Credit for Service Value (Non Forfeiture)
For unpaid Contract Price, the Plan Holder can still use the plan computed based on the Termination
Values Table by the Insurance Commission
Re-assignment (Living to Dead)
The Plan Holder may assign his Plan to any deceased immediate family member OR third party on
the following conditions:
o Plan holder insurance coverage (Prime or Premium), if any, shall cease automatically
o Assignment value shall be based on the Table of Termination Values provided by the
Insurance Commission
o All assignment value must be used; there will be no Cash Difference Benefit
Transfer (Living to Living)
The Plan Holder may transfer his rights and privileges under the Life Plan to any qualified third
party
o New Life Plan Application form dully filled out and signed the transferee must be
submitted to Loyola for approval
o All payments, made by the original Plan Holder shall be credited to the new Plan Holder
Reinstatement
The Plan Holder may reinstate the Plan within a period of 2 years from the end of the grace period
Un-rendered Service
If the memorial service was not delivered/rendered due to Fortuitous events/Acts of God:
o Full amount of Memorial Service Benefit due shall be paid to the Plan Holder
o Plan may be retained for future availment; shall be transferred to the designated
beneficiary and the Memorial Service Benefits shall continue to increase annually by 6%
of the Plan Base Value
If the memorial service cannot be carried out by Loyola due to family of the deceaseds negligence,
inadvertence, delay or violation of the applicable Life Plan provisions, the sole beneficiary of the deceased shall
have the option to retain the Plan for future availment
