Nurse Case Manager - Permanent, full-time Nurse Case Manager providing personal visits two to three times weekly to qualif ied hospice patients. Assessed patients every 60 days for hospice recertificatio n. Assessed physical status, monitored vital signs and pain control during weekl y visits and kept medications refilled, reported any changes in physical or ment al status to doctor and hospice group. Directed patient care staff to ensure per sonal hygiene was maintained to the highest standard. Visited with family weekly or more often if needed to ensure needs were being met and patient was comforta ble. Attended my patients funerals and comforted families. This fast-paced unit had patients with a variety of medical and surgical problems as well as cardiac dysrhythmias.
