Checklist

Please review and print this checklist to make certain you mail in the completed forms and documentation
necessary to complete your return.

Print Checklist

Did you make a photocopy of the front of your Social Security card or ITIN letter?

Did you print a copy and sign the tw o(2) Pow er of Attorney Forms? IIE Grantees, please download the printable
version of the Power of Attorney Form 2848 . (Please complete ONLY the sections on the form 2848 w ith the eight
(8) arrow s.) and download the printable version of the Power of Attorney Form 231. (Please complete ONLY
the sections on the form 231 w ith the tw o (2) arrow s.) FORM 231 MUST BE NOTARIZED. THE FORMS ARE NOT
AVAILABLE AFTER FINAL SUBMISSION OF THE QUESTIONNAIRE.
Did you describe any non-IIE grants, scholarships, fellow ships or stipends you received during calendar year 2010 from
a U.S. organization? We need the 1042s, W-2 or 1099-Misc forms you received and w hat the income w as used for. We
also need a photocopy of any aw ard letters or notifications you received describing this income.
Did you provide a photocopy of your IIE Terms of Appointment? (If available - it is a letter that details your IIE
scholarship/fellow ship - does not apply to LASPAU grantees.)
REMEMBER: IF YOU RECEIVED ANY OF THE FOLLOWING TAX FORMS, PLEASE SEND US THE ORIGINAL
AND KEEP A PHOTOCOPY FOR YOUR RECORDS:

FORM W-2 : Show s w ages earned and taxes w ithheld from pay.
FORM 1099-INT : Show s bank interest earned (banks are not required to issue this form to nonresidents).
FORM 1099-DIV : Show s dividends earned.
FORM 1099-MISC : Show s miscellaneous income received (please note that if federal and state taxes w ere not
w ithheld from your 2010 miscellaneous income, IIE w ill not pay the late filing interest and penalties assessments).
FORM 1099-B : Show s stocks sold (if more than one stock w as sold, please provide a spreadsheet show ing dates
sold, dates purchased, sale prices and purchase prices).
FORM 1042S : Show s scholarships, grants, or w ages received.
FORM 1098T : Show s tuition and fees billed by your university and tuition and fees paid by you.
FORM 1098E : Show s interest you paid on a qualified U.S. student loan.

Please also send any tax documents not listed above that you may receive.

REMEMBER: DO NOT PREPARE OR FILE ANY 2010 TAX RETURNS YOURSELF NOW THAT YOU HAVE
ENROLLED IN GRANTAX. PLEASE CONTACT GRANTAX VIA FAX (212) 984-5553 OR VIA EMAIL AT
GRANTAX@IIE.ORG IF YOU HAVE ANY QUESTIONS OR HAVE CHANGED YOUR EMAIL OR MAILING
ADDRESS.

Mail all legible copies of your supporting/required documentation in ONE Package to
