ACTOR AGREEMENT

This agreement is entered into by and between (PRINT NAME) _________________________ (the
“Actor”) and ThroatPunch Productions, LLC (the “Production Company”). In the event of the Actor’s
death or total incapacity, “Actor” means the Actor’s heirs, devisees, beneficiaries, trusts, assignees or
other successors-in-interest. “Production Company” includes the licensees, assignees, future owners, or
other acquirers or successors-in-interest of ThroatPunch Productions, LLC.

In connection with the motion picture tentatively titled “Sauless” which is based on the story and
screenplay by DM Rayton (the “Picture”), for payment of $0 USD, the Actor agrees to perform the role
of ______________________ in the Picture (hereafter the “Performance”), throughout each and every
required shooting, occurring over the months of April 2010.

Actor hereby grants to the Production Company and to their licensees, assignees, and other successors-
in-interest, all rights of every kind and character, in perpetuity and throughout the universe, the
following permissions:

a) the right to film, photograph, and otherwise visually and audiovisually record the Performance,
including but not limited to the Actor and the Actor’s voice, conversations, sounds, stories, statements,
whether written, spoken, sung or otherwise uttered or expressed by Actor, or information given by
Actor, captured or any such recordings (hereafter collectively referred to as “Recordings”);

b) exclusive ownership of the Recordings, granting the right to, without limitation, in perpetuity
throughout the universe, in any and all languages, in any and all media now known or hereafter
invented, exhibit, broadcast, use, reproduce, and license others to use any and all parts of the
Recordings, whether edited, dubbed, combined with other material, and/or incorporated into any other
program(s), in connection with the production, distribution, advertising, publicity, promotion, exhibition
and other exploitation of the Picture and in connection with the products and services of the Production
Company;

c) the right to use Actor’s name, voice, likeness, image, photograph(s), performance(s), expressions,
appearances, actions, activities, statements, personal experiences, and biographical material in
connection with the production, distribution, advertising, publicity, promotion, exhibition and other
exploitation of the Picture and in connection with the products and services of the Production Company;

d) exclusive ownership of any ideas written, suggested, or submitted as well as any and all physical
representations thereof in connection with or in relation to the Picture (hereafter collectively referred to
as the “Contributions”), without limitation, in perpetuity throughout the universe, in any and all
languages, in any media now known or hereafter devised, and the right to exhibit, broadcast, use,
reproduce, and license to others to use any and all parts of the Contributions, whether edited, dubbed,
combined with any other material, and/or incorporated into any other program(s), in connection with the
production, distribution, advertising, publicity, promotion, exhibition and other exploitation of the
Picture and in connection with the products and services of the Production Company.

The Actor agrees to show good spirits and a positive attitude for the duration of principal photography.
Unless otherwise agreed to in writing by the Production Companies, the Actor will receive no
compensation or credit in the event that the Actor does not complete the Performance.

The Actor hereby waives any right that he/she may have to inspect or approve the finished motion
picture, or the advertising copy or printed matter that may be used in connection therewith, or the use to
which it may be applied.

The Actor understands and agrees that filmmaking can be a challenging activity and may have
unforeseen delays which are out of the control of the Production Company. In such event, Actor agrees
to cooperate with the Production Company and understands that he/she may be required to perform their
role during unscheduled shoots as well. Actor further agrees to any pick-up shots and additional dialog
recording (ADR) required to complete the Picture.

For all lawful purposes pertaining to the Actor’s employment with Production Company, he/she will be
considered an independent contractor and not a direct employee.

The Actor further acknowledges that the Production Company owns all rights to the Picture.

Actors agrees that exact wording or phrasing of screen credits will be at the Production Company’s
discretion.

This Agreement constitutes the entire agreement by and between the Actor and the Production Company
and supersedes any and all prior contracts, understandings, negotiations, and agreements with respect to
the Production Company and the subject matter hereof, whether oral or written.

signed this document, on behalf of my child, of my own free will.
