complete information. Failure to do so will cause rebate disqualification.
3) Mail this form with the following proof of purchase of the specific product(s) to the

● Must include this mail-in rebate form.

● Copy of original invoice or receipt from Newegg.com or Newegg.ca.

● Must write Promo Code and Transaction ID on the back of your envelope.

● Must include original Manufacturer's SKU barcode label, which includes: Model #,

UPC code and Serial Number.

● Must include printout of the "Official Rebate Request" with Rebate Transaction ID
