A separate License formshould be filled for each weapon Formshould be filled in English using capital letters only Use a ballpoint pen with Black ink. Submit a photocopy of CEO and contact persons CNIC with the application form Paste one photograph of companys M onogram/Logo on the form and attached one ,. The companys Name must be written on backside of their respective Monogram. Thumb Impression and signature should not exceed out of rectangle allocated for it. Name of company to be printed on the card should not exceed from25 characters including space Incorporation certificate issued bySECP. N.T.N. certificate. Form-29 duly certified by SECP
