Kelainan degeneratif, sejalan dengan perubahan pola hidup dan pola makan penduduk yang cenderung berubah menjadi faktor risiko penyakit-penyakit degeneratif, baik di negara yang sedang berkembang seperti Indoonesia maupun di negara-negara maju maka menjadi sering dijumpai. Aterosklerosis Aterosklerosis adalah kelainan degeneratif pada sistem kardiovaskuler yang mengenai arteri besar dan sedang, dimana terjadi penimbunan lemak dan jaringan fibrosis, sehingga akan timbul penimbunan berupa plak ateroma di pembuluh darah yang mengalami kelainan tersebut. Karena adanya plak aterom ini maka akan terjadi penyempitan lumen pembuluh darah, dimana penyempitan ini,

biasanya tidak hanya karena sumbatan atau timbunan plak, namun biasanya ditambahi karena adanya gangguan elastisitas dari dinding pembuluh darah. Adanya plak ini bila mengalami kerontokan atau terlepasnya plak ateroma dari dinding pembuluh darah, dimana plak ini menempel, maka rontokan plak ini akan mengakibatkan penyumbatan arteri sebelah distal. Karena adanya penyumbatan arteri koronaria yang mendarahi jantung ini, maka akan mengakibatkan keadaan iskemia jantung (Penyakit Jantung Koroner). Bila keadaan serupa mengenai

pembuluh darah serebral di otak maka akan menyebabkan keadaan iskemia di otak, yaitu kelainannya disebut sebagai Stroke Non Perdarahan dan bila keadaan ini terjadi di jaringan ginjal, yaitu di parenkhim atau pada jaringan glomerulus dan tubulus maka akan terjadi Nekrosis Ginjal atau nekrosis tubuler yang biasanya bersifat kronik. Kelainan degeneratif biasanya jarang terjadi pada umur kurang dari 40 tahun, dan akan bertambah sejalan sesuai dengan bertambahnya umur. Pada usia di atas 70 tahun kelainan degeneratif merupakan kasus yang paling banyak dijumpai. Faktor risiko kelainan degeneratif dari aterosklerosis adalah : wanita post menopause, penyakit hipertensi, dislipidemi, yang ditandai dengan kenaikan kadar kolesterol (LDL + trigliserida) dan atau rendahnya kadar HDL. diabetes melitus, obesitas, gaya hidup dengan pola konsumsi energi berlebih dan kurang olah raga (aktivitas fisik).

Hiperlipidemia Gangguan metabolisme lemak yang menimbulkan peningkatan kadar lemak darah disebut sebagai hiperlipidemia.

Klasifikasi hiperlipidemia : Ada 2 jenis hiperlipidemia ditinjau dari sebabnya, yaitu ; hiperlipidemia primer

hiperlipidemia sekunder

Hiperlipidemia primer Ialah keadaan peningkatan kadar lemak darah yang tidak ada hubungannya dengan penyakit lain. Meleinkan herediter (genetik)

Hiperlipidemia sekunder Hampir 40% gangguan metabolisme lemak yang ditandai dengan peningkatan kadar lemak merupakan hiperlipidemia sekunder. Yang sering menimbulkan hiperlipidemia sekunder : obesitas alkoholisme (peminum alkohol) gangguan ginjal gangguan hati diabetes melitus iatrogenik (obat-obatan ; pil anti hamil, kortikosteroid jangka panjang) hipotiroidisme feokromositoma

Gejala hiperlipidemia : Hiperlipidemia tidak menimbulkan gejala apapun sampai terjadi komplikasi penyakit jantung koroner yang melanjut sebagai infark miokard. Diagnosis hiperlipidemia lebih banyak ditemukan saat pemeriksaan rutin atau pada waktu cek-up sebagai syarat peserta asuransi. Jenis sekunder biasanya ditemukan setelah diketahui penyakit primernya. Hipertensi

Hipertensi adalah keadaan dimana tekanan darah sistolik dan atau diastolik mengalami peningkatana dari angka atau tekanan batas normal. Batasan normal untuk tekanan sistolik pada orang dewasa adalah 140 160 mmHg dan diastolik adalah 90 95 mmHg. Hipertensi sendiri merupakan faktor risiko dari berbagai penyakit degeneratif, misalnya gagal jantung, aterosklerosis dengan segala akibat, penyakit stroke, penyakit ginjal, penyakit mata dan berbagai penyakit lain. Menurut manifestasi klinik tersebut hipertensi dibedakan menjadi hipertensi benigna dan hipertensi maligna. Dikatakan hipertensi benigna bila tekanan darah mengalami peningkatan akan tetapi peningakatan tersebut tidak melebihi 200 mmHg untuk tekanan sistolik dan 120 mmHg untuk tekanan diastoliknya. Sedangkan hipertensi maligna adalah suatu keadaan sebaliknya dari hipertensi benigna, yaitu tekanan diastolik maupun sistolik di atas yang tersebut di atas, ataupun tekanannya kurang dari tersebut di atas (120 mmHg (diastolik)/ 200 mmHg (sistolik)) namun telah terjadi gangguan atau kelainan pada organ target. Yang disebut dengan organ target adalah organ vital yang mengalami gangguan oleh karena keadaan tekanan darah yang abnormal tinggi tersebut. Organ-organ tersebut diantaranya jantung, ginjal, mata, dan otak. Kelainan atau gangguan yang muncul pada organ target tersebut akan sangat bergantung pada organ yang terganggu dan derajat berat ringannya keadaan hipertensi serta faktor-faktor risiko lainnya. Secara umum biasanya pada jantung akan mengalami serangan infark/ iskemia jantung, pada ginjal juga mengalami keadaan iskemia sampai dengan nekrosis tubulus/ glomerulus ginjal. Sedangkan pada mata biasanya akan terjadi gangguan atau kerusakan permanen dari retina. Pada mata ini juga merupakan suatu tempat dimana dapat dilihat seberapa berat hipertensi dapat mempengaruhi keseimbanan organ-organ lain, karena di mata (retina) bila dilihat dengan funduskopi akan terlihat adanya pola atau gambaran sklerotik pada pembuluh darah mikro yang bisa terjadi akibat keadaan hipertensi ataupun keadaan iskemia akibat perfusi yang tidak adekuat. Gambaran kelainan

retina ini bila disebabkan oleh keadaan hipertensi maka disebut sebagai retinopati hipertensi.

Etiologi / Penyebab Hipertensi secara causatif atau bila dilihat penyebabnya dapat dibedakan menjadi hipertensi primer (esensial) dan hipertensi sekunder Hipertensi primer merupakan kasus hipertensi yang sering dijumpai, tidak diketahui penyebabnya secara pasti. Namun demikian beberapa faktor yang diketahui berhubungan atau menjadi faktor risiko dan mempengaruhi kejadian hipertensi primer adalah : genetik (keturunan), kegemukan (obesitas), konsumsi makanan yang tidak seimbang dengan diit garam berlebihan, pola hidup tidak tenang, yaitu dengan kepribadian tipe A (ambisius, sering mengerjakan sesuatu dengan cepat dan dalam satu waktu ingin mengerjakan beberapa hal sekaligus) serta aktivitas fisik yang berlebihan atau bahkan sebaliknya yaitu keadaan kurangnya olahraga. Hipertensi sekunder adalah hipertensi yang disebabkan adanya kelainan atau keadaan dari sistem organ lain, seperti kelainan ginjal, yaitu : Gagal Ginjal Kronik, Glomerulonefritis Akut, Kelainan Endokrin : Tumor Kelenjar Adrenal, Sindroma Cushing, serta bisa juga diakibatkan oleh penggunaan obat obatan : kortikosteroid dan hormonal (pil, suntik dan susuk kontrasepsi. ---------------------------------------------------------------------------------------------------------------------------------------------------------1.1

Overview
Blood is pumped from the left ventricle of the heart to capillaries in the periphery via the arterial vessels of the systemic (or greater) circulation and returns via the veins to the right heart. It is then expelled from the right ventricle to the lungs via the pulmonary (or lesser) circulation and returns to the left heart (!A). The total Blood volume is roughly 45 L (!7% of the fat-free body mass; !table on p. 88). Around 80% of the blood circulates through the veins, right heart and pulmonary

vessels, which are jointly referred to as the low pressure system (!A, left). These highly distensible capacitance vessels function as a blood reservoir in which blood is stored and released as needed via venous vasoconstriction (!e.g., p. 218). When the blood volume increases due, for example, to a blood transfusionover 99% of the transfused volume remains in the low-pressure system (high capacitance), while only !1% circulates in the arterial high-pressure system (low capacitance). Conversely, a decrease will be reflected almost entirely by a decrease in the blood stores in the lowpressure system. Central venous pressure (measured in or near to the right atrium; normally 412 cmH2O) is therefore a good indicator of blood volume (and ECF volume) in individuals with a normally functioning heart and lungs. Cardiac output (CO). The cardiac output is calculated as heart rate (HR) times stroke volume (SV). Under normal resting conditions, the CO is approx. 70 [min1] " 0.08 [L] = 5.6 L/ min or, more precisely, a mean 3.4 L/min per m2 body surface area. An increase in HR (up to about 180 min1) and/or SV can increase the CO to 1520 L/min. The distribution of blood to the organs arranged in parallel in the systemic circulation (!A, Q . values) is determined by their functional priority (vital organs) and by the current needs of the body (see also p. 213 A). Maintaining adequate cerebral perfusion (approx. 13% of the resting CO) is the top priority, not only because the brain is a major vital organ, but also because it is very susceptible to hypoxic damage (!p. 130). Myocardial perfusion via coronary arteries (approx. 4% of the CO at rest) must also be maintained, because any disruption of cardiac pumping function will endanger the entire circulation. About 20 to 25% of the CO is distributed to the kidneys. This fraction is very large relative to the kidney weight (only 0.5% of body mass). Renal blood flow is primarily used to maintain renal excretory and control functions. Thus, renal blood flow may be reduced transiently in favor of cardiac and cerebral perfusion, e.g., toward off impending shock (!p. 218). During strenuous physical exercise, the CO increases and is alloted mainly to the skeletal muscle. During digestion, the gastrointestinal tract also receives a relatively high fraction of the CO. Naturally, both of these organ groups cannot receive the maximum blood supply at the same time (!p. 75 A). Blood flow to the skin (approx. 10% of the resting CO) mainly serves the purpose of heat disposal (!p. 222ff.). The cutaneous blood flow rises in response to increased heat production (physical work) and/ or high external temperatures and decreases (pallor) in favor of vital organs in certain situations (e.g., shock; !p. 218). The total CO flows through the pulmonary circulation as it and the systemic circulation are arranged in series (!A). Oxygen-depleted (venous) blood is carried via the pulmonary arteries to the lungs, where it is oxygenated or arterialized. A relatively small quantity of additional oxygenated blood from the systemic circulation reaches the lung tissue via the bronchial arteries. All blood in the pulmonary circulation drains via the pulmonary veins. Peripheral resistance. Flowresistance in the pulmonary circulation is only about 10% of the total peripheral resistance (TPR) in the systemic circulation. Consequently, the mean pressure in the right ventricle (approx. 15 mmHg =

2 kPa) is considerably lower than in the left ventricle (100mmHg = 13.3 kPa). Since the resistance in the lesser arteries and arterioles amounts to nearly 50% of TPR (!A, top right), they are called resistance vessels.

Hypertension

Hypertension is defined as a chronic increase in the systemic arterial blood pressure. The general criterion for diagnosis of hypertension is consistent elevation of resting blood pressure to more than 90mmHg diastolic (!p. 206). Untreated or inadequately managed hypertension results in stress and compensatory hypertrophy of the left ventricle which can ultimately progress to left heart failure. Individuals with hypertension are also at risk for arteriosclerosis and its sequelae (myocardial infarction, stroke, renal damage, etc.). Therefore, hypertension considerably shortens the life expectancy of a large fraction of the population.
The main causes of hypertension are (a) increased extracellular fluid (ECF) volume with increased venous return and therefore increased cardiac output (volume hypertension) and (b) increased total peripheral resistance (resistance hypertension). As hypertension always leads to vascular changes resulting in increased peripheral resistance, type a hypertension eventually proceeds to type b which, regardless of how it started, ends in a vicious circle. The ECF volume increases when more NaCl (and water) is absorbed than excreted. The usually high intake of dietary salt may therefore play a role in the development of essential hypertension (primary hypertension), the most common type of hypertension,

at least in patients sensitive to salt. Volume hypertension can even occur when a relatively low salt intake can no longer be balanced. This can occur in renal insufficiency or when an adrenocortical tumor produces uncontrolled amounts of aldosterone, resulting in Na+ retention. Other important cause of hypertension is pheochromocytoma, a tumor that secretes epinephrine and norepinephrine and therefore raises the CO and TPR. Renal hypertension can occur due to renal artery stenosis and renal disease. This results in the increased secretion of renin, which in turn raises the blood pressure via the reninangiotensin aldosterone (RAA) system (!p. 184).
