those words to yourself or your

pay close attention.
Are you nearing or in
retirement and dreaming
about the good old days,
when interest rates were
something that added-up
and could actually allow your
account value to grow or pro-
duce a level of income that
you could live on for the rest
of your life? Or, maybe the
stock market has taken its toll
on your portfolio coupled
with, abysmally low interest
rates, and bond yields making
your retirement years a fearful

When CD interest rates,
during the Carter years, were
as high as 18%, did you beg,

here is no pain quite like
it. Once experienced,
most people never forget
the pain of a rib being out.
Acute rib pain is typically felt as
a savagely sharp, debilitating
pain just to one side of the
spine and usually between the
shoulder blades. Exacerbated by
head, arm or torso movement,
the pain can be so severe breath-
ing is shallow and restricted, and
no position - lying, sitting or
standing - provides any signifi-
cant relief. Chronic rib pain,
though less severe and far less de-
bilitating, can also be a signifi-
cant problem for many people,
and is typically experienced as a
constant, deep, dull throb or
ache that persists despite exer-
cise and stretching. What are
the causes of rib pain, and how
is it best treated?
The rib cage is made up of
12 pairs of ribs and functions to
protect the lungs and heart. It
also serves as an attachment for
the diaphragm, making breath-
ing possible. The first 10 pairs
of ribs are attached to the
breastbone. The lowest two
pairs of ribs are called the
floating ribs because they
attach only to muscle at the
front. Moveable joints, like
those between the vertebrae,
attach the ribs to the spine,
enabling us to breath, twist and
turn. Occasionally, these cos-
tovertebral joints may lose
their normal movement or
become fixed in an abnormal
position. The result is rib pain,
acute if the joint becomes
extremely irritated or dull if
the irritation is minor.
Costovertebral joint dysfunc-
tion can also put strain on the
cartilage at the front of the rib
cage. Pain may be felt at the
junction between the rib and
the cartilage or between the
cartilage and the breast bone.
Most rib pain is due to two
main factors: chronic postural
faults and trauma. Traumatic
causes do not only involve
injury from accidents (ie: car
accidents, major falls or blows
to the back). Rib pain can also
result from microtrauma, such
as repetitive strain injury.
These underlying factors of
posture and trauma predispose
the spine and ribcage to irrita-
tion. The triggers which may
cause predisposed costoverte-
bral joints to become irritated
and painful typically include
reaching, twisting, minor falls,
or activities such as archery or
curling which use the middle
back and rib cage as a fulcrum.
Neck problems are often
another source of rib irritation,
usually the result of incorrect
head position (abnormal pos-
ture) or trauma.

atthew graduated with honors from New York
University in 1997 and began a satisfying career in
orthopedics specializing in spine rehabilitation. Ever
keen to learn and do more for his patients Matthew began
studying diligently for his certification in Orthopedic Manual

Matthew has also had the thrill of studying with world-
renowned therapists Brian Mulligan and David Butler, further

Matthew believes the combination of this training and

develop in people over age 55,
but they occasionally occur in
infants and young children.
As we become older, a chemi-
cal change within the eye
causes the lens to become
cloudy. However, other fac-
tors that may contribute to
cataracts are excessive expo-
sure to ultraviolet radiation
from sunlight, certain medica-
tions, and/or cigarette smok-
ing. Usually cataracts develop
in both eyes, but one may be
worse than the other.
A yearly eye exam is
important in detecting early
cataracts and is important in
monitoring the progression
for those who have already
been diagnosed. Polarized
sunglasses and wide brimmed
hats are important in helping
to prevent exposure to ultra-
violet light. Anti-reflective
coatings on glasses may help
those with cataracts who
experience glare at night. A
change in eyeglass prescrip-
tion may provide some
improvement in vision for
those with early cataracts.
Increasing the amount of
light used when reading may
also be beneficial.

cataracts are mature and
where it has affected their
quality of life, cataract sur-
gery would be an option.

ylitol is a sugar alcohol
that is commonly used
as an artificial sweet-
ener and is created from birch,
raspberries, plums and corn.
Xylitol is similar in sweetness
to sugar but half the calories
and without the common ef-
fects of sugar such as dental
cavities and diabetes. There-
fore, this sweetener is found in
many human sugar free prod-
ucts, such as gum, candies or
other sweets. In humans, this
additive has no serious side ef-
fects but in dogs, ingestion
could be fatal. Xylitol has also
been suspected of causing tox-
icity in ferrets.
Xylitol toxicity is one of
the newer culprits for toxic
episodes in our dogs. A com-
mon scenario is finding your
pooch stealing a pack of sugar
free gum. While previously it
was thought that only large
concentrations of xylitol could
result in problems, this may no
longer be the case. Just a few
sticks of gum ingested by a
small dog could cause severe
problems.
The main and initial
toxic event in dogs with xyli-
tol ingestion is hypoglycemia
(low blood sugar). What the
xylitol does is cause a rapid
release of the hormone
insulin, resulting in a sudden
decrease in blood glucose.
There also appears to be a
strong link between xylitol
ingestions and the develop-
ment of liver failure in dogs,
which can develop after the
initial toxic phase.
Signs of toxicity can be
seen as quickly as 30 minutes
after xylitol ingestion. You
may see clinical signs of hypo-
glycemia such as: vomiting,
weakness, ataxia or uncoordi-
nated movements, depres-
sion, seizures, coma, and later,
liver dysfunction and/or fail-
ure. Your dog could also see
problems from ingesting just
small amounts of xylitol.
Therefore do not base toxici-
ty on seeing clinical signs in a
half hour. You may not be in
the clear because with smaller
concentrations of xylitol, the
onset of clinical signs could
be delayed as much as 12
hours after ingestion.
Since these signs can
develop quite rapidly it is cru-
cial that pet owners seek vet-
erinary treatment immediate-
ly or contact the Animal
Poison Control Center. If it is
soon after your pet has eaten a
xylitol-containing sweet or
food and before clinical signs
develop, your veterinarian
may advise inducing vomiting
to expel the toxin and sup-
portive care to monitor and
treat the resultant low blood
glucose and possible low
potassium levels.
Prevention is very
important. Therefore be espe-
cially diligent in keeping
candy, gum or other foods
containing xylitol out of the
reach of pets. As with any
potentially toxic substance,
should accidental exposures
occur, be sure to contact your
veterinarian or Animal
Poison Control for immediate
assistance.

the clinic with parents present
in order to empower them to
carryover strategies to home.
Sessions are also offered as indi-
cated, in the home, the play-
ground, or other community
setting where her client and
family are having difficulty.
She maintains a policy of avail-
ability between sessions for
phone or e-mail contact from
parents. I am able to wait
before getting the opportunity
to discuss health issues on my
own behalf, however as a par-
ent I would become frustrated
having to wait until the follow-
ing week to discuss a concern
regarding my child, so I offer
this opportunity. Parents are
comforted having this option,

When I was going to
school to become an occupa-
tional therapist, I was frequent-
ly asked to describe what O.T.s
do. Now it seems it has
become widely recognized in
the general population.
Frommer continues to share,
The fact that so many chil-
dren nowadays require occupa-
tional therapy to support their
development raises concerns
for me as a mother and thera-
pist working with children.
Times have changed. We used
to hear sayings like: she never
crawled, but she walked early
so she must be advanced , hes
fine, he wont still be clumsy
when he goes to college, she
will grow out of it, he will
catch up, she will learn it in
school. Frommer feels that
these statements are not neces-
sarily true for our children of
present times, If something
seems not quite right we need
to investigate what the expec-
tations are for our little ones
and whether or not they are
meeting these demands. Early
detection and therapeutic
intervention will prevent sec-
ondary problems from arising
later on.
It is widely recognized
that school age children
encounter higher expectations
at an earlier age than previous
generations. What used to

r L is definitely one
of our more rowdy
patients from West
Milford. He likes to hunt,
ride his motorcycle and hang
out with the
boys boating
during the
w e e k e n d s .
The long week-
end of July 4 of
this year was no
different. Mr L
unfortunately
injured his right
shoulder during
a weekend of
boating. The
accident actually did not hap-
pen on the boat, but during a
playful wrestling match later
that evening when there were
plenty of flavorful beverages
flowing and much happiness
on the porch. My shoulder
just does not look right, doc,
Mr L said. On examination,
he clearly had a separation of
the acromioclavicular (AC)
joint, where the collar bone
joins the roof of the shoulder,
the acromion. More impor-
tantly, Mr L had obvious
drooping of his entire right
shoulder girdle and arm. His
collar bone had significant
superior displacement, or
sticking way up as Mr L said.
Fortunately, he had no other
injuries and the xrays con-
firmed an AC joint separation
of 3.5 cm.
The acromioclavicular
joint is an important stabilizer
for the shoulder girdle. There
are two important ligaments
that attach the clavicle (col-
larbone) to the shoulder
blade, called the trapezoid and
conoid. With an AC joint
separation as large as Mr Ls,
these important ligaments
have been torn and there is
little to keep the shoulder gir-
dle from drooping downwards
and the collarbone from shift-
ing upwards.
While smaller AC
joint separations
of 0.5 cm to 1 cm
often heal well
without further
i n t e r v e n t i o n ,
there is concern of
p r o g r e s s i v e l y
worsening shoul-
der deformity
with the larger
AC joint separa-
tions. As a right handed con-
struction worker, Mr L wanted
to proceed with operative
intervention.
There are numerous dif-
ferent techniques of repair of
AC joint separations. The
most common techniques that
we use at Western Janeda
Orthopedics are : the hook
plate with ligament transfer,
and the arthroscopic
tightrope. The advantage of
the arthroscopic tightrope is
that the entire operation is
performed with little stab
incisions and the patient has
significantly less pain after-
wards. With his presentation
to our office 6 days after injury
and an easily mobile clavicle,
Mr L was a good candidate for
the arthroscopic tightrope.
Five days after seeing him in
the office, Mr L was in the
operating room. The opera-
tion was performed using a 70
degree telescope that we
introduced into the shoulder.
Using precision guides that
reproduce exact bony walls
correct to the millimeter, a
drill hole was introduced
across the clavicle and
through the coracoid (a bony
process from the scapula).
Next, the repair construct
made of a very strong sub-
stance called fiberwire with
metallic fixation buttons at
the ends was introduced
across the clavicle and cora-
coid. With the AC joint in its
anatomic position as verified
by xrays at the time of the
operation, we proceeded to tie
down the fiberwire. The
result was a reconstruction of
the torn ligaments, the conoid
and the trapezoid.
After surgery, the most
difficult patients are the most
active patients such as Mr L
and the college athletes.
Especially after the arthro-
scopic tightrope operation,
there is typically not that
much pain after 2 or 3 weeks
and patients want to prema-
turely return to all activities.
We actually recommend at
least 6 weeks for a chance for
the repair to scar down. For
Mr L and the next 6 weeks, it
means : happiness on the
porch is in, boating is out, and
at most 1 or 2 flavorful bever-
ages per evening.

wherever gas came into con-
tact with the yard. But even
the smallest of spills can
cause the gas to evaporate
and pollute the air. When
refilling the gas tank, do so
on the driveway and not in
the yard to avoid damaging
the property. Use a gasoline
container thats easy to han-
dle and use a funnel or spout
to reduce the likelihood of
spillage. Once the tank has
been filled, make sure the
caps on both the power
equipment and the gasoline
container have been closed
tightly, and store the con-
tainer out of the sunlight,
ideally in a cool, dry place.
Go manual. Some lawn
maintenance requires the use
of gas-powered tools to get
the job done in a timely fash-
ion. Homeowners with large
yards, for instance, will likely
need a gas-powered mower to
cut their grass in a reasonable
amount of time. However, in
lieu of gas-powered tools to
trim hedges and trees around
the yard use a traditional
manual hedge clipper
instead. Many of todays
manual hand tools are
designed to be user-friendly
and, as a result, are much
lighter and less bulky than
the tools of yesteryear.
Homeowners with smaller
yards might even consider
using reel push mowers that
dont generate any emissions
whatsoever.
Cut down on cutting.
Instead of cutting the grass
every weekend, visit the
local lawn and garden center
and ask if there is any grass
and flower seed mixtures that
will make the grass grow
more slowly. This often
depends on the region and
the type of grass that thrives
in the region, but it can help
reduce the number of times
the lawn needs to be cut and
reduce emissions as a result.
Plant more trees. Planting
more trees around the prop-
erty can be advantageous for
a number of reasons. If the
yard has more trees, that
mean it will have less grass,
decreasing the amount of
time it takes to mow the
yard, and thereby reducing
emissions from the mower.
Another eco-friendly benefit
to planting more trees is that,
if planted strategically to
provide shade, energy usage,
in addition to cooling costs,
during the hot summer will
decrease. Homeowners who
plant native wildflowers and
plants also can reduce the
time it takes to maintain
their yard, as such plants typ-
ically require little to no
maintenance after planting.
Dont allow equipment to
deteriorate. Just like an auto-
mobile, gas-powered mainte-
nance tools work more effi-
ciently when theyre well
maintained. If filters arent
replaced or machines are
allowed to fall into disrepair,
they wont operate with
much efficiency, meaning
theyll burn more gasoline
and produce more emissions.
In addition to the environ-
mental benefits of maintain-
ing equipment, homeowners
can also save money by tak-
ing care of their equipment,
increasing the tools life
expectancy while improving
fuel efficiency.

At Kids Therapy Center,
we are always working toward
discharge. The therapist and
parents work together to cre-
ate short and long term goals
for the child. Each month a
set of short term goals are
selected. As the short term
goals are achieved, the child
advances toward reaching his
long term goals. Finally, when
the long term goals are all
achieved, the child will be dis-
charged.
Gena Livingston is a pediatric
physical therapist, who brings
over five years of experience to
Kids Therapy Center of

Serves 4
Ingredients:
12 ounces Santa Barbara
Bay(R) All Natural Greek
Yogurt Dip, Spinach
1 cup fresh bread crumbs
cup grated parmesan
cheese
1 cup chopped celery
1 cup diced red onion
1 head roasted garlic
4 pork chops
Salt and pepper to taste
Directions: Preheat oven to
350 F. In a large bowl, combine
spinach dip, bread crumbs,
cheese, celery, red onion and
roasted garlic. Slice each pork
chop horizontally halfway
through, creating a pocket for
the stuffing mixture. Stuff each
pocket with of the stuffing.
Place stuffed chops in a pre-
pared baking pan and place in
preheated oven for 15 to 17
minutes, or until internal tem-
perature reaches 160 F.
Remove from oven and allow
to rest for 10 minutes.

Serves 4
Ingredients for dressing:
12 ounces Santa Barbara
Bay(R) All Natural Greek
Yogurt Dip, Zesty Ranch
1 cup red wine vinegar
1 cup olive oil
Ingredients for salad:
1 cup tomatoes, diced
1 cup pitted premium olives
of choice
1 cup lettuce, shredded
1 large avocado, diced
1 cup bacon, chopped
1 cup carrots, shredded
1 cup feta cheese crumbles
1 cup chick peas
4 hard boiled eggs, chopped
Directions for dressing: In a
blender or with an immersion
stick blender, mix ranch dip
with vinegar and oil to create
Greek Ranch vinaigrette.
Directions for salad: Assemble
chopped ingredients in rows
across dinner plates and serve
with 2 ounces of homemade
vinaigrette.

work, its time to enjoy your-
