• Housekeeping in the catwalk and mud pumps areas is not satisfactory as tools
observed left without proper storing. (OBS)
• Safety clamp of the high pressure hose is not installed properly
• A high pressure circulation hose found in operation on rig floor although it is in bad
condition having damaged outer layer and corroded metal reinforcements forming
hose burst hazard.
• The secondary retention of most of the mud system and mast flood lights is
incorrectly attached to the fixing frame; it should be attached to the body of the light.
• No inspection carried on nitrogen cylinders basket.
• Many mud agitators’ switches panels and junction boxes are sealed with silicon
forming fire hazard
• Three paint buckets found stored open in the paint store forming fire hazard
• There is no hazardous chemicals sign available in the chemicals storing area for
hazardous chemicals like soda ash.
• Cigarettes residuals observed thrown under the main camp skid.
• Recommended to install a steel mesh on the two bumper blocks to prevent the falling
of small wooden parts in case of travelling block collision
• No SWL on the mud pumps swing jib beams.
• Cleaning level in the galley and mess hall is not satisfactory.
• Pipes layers separation wires in the pipes boxes are bulging outside the basket and
have sharp edges forming struck-by hazard
• No designated area for keeping non-conforming drilling stems components
• Exhaust fan inside the catering store missing the guard forming hand injury hazard
• No earth cable connected for Baker unit.
• Wide openings observed in the rig floor, beside the hydraulic winch and under the
stand pipe manifold, are forming a dropped object hazard, covers need to be installed.
• Random check of 30 min breathing apparatus & Elsa escapes was performed and
found in good storing conditions. (NWE)
• Many caravans’ ladders are missing forming tripping hazard e.g. mess hall
emergency exit, doctor caravan and camp store.
• Switches board is not identified forming user confusion
• Exhaust fan motor is greasy forming fire hazard
• Fire drill at camp site was not regularly performed as no drill performed since
27.08.2010
• Some pipes observed racked on two pipes on the ground without stoppers forming
struck-by hazard.
• The gap between the kicker plates of the hand rails and the rig floor is too wide which
allows dropped tools, bolts and nuts.
• Safety relief valves of the air receivers are due for calibration as they were last
calibrated in June 2007.
• Safety flashes are not clearly mentioned in safety meetings
• Engine # 2 fan belts are not guarded forming hand injury hazard.
• Safety relief discharge lines for mud pumps # 1 & 3 are not sloping downwards to
mud tanks which could cause blockages due to solidified mud.
• Many chemicals bags observed scattered and damaged inside the chemicals
storing room.
