Prior to entry of TD Sendong in the Philippine Area of Responsibility, NDRRMC OpCen was already activated as early as 09 December 2011 under Blue Alert status to provide necessary alert and warnings to the RDRRMCs concerned for further dissemination to their respective provincial, municipal, and city DRRMCs. On 16 December 2011, NDRRMC OpCen was further elevated to Red Alert status wherein representatives of member agencies rendered duty at the Operation Center to fast track coordination with their local counterparts for the necessary preparations and possible emergency response. Member agencies represented at OpCen such as DSWD, DOH, AFP, PNP, PCG have been closely coordinating with their local counterparts to address the needs and requirements particularly in the heavily devastated areas specifically in Cagayan de Oro City and Iligan City NDRRMC convened on 17 December 2011 which was presided over by the SND and Chairperson, NDRRMC, and attended by all member agencies wherein His Excellency President Benigno Simeon C. Aquino III provided guidance to the NDRRMC Member Agencies Chairperson, NDRRMC recommended to the Office of the President the declaration of the cities of Cagayan de Oro and Iligan under State of Calamity - the areas that suffered most in terms of casualties and population affected

Accepted the offer of assistance from the international humanitarian community thru the United Nations Resident Coordinator and Humanitarian Coordination in the Philippines to the Philippine government for the victims of Tropical Storm Sendong Convened on 22 December 2011 for the planning and re-implementation of One-Stop-Shop (OSS) in anticipation of more international assistance for the quick release to victims of TS Sendong

Search, Rescue and Retrieval Operations SAR and retrieval operations have been effected in heavily devastated areas through the concerted efforts of AFP, PCG, PNP, BFP, LGUs concerned and other emergency responders from NGOs/POs Philippine Coast Guard (PCG) Cagayan de Oro City (Rescued 1,722 survivors and 34 cadavers retrieved) Coast Guard District Northern Mindanao immediately deployed two (2) teams from Special Operations Unit (SOU)-NM together with CGDNM and CGS Cagayan de Oro (CDO) personnel with rubber boats to conduct SAR operations at Brgys Macasandig, Mabini, Burgos and Isla de Oro. Team A (8 personnel with rubber boat rescued 200 survivors) Team B (8 personnel with rubber boat rescued more or less, 50 survivors) Other team (3 personnel on board M/V Maria Stella) Vessel Traffic Monitoring System of Macalajar Bay manned and controlled by the DCC personnel directed all shipping lines and vessels transiting along Macalajar Bay to conduct surface SAR operation MV Solid Gold, MV Snoppy, MV Super Shuttle Ferry 3, MV Maria Stella 1 and MT Tagoluan successfully rescued 121 survivors All survivors were provided with assistance through CGSNM and were brought to City Central School evacuation center; cadavers were brought to Bulusog Funeral Parlor in Buluo CDO Four (4) SOU-NM with rubber boat onboard PG371 of PN conducted joint SAR mission in Macalajar Bay, CDO and successfully rescued 2 persons BRP Davao del Norte (SARV 3504 left the Port of CDO enroute to Camiguin Island and retrieved 8 cadavers which were brought to funeral parlor for proper disposition and identification - a total of 255 survivors were rescued and 14 cadavers were retrieved BRP Davao del Norte (SARV-3504) and MCS 3004 conducted SAR operations in Macajalar Bay, CDO and retrieved 12 cadavers (4 cadavers by SARV-3504; 8 cadavers by MCS 3004), while in Iligan City, 49 survivors were rescued and 5 cadavers were retrieved CGDBCL stations/detachments conducted monitoring and security to stranded passengers in ports in their respective AORs Quick Reaction Team (QRT) composed of PCG personnel, auxiliary and PNRC volunteers on board MT Seaborne and PG-390 rescued 23 survivors who were turned over to LGUs of Iligan while 4 cadavers were brought to Cavin funeral homes CG Detachment Lugait coordinated with HOLCIM Philippines to direct anchored vessel in the conduct of Maritime Search and Rescue at vicinity of Manticao, Lugait and Gitagum areas. MV Alai Capricorn 3, MV Lorenzo and PG390 of Philippine navy conducted SAR operations covering 20 Sq miles of surface area resulting to the rescue of 10 survivors

MCS 3004 with 2 rubber boats on board departed the port of Iligan and conducted SAR and retrieval operations and Jibe medical evacuation for possible victims reported to be isolated at Brgy Santiago, Hinaplanon and Bayug island - Rescued 16 survivors and retrieved 1 cadaver

PNP PNP Crime Laboratory dispatched 10-man team led by PSupt Francisco A Supe Jr, MD on 19 December 2011 to conduct dead victim identification (DVI) in Cagayan de Oro City TG Tabang Iligan proceeded to its designated area of operations to conduct SAR operations RD PROX proceeded to Iligan City to oversee the ongoing SAR operations conducted by combined elements of ICPO, one platoon RPSB 10, one platoon Lanao del Norte PPSC and RTS 10 led by City Director PSSUPT Celso Regencia PRO X ladies visited MSU Iligan Institute of Technology (the biggest evacuation center) and distributed food, water and other relief goods worth Php120,000 to more than 300 greatly affected families RD PRO X and party including the PRO 10 ladies proceeded to Cagayan de Oro City and provided relief goods worth PhP100,000 to 350 families at Brgy Balulang, Cagayan de Oro City A total of 150 personnel from PRO CARAGA and 96 personnel from PRO 11 arrived PRO 10 and provided support in the SAR efforts in Iligan City Cagayan de Oro City Students of PNP Regional Training School were mobilized to conduct SAR operations in Brgys of Macasandig, Balulang and Consolacion in Cagayan de Oro City RPSB personnel and RTS Police recruits augmented police personnel of COCPO PNP through PCRG established a receiving area for donation at PCRG Camp Crame for the victims of TS Sendong PDG Nicanor A. Bartolome, proceeded to Iligan City together with personnel from the headquarters in Cagayan de Oro Police Office and Staff from CPSC to oversee the continuing rescue, recovery, and retrieval operations; headed the distribution of relief goods in selected evacuation centers; and visited the body of SPO1 Chalon Edrote who died when he was swept away by the flood PRO 13 members of TG TABANG ILAGAN utilized one (1) heavy equipment to recover the belongings and remove waist-level mud and debris in several houses in Iligan City PNP Santiago Iligan delivered assorted relief goods to the office of Task Group Tabang Ilagan for distribution to the typhoon victims

AFP CENTCOM-AFP directed two (2) UH-1H helicopters with personnel to complement the 5052nd SRS in Mactan Airbase to augment SAR operations in Sibulan, Valencia, Tanjay, and Dumaguete City The Philippine Air Force (PAF) aircrafts (C-130 and Fokker planes) airlifted PhP24, 951, 104.22 worth of relief supplies (food, medicines and non-food items from DSWD, UNWFP, Maynilad, PRC, DOH as augmentation support to the ongoing relief operations in Region X 4ID dispatched Rapid Assessment Teams (RRT) and the Tactical Operations Group 10 (TOG10) to the affected areas of Region X

Philippine Navy (PN) BRP Dagupan City (LC551) actual time of departure 10:27PM 20 December 2011 bound for Iligan/CDO loaded with 179.82 tons of relief goods and equipment from donors (Senate, AFP, Prolife Phil, ABS-CBN, Maxilom family, La Salle Green Hills, CFC, Coca cola Bottlers, UNICEF, DSWD, PSTD, Aces Mareine, Philippine Red Cross, Cruising Department Store, DOT, Shopwise, Ateneo, PMSEA PAF C130 transported 9 sorties (269,255 lbs) of assorted relief goods c/o DSWD while a PAF Fokker Plane F-27#669 also transported 10 sorties (98,463 lbs) of relief goods c/o AFP and DWSD on 18-23 December 2011, all bound for CDO AFP personnel conducted clearing operations in Brgys Macasandig, Carmen and Balulang, all in Cagayan de Oro City 2AD, PAF DRTU thru 2-UH-1H Helicopters with 8 personnel of 5052 SRS transported relief goods to 6 affected Barangays in Valencia, Negros Oriental NFC, PN thru PN vessel PG140 proceeded to Dumaguete City to transport relief goods from GMA News-Cebu and Maria Montessory of Balamban, Cebu. They also deployed PNI 311, PG 396 and 4 personnel of NAVSOU5 with rubber boats and OBM to help other DRTU in CDO and Iligan City

NGCP The NGCP has restored all its 69-KV transmission lines affected by Tropical Storm Sendong in Panay area and Mindanao areas (Lanao, North Central and Northeastern) and assured the public that it is ready to implement similar preparations and precautions to minimize the impact of succeeding typhoons on NGCP operation and facilities NGCP President Henry Sy Jr. authorized the use of NGCP hangar in Cagayan de Oro to accommodate the forensic examination of dead bodies of Tropical Storm Sendong DOH As of 22 December 2011, DOH had so far provided logistical support amounting to PhP12,524,389.01 Coordinated with NBI for the implementation of protocol for management of the dead and missing; with DOH Central warehouse for mobilization of logistics, with cluster partners (WASH, Health and MHPSS); and with all RHEMS Coordinators concerned in the affected regions As of 20 Dec, one (1) medical team and one (1) psychosocial team from CHD XI were deployed to CHD X; one (1) medical team from Mayor Hilarion Ramiro Regional Training and Teaching Hospitals augmented assorted medicines/ supply to CHO Iligan City; and Northern Mindanao Medical Center mobilized ambulance to Coast Guard to transfer patients retrieved by PCG CHD CARAGA augmented 9 cadaver bags and 72 boxes of hyphosol to CHD X; pre-positioned medicines and supplies; issued alert messages to local HEMS in respective provinces and cities for appropriate response and coordination CHD VII mobilized team to Sibulan, Valencia, Dumaguete City, San Jose, Pamplona and Bacong and augmented assorted medicines, sodium hypochloride and IVF fluids in Negros Oriental worth PhP30,000.00 Public health concerns: WASH lack of potable water supply in CHD X, ARMM and Lanao del Norte Established Surveillance in Post Extreme Emergencies and Disasters (SPEED) since 21 December 2011 in all evacuation centers in CHD Northern Mindanao for continuous monitoring

DepED DepEd dispatched an inspectorate team composed of technical staff (DepEDPFSED) from DepEd Central, Regional and Division Office to conduct damage and needs assessment to all affected schools in all affected regions Assisted in the identification of recipient schools for the pre-positioning of the CFS/PLAY and school tents from the Save the Children Indonesia/ UNICEF Dispatched technical staffs (Division Project Engineers/Architects and PFCs) to conduct ocular inspection and evaluation to quantify the amount of damages of all affected schools and Northern Mindanao flashflood. COMVAL CES (14 classrooms) are being utilized as evacuation centers as temporary shelter of affected families in the province DepEd together with members of the Education Cluster conducted national and regional coordination meetings for the Rapid Needs Assessment and immediate plans for response, recovery and rehabilitation Assigned staff/coordinators that will join the team/group formed by OCDNDRRMC and BOC that will facilitate and monitor the 24/7 activities and operation of the One-Stop-Shop for the relief goods at designated ports at CDO DSWD The Disaster Response Operations Monitoring and Information Center (DROMIC) has been closely coordinating with DSWD-FOs concerned for any significant reports caused by said weather disturbance; SWAD Teams are closely coordinating with their local counterparts in terms of validation of data as basis for augmentation anytime DSWD NROC airlifted thru AFP Aircraft relief assistance worth PhP24,951,104.22 to DSWD FO X broken down as follows:

PhP7,271,109.60 worth of food and non-food items as additional standby relief resources for the victims of TS Sendong thru DSWD -NROC P17.68 Million worth of assorted relief commodities from the UNWFP, MAYNILAD, DOH, PNRC, JICA, ROTARY INTERNATIONAL, PCOO and various donors thru the DSWD An undetermined number of food and non-food items were also donated by foreign and local donors

Region IX PDRRMC ZDN and CDRRMC Dipolog City conducted emergency meeting to assess the condition, damages and needs of reported families/persons CDRRMC Dipolog City is in constant coordination with MDCC Polanco Action Officer for any development of Polanco River; provided rubber boat to augment rescue equipment; and conducted assessment and profiling of affected families in collaboration with CSWD; and provided 1 sack of rice, noodles, and sardines to support relief operations in BDCC Turno BDRRMC Turno headed by PB Meynardo Baes with the support of rescue teams evacuated the affected families and brought them to Turno Elementary School for temporary shelter; established community kitchen; and provided initial food assistance to the evacuees st 101 Bde Rescue Team headed by Capt. Art Villanueva together with Special Forces assigned in the area responded in Municipality of Polanco and Dipolog City and assisted in transporting passengers trapped by floods along the National Highway at Brgy Guinles, Polanco Polanco MPS led by PI Carmelo Cruzada Velasco, one (1) team from PPSC, Zamboanga del Norte PPO together with troops of 101st IB PA, LGU Polanco, and PRC ZDN chapter conducted SAR operations at severely affected areas Dapitan City Police Station led by PSupt Glenn Macaio Dulawan together with one (1) team from PPSC, AFP Condor 3, and CDRRMC Dapitan conducted SAR operations in affected areas; provided assistance to the vehicles passing the National Highway near Brgys Sulangon, Oyan, Burgos, and Ilaya; assisted in the evacuation of all the families near the Dapitan river to a safer place ORACIS Radio Communications Group headed by Dr. Asis provided radio communication link, rescue ropes, transport vehicles and additional rescuers in the Municipality of Polanco and Dipolog City Dakak Beach Rescue Group headed by Roel Hamoy assisted in the conduct of rescue operations using two (2) jet skis Coast Guard Station in Dipolog City augmented the rescue operations utilizing 1 speedboat and 2 rubber boats Local Government of Polongco distributed 30 bags of noodles, sardines, mineral water, rice and canned goods to the affected families

Dipolog City flashfloods Region X RDRRMC X together with USEC Benito Ramos conducted a briefing on the situation to his Excellency Benigno Aquino; continuously processing DRRMC Financial Aid, and gave PHP 10,000.00 NDRRMC financial assistance to the beneficiaries of dead victims PCG, 4ID PA, TOG PAF, PNP, and OROALERT continue SAR operations At the height of flooding, 10 evacuation centers were being utilized in Cagayan de Oro City, Misamis Oriental OCD-CARAGA, OCD Reg. XI and XII arrived at OCD X for augmentation Joint OCD-RDRRMC V/Albay WASH (Water, Sanitation and Hygiene) Team dispatched in affected areas in Cagayan de Oro on 24 December 2011 OCD Reg. X Regional Director Ana C. Caeda presided over the Inter-agency cluster meeting attended by government cluster leads and cluster members; UN agencies and international organizations held at DSWD X Cagayan de Oro were attended by 30 organizations; Conducted joint humanitarian assessment by UN agencies and International organizations in Cagayan de Oro and Iligan Citys affected areas PCG Iligan deployed one (1) Search and Rescue (SAR) to Brgy Hinaplanon, Iligan City and rescued three (3) unidentified persons with bruises CHD X raised to Code White Alert and conducted rapid health assessment in the affected areas; prepositioned medicine worth PHP 50,000.00 per province (all 6 provinces in region X) and coordinated with the PDRRMC, Philippine National Red Cross, Rescue 118, and other responders CHD X conducted assessment status of victims inside evacuation centers and provided medicines and psychosocial interventions CDRRMC Iligan established four (4) evacuation centers at the height of flooding, namely: City Central School; Sta. Filomena Gym; Santiago Covered Court; and the Upper Hinaplanon Barangay Hall & covered court for the affected families in Iligan City TOG-PAF mobilized three (3) choppers: 2 from Davao and 1 from Butuan RDRRMC/OCD X Operations Center conducted ocular inspection in affected areas; assessment of immediate needs; leading the mobilization of resources, assets, and responders; and continuing survey and monitoring of affected areas Members of the 4ID-PA, TOG-PAF, PNP, and OROALERT were deployed to different areas to rescue stranded commuters and persons in areas reachable by the rescue teams CDO CDRRMC established evacuation centers in the City Central School, West Central School, and Macasandig Covered Court. Region VII Negros Oriental Sibulan, Valencia, and Dumaguete City Region IX Zamboanga del Norte - Polanco- were temporarily evacuated to higher grounds for safety Barangays Turno (Sitio Wakwak) and Dicayas in Dipolog City on 15 December 2011 at around 10:00 AM Sixty (60) families/300 persons from Sitio Wakwak, Brgy Turno evacuated to Turno Elementary School. One Cargo Ship Ever Transport Cargo anchored at Brgy. Calindagan, Dumaguete City sank due to strong winds and big waves. Said cargo ship is owned by Kherl Lines loaded with Asia Brewery products LCT Tampi Twin submerged halfway while moored at the Maayo Private Wharf in Sibulan, Negros Oriental due to big waves. The sea water entered through the open portion of its stern and thereby flooded the engine room A pump boat named Bantay Dagat owned by LGU Lazi, Siquijor sank while docked at the Lazi Port after being battered by strong winds and big F/BCA ALLEN JOHN with eight (8) fishermen on board, was reported missing since 16 Dec at Vicinity 60 NM East of Cateel Bay , Bislig City. Said sunk when battered by strong waves and was recovered. However, the fishermen on board were safe
