Implementing Guidelines for Medical Examinations Requirement

1. All field personnel must avail the Basic 5 Package (Physical Examination,
Chest X-ray, CBC, Urinalysis and Fecalysis) as part of their Annual Medical Examintaion/Pre-Employment Examination at a rate of Php________ in any of the following clinics: FriendlyCare Clinic Addresses 710 Shaw Boulevard, Mandaluyong City - 2/F Metrolane Complex, P.Tuazon cor. 20th Ave., Cubao, Quezon City - G/F Silicone Bldg., 168 Sumulong Hi-way, Brgy. 2. All field personnel are also entitled to a Bonus Family Planning Counseling
during P.E.E./A.M.E. and Ten Percent (10%) discount on all laboratory and disgnostic services at any FriendlyCare Clinics. 3. In order to avail these services, all examinees should present a duly signed referral or authorization slip and a valid Company I.D. 4. All regular field employees will be examined and charged to their companies/agencies the actual cost of the medical examinations conducted which will be later on deducted from their salaries. 5. Applicants and new hirees can also avail the said medical examination package rate but they have to pay in cash directly at BDMSC.

6. BDMSC will be billed every 15th and 30th of every month and payment should
be made within fifteen (15) days after receipt of the Statement of Account. 7. All laboratory results should only be submitted by the clinics authorized representative to BDMSC and should be treated confidential. Turn-around time of results: 24 hours - for applicants/new hirees 3 days - regular employees 3 days - on site testing Note: Applicants/new hirees can claim their own results to avoid delays. 8. All affiliated clinics as well as BDMSC should abide by the standards set by Department of Health in rating our employees. These is classified into four:

9. The Information Support Group shall provide a master list of the personnel
scheduled for examination in a monthly basis at least two (2) weeks before the end of every month.

10. BDMSC will coordinate with the clinic and Operation (agency) involved in
cases of on-site testing to ensure that all the needed requirements are met (manpower, electric and water supply, venue).
