I. INTRODUCTION Background of the Study
Osteomyelitis is a severe pyogenic infection of the bone and surrounding tissues
thatrequires immediate treatment. Though the bone is affected site, normally ro
ot cause of such infection is not injuries caused to bone. Most often the infec
tion is originated fromsome other body parts and is spread to bone through blood
stream. If the bone hasundergone trauma recently, such bone becomes predispose
d to the infection. It is abone inflammation caused by bacteria. Generally there
are three routes where abacterium enters the body such as in the bloodstream or
hematogenous spread,adjacent soft tissue infection or contiguous focus and dire
ct introduction of microorganisms. Chronic Osteomyelitis occurs due to loss of b
lood supply to bone.Such loss of blood supply is caused when bone tissues dies.
As is suggestive from thenomenclature µchronic¶ (which means prolonged), the Osteomye
litis conditions persistsfor many years. People with diabetes, hemodialysis, pe
ople who have suffered traumarecently and people who abuse IV drugs are consider
ed on higher risk of developingchronic Osteomyelitis. Fortunately, it is a rare
health condition. According to theavailable statistics, one person among 5000
people gets affected with Osteomyelitis.
Rationale for Choosing the Case
The group decided to choose this case because it is interesting, rare and newly
encountered health crisis experienced by a patient confined in the hospital wher
e theyhad two weeks exposure. The case study gives the story behind the disease
process bycapturing what happened to bring it about and can be a good opportunit
y to plan for actions in order to treat or prevent it from happening to healthy
individuals. It is a veryspecial case caused by Pseudomonas aeruginosa, the vers
atile "blue-green pus bacteria" that opportunistically infects people, especiall
y those who areimmunocompromised.

. OBJECTIVES OF THE STUDY

The primary objective of this case study is to contribute to improved globalheal
th by providing necessary knowledge, assessment or findings,diagnostics and prev
ention about a type of pseudomonas infection whichis Osteomyelitis.

This study will help the nursing profession by providing information aboutthe pr
oper management and care for pseudomonas infected person.
y
It will also educate the people, especially those infected with a type of bacter
ium called Pseudomonas aeruginosa to seek medical care in order to prevent furth
er complications.
y
It will increase awareness about the importance of having a healthylifestyle, di
et and exercise.
y
This study will elaborate the inter relatedness of environment, life stylehabits
and an attack of bacteria causing chronic or acute osteomyelitisand other pseud
omonas infections.

This study aims to convey the patient and significant others withinformations ab
out the disease process, medications, treatment, diet andits course of medical a
nd nursing interventions.
y
To assess patient thoroughly, identify health problems, plan care,application of
planned activities and evaluate its effect to the patient.

To present the case study to the group members and clinical instructorsthen eval
uate and learn the outcome of the study to the patient and to thestudents.

To understand the course and essence of the chosen care study and addup addition
al knowledge and understanding in the nursing profession.

Limitations

This study is not limited to patients with osteomyelitis alone, but it is for al
lpeople who are interested in pseudomonas infections. We are more focused

on primary prevention through health education because primary preventionis the
true prevention.

This study in general with limited background and skills of students to care for
the patient and problem identified carry through the process of referral to the
clinical instructor, and doctor¶s orders during the 36 hours duty in PuertoCommunit
y Hospital. Interventions were rendered gradually depending on theobjective asse
ssment of the students. The following information only involvesthe exact words a
nd answers supported by the client and significant others.

B. HISTORY OF PRESENT ILLNESS:
A case of patient Mateo 73 years old, married and a roman catholic,residing at V
illanueva, Misamis Oriental was admitted for the first time at PuertoCommunity H
ospital on November 17, 2010 at 2:45 PM under Dr. Peralta with thechief complain
t of unhealed right leg wound. 2 years prior to admission, patientsustained an i
njury to his right leg from an accidental fall. The injury sustainedwas a small
wound which was not treated immediately due to lack of knowledgeof how serious i
t was. The client then observed the wound character is not healingover time and
discharges accumulated prompting him to seek medical advice.Upon consultation he
was given Bacrooban ointment as daily treatment for thewound. Two years later,
the resistance of the wound from medications even withpatient¶s compliance to medic
ation persisted and then subsequently rushed toPuerto Community Hospital for fur
ther evaluation and management.

Sigmund Freud (1856-1939) is probably the most well known theorist when it comes
tothe development of personality. Freud¶s Stages of Psychosexual Development are,
likeother stage theories, completed in a predetermined sequence and can result i
n either successful completion or a healthy personality or can result in failure
, leading to anunhealthy personality. This stage is marked by a renewed sexual i
nterest and desire,and the pursuit of relationships. This stage does not cause a
ny fixation. According toFreud, if people experience difficulties at this stage,

ed person. It is duringthis time that we contemplate our accomplishments and are
able to develop integrity if we see ourselves as leading a successful life. If
we see our life as unproductive, or feelthat we did not accomplish our life goal
s, we become dissatisfied with life and developdespair, often leading to depress
ion and hopelessness.
