A completed application form constitutes submitting the following on or before January 12, 2012; 5:00 pm (Pakistan Standard Time): A completed online application form; The application processing fee of Rs 4,000* (for Pakistani Residents only) The application processing fee of US $100 (for Foreign/overseas residents only) 1The LCAT registration fee of Rs 1,500* (if applying to SHSSL and/or SDSB) The SSE Admission Test registration fee of Rs 1,500* (if applying to SSE) 2Attested/Certified Copies of Complete Academic Transcripts and Certificates of:

Matric or O' Level or American High School - 9th and 10th grade complete results; FA/ICOM (if applicable)/FSc/ICS complete result or FA/ICOM (if applicable)/FSc/ICS first year result (in case the FA/ICOM/FSc/ICS final exam has not been taken yet) or A' Level complete result or grades for any A' Level subject(s) completed or A' Level first year results (in case the A' Level final exam of three principal subjects has not been taken yet) 3 or American High School complete result or up to 11th grade results (in case the High School Diploma 12th Grade final exam has not been taken yet) or International Baccalaureate complete result or International Baccalaureate first year complete results(in case the IB final exam has not been taken yet);

Equivalence Certificate (for applicants who have completed their degrees other than FA/FSc/ICS/ICOM/ALevels or IB; SAT Score reports

SAT score report (s) (required for overseas applicants exempted from the LCAT/ SSE Admission Test); Students who are taking LUMS Admission Test(s) and have also taken/are planning to take the SAT are encouraged to submit a copy of their SAT score report(s);

Four passport size photographs with white background; Proof of payment;


1 Applicants taking both LCAT and SSE Admission Test will pay a total of Rs. 3000/- as Test Registration Fee in addition to the application processing fee of Rs. 4000/-. 2

Transcripts and certificates should be attested from Schools administration.

3 Applicants taking their A levels or AS level exam in November 2011 are required to submit the final result within three days of its announcement; however, they must provide this information through online application form and by submitting a copy of the Statement of entry along with all other supporting documents.

**Financial Aid Application Checklist (if applicable): You will need to provide photocopies of the following documents with the Financial Aid form. Check off each item as you attach it to your form. Salary certificate (or pension slip) of all earning family members Bank statements for last year of all family members having an account Income tax returns of all earning family members for last year Fee bill and any concession document from the last institution you attended Utility bills (May 2011 through November 2011) covering electricity, gas, telephone and water Saving certificates, bonds, shares, investments, CDC account statement (if applicable) Property ownership documents, including agricultural land, plots and houses (if applicable) Rental lease agreement (if applicable) Loan document(s), including credit card bills and bank loan statements (if applicable) Medical bills/expenditure related documents (if applicable) Fee concession (scholarship/loan) document(s) of sibling(s) (if applicable) Wealth statement for all family members for last year (if available) Your domicile certificate (if available) Your complete passport (if available) Any other document(s) demonstrating financial need

LUMS Financial Aid Office may request additional information, or conduct a physical audit, in order to properly evaluate your financial need. Please respond promptly to avoid delays. All data and documents submitted with the Financial Aid form will be treated as confidential. Applications without complete documents will not be processed. Please print & paste the MAILING LABEL on the package containing your supporting documents before sending it to the LUMS Admissions Office. Note: * The Application Processing fee and the LUMS Admission Test Registration Fee can be paid: Through a bank draft/pay order drawn in favour of Lahore University of Management Sciences. All applicants are required to submit original bank draft/ pay order; Online through the following link: http://pay.lums.edu.pk/default.asp; By depositing in cash at the MCB Booth located within the LUMS campus.

Documents sent/postmarked after January 12, 2012 will not be processed. Documents received after the deadline/in multiple packages/without a mailing label will not be processed. It is the responsibility of the applicant to provide correct and timely information to the Admissions Committee. Applications without complete and correct information or documents will not be considered for admission. All information submitted through online application should be supported by the provision of attested copies of relevant transcripts and certificates.
