rida since 1995. The company medically manages more than 200,000 lives. In addit
ion to the Disease/Population Management programs, the company offers TPA servic
es to more than 10 Healthcare organizations in Florida.
* As director of the Population Management Department, I developed and implement
ed a cost effective population/disease management program. The program resulted
in a 14% savings to Florida Medicaid between 2003 and 2008 (AHCA 2009 evaluation
) and quality improvement as demonstrated by the HEDIS indicators between 2004 a
nd 2009. Improvements included at least 35% increase in HbA1c for diabetic patie
nts; a 28% improvement in proper medication management for asthma patients.
* The utilization of the Advanced Data Management System was fundamental to the
improvement of the quality of care for patients categorized by the system as hig
h and moderate risk groups. This clinical, quality and financial software empowe
rs Primary Care Providers with patients' information and doctor's performance fe
edback.
* Successfully customized the Advance Data Management System for Pediatric Assoc
iates for the clinical management of the company's pediatric Medicaid contracted
population in 2009.
