AUDIT - EXECUTIVE SUMMARY

This audit was conducted as an internal maintenance audit to satisfy the requirements of
the Certificate of Recognition process and provide insight and understanding of The
Company’s Health and Safety Program. The company has actively enforced, supported,
and encouraged involvement in their program and the results have produced a safer and
more conscious work force.

The scope of the audit included the several worksites including those in:

All levels of workers, in each division of the company were included in this audit.

The overall score for this audit is ____%. This satisfies the minimum requirement of
80% to maintain the Certificate of Recognition. The company experienced changes in
the work load due to fluctuations in the economy. This resulted in a need for high
performance, maximum safety on all jobs, and quality programs & leadership. The
company’s success in safety was largely due to their exceptional performance in
Communication, Training & Incident Reporting and Investigation.

Excellence
Communication - The multi faceted communication network allows for workers to gain
access to company information in a variety of ways. Learning in more than one medium
increases understanding and the recall of information.

The company has identified their workers require verbal training through orientation and
meetings, written reference material through notes, bulletins, signs, and memos; and
visual training through the use of computer based programs, videos, and hands on
training. Congratulations on meeting the needs of your workers and maximizing learning
abilities!

Training – Standardization of the orientation program has led to a strong basic training
system for all workers. A matrix has been developed and implemented for continuing
training requirements and allows for easy reference and follow up of additional training
elements. The Company uses in house and third party providers to ensure their workers
are trained and certified in accordance to industry, client, and legislated requirements.
Great progress on the consistency of the training program!

Incident Reporting and Investigation – Continuous effort has been made by

management to ensure that all incidents are reported to supervisors and that appropriate
investigation and reporting is conducted. The company is receiving timely and accurate

reporting of incidents that occur on their worksites and participation from workers is a
key element to ensuring this program remains successful.
Providing workers with options for reporting such as near miss reports, anomalies, hazard
id’s, incident reports, and so forth allows the workers to select the most appropriate
document and effectively communicate the incident at hand. Well done!

Opportunity
As with all projects, there is always opportunity for growth and development. The
overall performance is excellent and these standards will be even higher with
development in the following areas:

Management Involvement & Commitment – Senior & Middle Management must be a

known presence in the field. For a company of this size the most effective way to ensure
this occurs will be through planned field visits. Dedicate a bi-annual site tour for
management to tour active worksites, perform inspections, host safety meetings and talk
with workers one-on-on. Be sure to document the planning of the tours as well as the
execution of the tours.

Rules and Work Procedures – The company should continue to review, update, and
integrate the company work procedures into the company program. Existing procedures
are basic and not fully integrated for use by workers. Involvement from workers in the
development and review process will encourage participation and buy in from personnel
and self-promote the program.

As the company has continued to grow they have remained committed to the continuing
development of their health and safety program. This is evident through excellent
performance, worker knowledge, and company records. The company is now at a level
within their own system where they are ready to challenge themselves with new
initiatives and exciting expansion to the safety program. All members of the company
are on board with the safety program and this in itself is a great success!

Congratulations on the success of your Health and Safety Program and the subsequent
success of this audit. Hard work has paid off and will continue to do so with the effort
and support of management and staff. Best wishes for next year!

Sincerely,
