Declaro para os devidos fins que estou ciente dos seguintes itens:

• Observar e cumprir estrita e rigorosamente tudo o que for determinado durante o aprendizado,
inclusive no que diz respeito aos horários de aulas práticas o que devo chegar com antecedência
minima de 20 minutos do horário marcado para inicio. Havendo atraso, a aula não será reposta;
• Só será possível a desmarcação das aulas práticas com antecedência minima de 24 horas, caso
contrário, o aluno terá que pagar uma nova aula (R$80,00). Para a demarcação das aulas na segunda
feira, o prazo será até 12 horas do sábado que antecedem;
• Será obrigatório o porte da LADV durante as aulas práticas;
• É obrigatório a utilização de calçado adequado para realização das aulas;
• É proibido á presença de um terceiro ocupante no veículo, além do instrutor e aluno;
• É obrigatório que o aluno traga seu próprio capacete para suas aulas de moto (por questões de
higiene), caso esqueça, não será possível realizar a aula.
