LUIS BEACH MINI STORAGE

Luis Beach, Ca. 93277

San Luis Beach Mini Storage is ordered to provide the following records regarding video surveillance
captured on May 29, 2010 between Complaining Victim and Willing Suspect (Avis Car Rentals):

Copy of compact disc containing all video surveillance, still photographs, and any and all recordings regarding
the incident that occurred between Complaining Victim and Willing Suspect (Avis Car Rentals) on May 29,
2010, marked by San Luis Beach Mini Storage as “May 29, 2010 Incident.”

These records are to be returned to the Affiant within ten (10) days from the date in which the order is served.

(AFFIDAVIT IN SUPPORT OF COURT ORDER)
I, Detective Arrestor, declare under penalty of perjury, that I am a Police Officer for the City of San Luis Beach,
County of Los Angeles, State of California, and have been employed as such for the past 10 years. I am
presently assigned to the Investigations Bureau.

On May 29, 2010, the San Luis Beach Police department took a police report detailing a battery that occurred
between Complaining Victim and Willing Suspect. The police report (10-6587), attached with this court order,
details an incident that occurred between Complaining Victim and Willing Suspect at 8311 N. Maple Avenue.
Both parties are listed as both victims and suspects. Complaining Victim claims that Willing Suspect assaulted
him by pushing open her vehicle door forcibility into him, striking his right hand, causing pain (misdemeanor
violation of 242 PC). Willing Suspect claims that Complaining Victim yelled profanities at her and knocked on
her vehicle window (misdemeanor violation of 415(3) PC). The reporting officer for this report was Officer
Jackson, who noted in the report that CCTV cameras are present in the area where the incident took place.

Customer Helper, manager at San Luis Beach Mini Storage told Officer Jackson that the CCTV consists of stop
action images. Your affiant has spoken with Customer Helper, and Customer Helper told your affiant that the
company, San Luis Beach Mini Storage does have a compact disc with the video surveillance of the incident,
but the company will not release the compact disc of the video surveillance without a court order of court issued
subpoena.

The San Luis Beach City Prosecutors office has requested that your affiant obtain a copy of this compact disc in
order for the City Prosecutors office to review the video surveillance in order to assist in their determination of
filing charges against Willing Suspect and/or Complaining Victim, the parties involved in this incident.

Your Affiant requests the Court’s permission to obtain records from San Luis Beach Mini Storage regarding the
items listed on the face page of this document. Your affiant believes the records will assist in the investigation
to prove or disprove the allegations claimed by both Complaining Victim and Willing Suspect regarding the
misdemeanor assault and battery charge and misdemeanor disturbing the peace charge.

This affidavit is sworn under penalty of perjury and is executed in Los Angeles County on ____ day of
