Client: AYALA LAND INCORPORATION Period Start: October 7, 2010 to present DUTIES AND RESPONSIBILITIES: Provide support, advice, co-ordinate and supervision of HSE aspects to construction authorities like project manager, engineers, foreman, supervisors in the safe operation of all construction activities so as to remove the risk of accidents/incidents & near misses. Supervise and handling number of Jr. HSE officers as per project base and provide HSE support to construction team to ensure personnel are fully aware of client and own HSE policies as well as the local legislation/procedures by Philippine regulations or codes of the project. And Respond to inquiries or complaints from clients, regulatory agencies or community groups. Coordinating with to Project Manager any safety issues safes and safety recommendation for the site safety/order. Interface with the construction team members responsible for the permitting, QC/QA and construction on daily basis to ensure that construction are performing to HSE requirements, planning ahead these individuals to anticipate and avoid problems.

Plan and execute field drills in conjunction with client to ensure that all construction personnel are familiar with the emergency procedure. Conduct safety orientation/induction on new employees, in charge with their security gate pass, and coordinate with physician in filling medical forms if they are fit to work. Ensure and Prepare daily and monthly report that timely preparation and submission through e-mail of required company records, maintained and in all times available to company Safety manager. Such as up to date HSE Safety alerts ,HSE information, Statistics & Incident, manpower report, total monthly manpower report, and attending HSE monthly meeting and HSE emergency meetings at main office. Participate in the internal safety audit program by conducting system safety audits. Plans inspection of system of work provided and work place precautions.1.access and egress, working at height, contingency plans.2.electrical equipments.3. Plant equipment ex. cranes, man lift, dump trucks etc., portable power tools, hydraulic power tools.4. Inspection of fire extinguishers, Checking of scaffolding, excavations and rigging and other construction related job. Responsible for the daily toolbox topics, participate in Client weekly safety walk through and weekly meetings wherein responding through their inspection audit and concerns. Plan and administer construction management services safety committees by chairing weekly safety committee meetings, developing meeting agendas, writing meeting minutes, and conducting follow-up on safety-related issues by tracking, reporting, and resolving. Coordinate and administer a continuing program of safety information and perform trainings like safe using of power hand tools, training of concrete crew, rigging, scaffolding etc. Conduct safety and hygiene inspections, issue corrective action lists and follow up on the close out this action. Providing advice and whenever necessary hands on support to the line in such activities monthly as job hazard analysis, risk assessment and incident investigation and analysis. Initiate, co-ordinate and record emergency drill, and make recommendations for the prevent of reoccurrence. Study and understand methods of work on every activity, materials and terminology of building construction trade and interpret bldg. construction drawings, plans and specification. 2. First project assigned: JUBAIL CHEVRON PHILLIPS K.S.A. * A joint venture between Saudi Industrial Investment Group & Arabain Chevron Phillips Petrochemical
Company Limited. Worley Parsons was the FEED Contractor and PMT partner w/ Arabian chevron Phillips. The scope includes a unique patented ethylene unit, the worlds largest single train ethyl benzene & styrene monomer unit w/ various supporting utilities such as steam boilers, air compressors, waste treatment facilities and storage tanks. MANAGING CONTRACTOR: JAPAN GAS CONSTRUCTION (JGC-Arabia) SUBCONTRACTOR : CONSOLIDATED CONTRACTORS COMPANY(C.C.C.)

DUTIES AND RESPONSIBILITIES: Teamwork with HSE Team, implement HSE policies, training plan and HSE Management System Requirement that confirm w/ ISO 14001:2004 and OHSAS18001:1999. Supervise/Advice site on achieving HSE Standards and Requirements in accordance with Company and Client regulations Participate/conduct daily toolbox meeting as per area, assists workers in weekly toolbox talk or deliver weekly toolbox talk as scheduled by the HSE supervisor and Participate/adv in the weekly ice concerned authority their HSE toolbox meeting by Project Manager or Construction Manager only for dept. heads/superintendents, engineers, supervisors, foreman, charge hands. Key player in the full implementation of the Control of Work Standard including the valid permit to work System and its related risk assessment for each work w/in the Project/Construction Site. Ensuring permit issuer and receiver have a valid work permit & conducted site inspection before commencing work. Assists in Induction of new employee and one day Safe Trainings like work at heights, proper use of PPE for the right job, barricades and signages, moving equipments etc. to promote safety awareness to workforce with right information and instruction carry out sound method of work. Carry out regular hazard identification on unsafe act and unsafe condition and any safety concern to relevant supervisor for action and carry out follow up to ensure close out and daily report/submit to HSE supervisor inspection checklists including fire fighting equipments designated at site and used for a certain job Participate on joint weekly assessment or walk through inspection on site with client, line management and note observed discrepancies to be rectified, note good practices and bad practices observed and after to be discussed in minutes of meeting. Ensuring daily inspection checklist of authorized operators/workers on their equipment/tools like the crane operators, welders, fabricators, scaffolders, pre-commissioning people etc. Ensuring the inspection and color codes every month by concerned authority of lifting equipments, electrical equipments, electrical hand power tools, pneumatic tools, welding and cutting equipments, portable fire extinguishers, ladders, full body harness etc. Ensuring that all cranes and loose gear are audited to Kingdom of Saudi Arabia, Client standard or approved contractor standards, in order to verify safe working condition at the various locations where work is carried out for the project. Supervise High Hazard Activities like steam blowing, hydro testing, pneumatic testing, critical/heavy lift, deep excavations, structural erection, scaffolding erection & dismantling, piping erection, piping & structural welding, concreting activities, etc. Given the task in shifting schedule for 4 month to supervise a night shift work at Ethylene Area(Chevron Phillips project) and some part of Main Pipe Rock and Flare Area the work involve mainly on welding jobs ensuring safe work area conditions relaying to supervisor involve to give action or rectification, acts as an assistant/coordinate to client HSE officer for the night work on Steam Blowing, hydro tests, pneumatic testing, pipe cleaning and NDT work and daily checking noise level using noise level monitor in different affected areas during steam blowing.(precommissioning phase). Coordinate and ensure compliance by the concerned discipline to approved procedures with regards to safety requirements during testing (NDT, Hydro test, Pneumatic, PWHT, Pre-commissioning of a unit and others as per projects requirements or specifications), daily checking noise level using noise level monitor in different affected areas during steam blowing.(pre-commissioning phase) and Performs gas tests as required on hazardous confined space on pipes, tanks, drums, cracking furnace, vessels, excavation etc. before executing work. Conduct investigation, identifying the root cause and report any cause of accident, incident and near misses to recommends means of preventing recurrence of such incident/accidents and after follow up done Participate every month in Emergency drills and emergency response team acted upon in timely manner.
