Abstract
Introduction: The third molar is one of the most common procedures in oral surgery.
Some complications are associated with the process, but the displacement of dental
elements is a related problem and hardly encountered. Case report: A 17-year-old
female patient attended the dental office with pain complaints after an attempt was
made to remove the germ of the left upper third molar in an intraosseous position that
the patient moved to an infratemporal fossa. The treatment with the surgical approach
occurred, without complications. Conclusion: The professional should be prepared for
cases, for access to surgical removal of the tooth in the infratemporal fossa is not
difficult, but also to change the morbidity of structures that cross it, so the use of
radiographic exams of excellence is indispensable.
Introdução
A remoção do terceiro molar sob anestesia local é um dos procedimentos mais comuns
em cirurgia oral1,2. Algumas complicações são associadas a esse procedimento, sendo as mais
citadas: Hemorragia, infecção, disestesia, osteíte alveolar, fratura da mandíbula e danos aos
dentes adjacentes2.
O deslocamento acidental de raízes fraturadas ou de dentes para os espaços anatômicos
adjacentes, como o seio maxilar, a fossa infratemporal (FI), o espaço submandibular, o espaço
faringeano lateral, ou o espaço pterigomandibular é um problema raramente relatado, porém com
complicações potencialmente severas 3,4.
Alguns autores relatam que a maior frequência de deslocamentos de terceiros molares
superiores para o interior de espaços anatômicos é principalmente para o seio maxilar3. Sendo
mais incomuns e com potenciais complicações os deslocamentos para a fossa temporal e
infratemporal1,5.
A fossa infratemporal é um espaço anatômico de formato irregular localizada entre a
superfície interna do zigoma e a superfície externa do osso temporal e asa maior do osso
esfenóide. Ela possui relação com estruturas importantes como os músculos pterigóideos
mediais, o terceiro ramo do nervo mandibular, o gânglio ótico, o nervo da corda do tímpano, a
artéria maxilar, e o plexo venoso pterigoideo. Devido ao possível dano a essas estruturas, é

Diante dos achados, foi estabelecido remoção cirúrgica sob anestesia geral, devido ao alto
grau de ansiedade da paciente. Foi confeccionado um retalho mucoperiosteal, com incisão
relaxante mais anterior. Após amplo descolamento e exposição do pilar zigomático realizou-se
divulsão com instrumento rombo permitindo a localização e remoção do dente 28 (Figura 3).
