jacob's vehicle at a forest preserve in Lemont some time
[ast night. A K-9 search was conducted with negative resu!ts.

Narrative Informaiion

On 08/08/2020 at 2122 hrs Commander Kaczorowski, OFC Carney, OFC Jaffe, and I went "to^ggg^F to attempt to locate and check the we!!
being of Jacob Cefolia. Upon arrival, we discovered no vehicles in the driveway and minimal lights on inside the home.

We checked the perimeter of the house and did not locate Jacob. The house is currently Sisted for safe and Commander Kaczorowski contacted
the real estate agent, Melinda Josephs, who advised she would respond to the house and unlock the inside so we couid check the interior.
MeEinda arrived on scene and at 2209 hrs she unlocked the rear door. Due to the circumstances surrounding Jacob's disappearance,
Commander Kaczorowski advised OFC Carney, OFC Jaffe, and I that we wouid be making entry into the home due to exigent circumstances.

We cleared the interior of the home and at 2217 hrs exited the residence, having not located Jacob. Melinda advised us that she had not heard
from Jacob in a few days and he was supposed to meet her at the ^^g^feddress on Thursday 08/06/2020 at 1730 hrs; however, he never
showed up. Melinda said that was very unlike him. Melinda also advisecfthat on Tuesday 08/04/2020 he flew to San Francisco to break up with
his girlfriend. Meiinda further told us she was a friend of his for many years and she was concerned about his well being.

R/0 contacted Meiinda 'Mindy' Josephs, who explained that she is the realtor for the home owned by the Victim, which she added there
currentiy is a pending contract on his house. MEndy stated she !ast spoke to Jacob on Thursday through a series of texts earlier in the day,
because they were supposed to have a meeting in the evening around 5:30 PM, but he didn't show. Mindy's husband, Bruce Josephs, was
Jjsteninq to the ca!l, and explained that he worked with Jacob for many years, and reported that a co-worker of Jacob, Joe Tibble
iad spoke to Jacob Thursday night. Mlndy nor Bruce has seen or spoke to Jacob since.

The neighbor across the street, Dave Gass @WSBBS^ reports that he is 60-70% sure he saw the dark Land Rover leaving the driveway
yesterday, 08/07/2020 just before noon. Dave could not add any further information as to if anyone else with Jacob or if he had any baggage
with him.

Officer Coughlin checked the registration for the Land Rover through the LPR system and found that the vehicle was captured on 08/07/2020 at
0923 hours, westbound Butterfield at Commonwealth.

Checking the Registration through LEADS, it was found that the registration was run by DuPage County Sheriff's MDC (MGY) on 08/08/2020 at
both 0113 hours and 0407 hours. DuComm was requested to contact DuPage Sheriffs concerning why the plate was run, but the Sheriffs
Dispatch was unable to find any record of why the plate may have been run, being any Service Calls associated with that plate, nor could they
track who ran the plate, other than to say it was En a squad. DuPage Sheriff's Dispatch advised a message will be sent out for the late shift
Officers starting duty to see if anyone recalls running the Victim's plate.

R/0 contacted the Land Rover Dealership in Naperville to see if the vehicle could be GPS tracked or located. R/0 was directed to Roadside

Assistance @ 1-800-Find 4WD or 800-637-6837. R/0 spoke to Roadside Assistance who said R/0 needed to contact Customer Sen/Ece.
Customer Service was found to not work weekends. R/0 was able to find an Agent of Land Rover who explained that this vehide, based off the
last 8 of the V!N, is In-Contro! Active, which means it can be located, but that has to be done through Sirius @ 888-539-7474. SErius was also
found to not be available on the weekends.

R/0 was able to connect to Sirius via a Chat Conversation on the computer, but was informed that Sirius cannot locate the vehicle,

Narrative Information

On 08/10/20,1 assisted with this missing person investigation, Jacob CefolEa.

Investigation revealed by Detective Torza that Cefofia's vehicle was at the Shell gas station, 301 W Butterfieid Road, Elmhurst, at 0919 hours on
the 7th.

We responded to the above-mentioned Shell Gas Station, where I was allowed access to the store s surveillance camera system. There E was able
to locate surveillance video of what appeared to be CefolEa's biack Range Rover, puil off of Spring road and up to a gas pump. A male subject
exits the driver's door, and looks to pump gas into the vehicle. The driver gets back into the car, and it ieaves the parking lot. Detective Torza
was able to get a copy of the transaction from the store's computer. It was determined that the credit card used for this transaction belonged to
thatofCefolia.

Contact was made with Pieter Post, a regional investigator with Chase, to see if the missing person had used this credit card since reported
missing. Post was able to supply me with documentation showing that Cefoiia had no activity on his credit card since August 7th, with the last
transaction being at the Sheil Station in Elmhurst, 218 E Butterfield Road.

On 08/12/20, contact was also made with David Gamez, an investigator with USAA Banking. Gamez relayed to me that Cefofia did have an
account with USAA, which is an active checking account that appears to be used for automatic bill payments and not for daiiy use. Gamez also
relayed that there have been no large cash withdrawals in the past 7-10 days.
