Gebrauchsanleitung
Um den LT sicher anwenden zu knnen, muss der Benutzer mit dieser Gebrauchsanweisung vertraut sein. Der Hersteller garantiert 50 Aufbereitungszyklen bei sachgerechtem Gebrauch und Einhaltung der Reinigungs-, Desinfektions- und Sterilisationsanleitung.
Warnhinweise
Der Anwender sollte bei der Anwendung des LT mit folgenden Warnhinweisen vertraut sein: Durch zu hohe Beatmungsdrcke knnen Gase sowohl in den Magen als auch in die Atmosphre gelangen. Nach der Platzierung suffiziente Beatmung am besten durch CO2-Messung oder durch Auskultation der Lunge und Thoraxbewegungen sicherstellen. Kann der LT nicht entsprechend gereinigt, gesplt und getrocknet werden, besteht die Gefahr von potentiell gefhrlichen Rckstnden oder unzureichender Sterilisation.
Latexfrei
Der LT ist 100% latexfrei und daher fr den Einsatz an Patienten mit Latexallergie geeignet.
Achtung: Ungeeignete Reinigungs- und Desinfektionsmittel bzw. Rckstnde von Reinigungs- und Desinfektionsmitteln knnen die Materialeigenschaften des Silikons negativ verndern, z.B. versprden und Materialalterung beschleunigen. Ein Larynx-Tubus, der solche irreversible Materialschdigungen aufweist, darf auf keinen Fall wieder verwendet werden.
Zur Reinigung bzw. Desinfektion des LT knnen die maschinellen Verfahren: - thermisch-alkalisch 93C (Neutralisation nach alkalischer Reinigung auf Zitronensurebasis) - thermisch-neutral 93C angewandt werden.

Fr die maschinelle Aufbereitung: Sekumatic FR 0,5% maschinen dosiert (Miele Vario Programm) Sekumatic FRE 0,5% maschinen dosiert (Miele Vario Programm) Sekumatic FNZ 0,3% Neutralisation (Miele Vario Programm) Die o.g. Aufbereitungsprodukte sind entsprechend der ECOLAB Gebrauchsanweisung einzusetzen (ECOLAB Deutschland - Anwendungstechnik - Tel.: +49 (211) 98 93-0 - www.ecolab.com).
b) Sterilisation
Vor dem Sterilisieren muss das Produkt gereinigt und mit Wasser rckstandsfrei gesplt und getrocknet werden. Rckstnde von Reinigungs- bzw. Desinfektionsmitteln knnen whrend der Dampfsterilisation zu Materialschdigungen fhren. Zur Sterilisation empfiehlt VBM ein validiertes Dampfsterilisationsverfahren nach EN 554 bei 134C. Dabei sind nachfolgende Anweisungen unbedingt einzuhalten, um die einwandfreie Sterilisation des LT unter Ausschluss von Beschdigungen zu gewhrleisten: Der Ventilffner ermglicht whrend der Sterilisation den freien Gasaustausch von und zu den Cuffs. Das Ventil des Pilotballons ffnet sich bei Verbindung mit dem roten Ventilffner (Abknicken der Zuleitung verhindern!). Achtung: Den Ventilffner fest aufdrehen und nur fr die Sterilisation verwenden.
(Wird der Ventilffner nicht eingesetzt, sind folgende Anweisungen zu beachten: Unmittelbar vor dem Einlegen des Produktes in den Dampf-Autoklaven ist mit einer Spritze smtliche Luft aus den LT Cuffs zu evakuieren. Der LT muss trocken sein. Die hohen Termperaturen und der Unterdruck im Autoklaven fhren zur Ausdehnung der im Cuff verbleibenden Luft bzw. Feuchtigkeit, was irreparable Schden des Cuffs und des Pilotballons zur Folge haben kann. Vorsicht: Fllen sich die entlfteten Cuffs sofort wieder - wenn auch nur leicht - ist dies ein Zeichen dafr, dass ein Cuff oder das Ventil einen Defekt aufweist. Der Tubus darf daraufhin weder autoklaviert noch wieder eingesetzt werden. Jedoch ist es normal, dass sich der LT ber den Zeitraum mehrerer Stunden hinweg langsam wieder fllt, da das Silikonmaterial geringfgig gasdurchlssig ist.)
Vor dem Autoklavieren ist der LT in einen hierfr geeigneten Beutel einzulegen. Beim Autoklavieren sind die spezifizierten Sterilisationsparameter 134C einzuhalten. Der LT bleicht mit zunehmendem Alter und mehrmaliger Anwendung nach und nach aus. Nach jeder LT Sterilisation die Record Card ausfllen.

b) Vor jeder Anwendung des LT ist dieser dem nachfolgenden Leistungstest zu unterziehen: - Das Innere des Tubus auf lose Partikel und verengte Stellen untersuchen. - Die Cuffs belften, um sie auf mgliche Undichtigkeiten zu untersuchen.

e) Vorherige Verabreichung von Sauerstoff ist in jedem Falle zu empfehlen. f) Erreichen eines angemessenen Narkosestadiums Vor Einfhren des LT muss eine ausreichende Narkosetiefe gewhrleistet sein. Bei der Einleitung der Narkose sollten Standard berwachungstechniken angewendet werden. Generell betrgt die ntige Narkosetiefe nur wenig mehr als bei der Einfhrung eines Guedel-hnlichen Tubus. Dem weniger erfahrenen Anwender wird die Wahl eines tieferen Narkosestadiums empfohlen. g) Positionieren des Kopfes Die ideale Kopfstellung zum Einfhren des LT wird durch eine erhhte Kopflagerung erreicht. Jedoch ermglichen die Krze und die gebogene Form des Tubus, dass er auch in neutraler Kopfstellung eingefhrt werden kann. Bei Kindern wird aufgrund der Anatomie ein Anheben des Kinns (chin lift) und ein leichtes Kopfberstrecken empfohlen. h) Einfhrung des Larynx-Tubus LT Um den LT zu positionieren, muss eine Technik angewendet werden, welche die Anatomie bercksichtigt. Die folgende Gebrauchsanweisung ist daher zu beachten: Es wird empfohlen, den LT im Bereich der schwarzen Linien (Zahnmarkierungen) wie einen Stift zu halten.

Mit der freien Hand wird der Mund offen gehalten und sichergestellt, dass die Zunge whrend der Abwrtsbewegung des LT nicht nach hinten fllt. Tubus nicht mit Gewalt einfhren! Den LT mit der Spitze gegen den harten Gaumen des Patienten einfhren. Den LT sanft am Gaumen entlang mittig in den Hypopharynx schieben. Die mittlere schwarze Linie sollte nun auf Hhe der oberen Zhne liegen. Bei Problemen kann auch ein seitliches Einfhren mit Drehbewegung hilfreich sein. Die beiden Cuffs mit dem auf der Spritze markierten Volumen belften. Durch die speziell entwickelte Ballonzuleitung wird dabei zuerst der proximale Cuff gefllt, welcher den Tubus stabilisiert. Sobald sich dieser der Anatomie des Patienten angepasst hat, wird automatisch der distale Cuff belftet. Bei elektiven Anwendungen, vor allem bei Spontanatmung, wird empfohlen, den LT bis zum Konnektor einzufhren. Danach die Cuffs mit dem VBM Cuffdruckmessgert Universal auf 60cmH2O belften und den Tubus an das Beatmungssystem anschlieen. Whrend Bettigung des Atembeutels den Tubus gleichzeitig zurckziehen, bis eine widerstandsfreie Beatmung mglich ist (groes Tidalvolumen mit niedrigem Beatmungsdruck). Den Cuffdruck nochmals anpassen und kontinuierlich berwachen, um erhhte Cuffdrcke zu vermeiden.

Der LT ist nun platziert und der Patient kann beatmet werden. Erfolgreiche Beatmung durch Auskultation, Thoraxbewegungen und Kapnographie berprfen. Ausschluss einer Magenblhung durch Inspektion, Palpation und Auskultation des Epigastriums. Bei unzureichender Beatmung kann der LT zwischen den Zahnmarkierungen repositioniert werden. Bei erfolgreicher Beatmung kann der LT mit dem Beiblock fixiert werden.
Achtung: Zum Entfernen des LT mssen die Cuffs mit der beigefgten Spritze vollkommen entleert werden, da sie sonst beim Herausziehen des LT beschdigt werden knnten. Den Tubus nur unter Absaug- und Intubationsbereitschaft entfernen.

Instructions for Use
In order to use the LT safely the user must first be familiar with these instructions for use. The manufacturer guarantees 50 uses if the following cleaning, disinfection and sterilization instructions are followed strictly.
Warnings
The user should be familiar with the following warnings when considering or attempting to use the LT: High airway pressures may divert gas either to the stomach or to the atmosphere. After placement, perform standard checks for breath sounds and utilize an appropriate carbon dioxide monitor as required by law or hospital protocol. Failure to properly clean, rinse, and dry the LT device may result in retention of potentially hazardous residues or inadequate sterilization.
Latex-free
The LT is 100% latex-free and should be considered safe to use on patients who are latex sensitive.

b) Sterilization
For sterilization we recommend a validated steam sterilization process according to EN 554 at 134C / 273F. Prior to each sterilisation process the devices must be cleanded, rinsed with water and dried. Residues of the cleaning agents and disinfectants could cause damage of the material. Strictly adhere to the following instructions to ensure proper sterilization and to prevent damage: The Valve Opener acts as a vent during the sterilization procedure and allows a free passage of gases from and to the cuffs. The valve of the pilot balloon is opened after connection to the red Valve Opener. (Avoid kinking of the inflation line). Caution: A firm connection is required. Only use the Valve Opener for sterilization procedure.
(In case that the Valve Opener is not used please adhere to the following instructions: Prior to packaging for sterilization use a syringe to remove all air from the LT cuffs. Ensure that the LT is dry. Any air or moisture left in the cuff will expand in the high temperature and low pressure environment of the autoclave, causing irreparable damage to the cuff and the pilot balloon. Caution: If the deflated cuffs immediately re-inflate, even slightly, do not autoclave or reuse this tube. This may indicate a presence of a malfunctioning valve or a leak in the cuff. It is normal, however, for the LT to re-inflate slowly over a period of several hours as the silicone material used in its manufacture is slightly gas permeable.)
The LT should be placed in an appropriate autoclave-proof bag before autoclaving. Always follow the manufacturer's recommendation when autoclaving any medical device. All autoclave cycles typically used for porous items are acceptable for the sterilization of the LT, provided the maximum autoclave setpoint temperature does not exceed 134C (273F). The LT will gradually discolor with age and reuse. Complete the Record Card each time the LT is sterilized.

b) Before each use of the LT the device must pass the following performance test: - Examine the interior of the airway tube to ensure that it is free from blockage or loose particles. - Inflate the cuffs to check for any possible leakage. Caution: Do not use the LT if the cuff deflates. c) Check that both cuffs are completely deflated and that the LT has been lubricated with water-soluble lubricant. d) Have a spare LT ready and prepared for immediate use e) Pre-oxygenation is advisable in all cases

f) Achieve the appropriate depth of anesthesia An adequate level of anesthesia is required before the insertion of the LT. Standard monitoring techniques should be followed when inducing anesthesia. In general, the depth of anesthesia needed is little more than that required for the insertion of a Guedel-type airway. It is recommended that the less experienced user choose a deeper level of anesthesia. g) Positioning the head The ideal head position for insertion is the sniffing position. However the shortness of the tube and the s-shape allows it to be inserted with the head in a neutral position. For children it is recommended (due to the anatomy) to have the head slightly extended with a chin lift manoeuvre. h) Insertion of the Laryngeal Tube LT To position the LT it is necessary to use a technique which takes the anatomy into account. Follow the instructions of use in the following sequence:
The recommended technique is to hold the LT like a pen in the area of the black lines (teeth marks).

With the free hand, hold the mouth open and make sure that the tongue is not folded back during the downward movement of the LT. Never use force! Place the flat edge of the LT tip against the hard palate of the patient and slide it along the palate in the midline of the mouth down into the hypopharynx until the center black line is level with the upper teeth. In case of insertion problems a lateral insertion might be useful. Inflate both cuffs with the volume which is indicated on the syringe. Due to the specially designed inflation line the proximal cuff is filled first which stabilises the tube. Once the proximal cuff has adjusted to the anatomy of the patient the distal cuff will be automatically inflated. In elective use, especially during spontaneous ventilation, it is recommended to advance the LT to the connector. Then inflate the cuffs with the VBM Cuff Pressure Gauge Universal to 60cmH2O and attach the tube to the breathing circuit. While gently bagging the patient to assess ventilation, simultaneously withdraw the airway until ventilation is easy and free flowing (large tidal volume with minimal airway pressure). Re-adjust cuff pressure and monitor continuously to avoid excessive cuff pressures.

The LT should now be properly positioned and the patient can be ventilated. Check lung ventilation by auscultation, capnography and chest movement. If ventilation is not sufficient, re-position the tube between the teeth marks. If efficient ventilation is achieved, the LT can be fixed in position. The biteblock can do this, but is not required.
Note: For LT removal, it is important that both cuffs are completely deflated with the included syringe to avoid cuff damage. Removal should always be carried out in an area where suction equipment and the ability for rapid intubation are present.

Mode d'emploi
Pour l'utilisation du LT, l'utilisateur doit bien connatre ce mode d'emploi. Le fabricant garantit 50 utilisations si l'emploi du produit est conforme aux instructions et si les consignes de nettoyage, de dsinfection et strilisation sont respects.
Avertissements
L'utilisateur doit connatre les avertissements suivants avant dutiliser le LT : En utilisant une pression de ventilation trop leve on risque d'avoir des fuites de gaz non seulement dans l'estomac mais aussi dans l'atmosphre. Aprs le positionnement du LT il faut utiliser des techniques standards de contrle comme le mouvement du thorax, en utilisant un moniteur CO2 adapt. Si le LT nest pas nettoy, rinc et sch correctement, il y a risque de persistance de rsidus potentiellement dangereux ou de strilisation inoprate.
Sans latex
Le LT est 100% sans latex, il est donc appropri pour des patients sensibles au latex.
1. Prparations l'utilisation du LT
Le LT est livr en emballage propre prt lusage. Le nettoyer, le dsinfecter et si ncessaire le striliser soigneusement aprs le premier emploi et chaque ultrieure usage.
a) Nettoyage et Dsinfection
Utiliser des lessives caustiques, dtergents alcalins ou des agents pH neutre pour le nettoyage des ballonnets et du tube. Il faut absolument viter tout contact avec des objets vifs et pointus lors du traitement et lutilisation comme cela peut rsulter dans une dtrioration du silicone. Prvenir le contact des dispositifs en silicone aux huiles de silicone ou graisses. Rincer le LT abondamment leau dminralise afin dliminer tous rsidus des dtergents utiliss. Pour la dsinfection, utiliser des dsinfectants pour instruments, appropris et approuvs pour silicone et matires plastiques thermostables. Des dsinfectants contenant des composants comme les phnols ou les alcylamines (glucoprotamine) ne sont pas du tout appropris et peuvent rsulter dans le durcissement du matriau. Observer les instructions pour les temps de contact et les concentrations prconiss par le fabricant sinon le matriau peut tre dtrior. Veiller rincer les produits abondamment leau dminralise pour sassurer que tous les rsidus des dsinfectants utiliss sont limins. Lexaminer pour sassurer que toutes les matires trangres visibles ont t limines. Avertissement: Des dtergents et dsinfectants non appropris ou des rsidus des dtergents et dsinfectants peuvent modifier les caractres du silicone en ngatif et rsulter dans un durcissement par exemple. Ne jamais rutiliser un LT qui est dtrior. Lexaminer pour sassurer que toutes les matires trangres visibles ont t limines.

b) Strilisation
La mthode recommande pour striliser le LT est la strilisation la vapeur (autoclave) selon EN 554 134C / 18mn. Avant la strilisation il faut nettoyer, rincer abondamment leau dminralise et scher les dispositifs. Des rsidus de dtergents ou dsinfectants pourraient provoquer une dtrioration du matriau pendant la strilisation la vapeur. Pour la strilisation, observer absolument les instructions suivantes pour assurer une strilisation optimale du LT en vitant toute dtrioration: Pendant la strilisation, le connecteur pour valve permet lchange de gaz de lextrieur vers les ballonnets. La valve du tube de gonflage est ouverte lors de sa connexion avec ce connecteur rouge (attention de ne pas plier le tube). Avertissement: Assurer une connexion ferme. Nutiliser le connecteur pour valve que pendant la strilisation. (Si le connecteur pour valve nest pas utilis, il faut respecter les instructions suivantes:
Immdiatement avant l'insertion du LT dans l'autoclave vapeur, vacuer avec une seringue l'air compltement des ballonnets. S'assurer que le LT est sec. Les tempratures leves et les pressions basses dans l'autoclave conduisent l'expansion de l'air et de l'humidit dans les ballonnets. Cela peut provoquer des dommages irrparables aux ballonnets. Attention: Si les ballonnets se regonflent immdiatement - mme lgrement - cela signifie que les ballonnets ou la valve ne sont plus tanches. Ne plus utiliser le LT dans ce cas. Cependant un regonflement lent, en plusieurs heures, est normal; le silicone tant lgrement permable aux gaz.)
Avant d'autoclaver le LT, l'insrer dans un sachet de strilisation appropri. Observer les paramtres spcifis pour la strilisation de 134C / 18 minutes. Le LT se dcolore quand il vieillit au fur et mesure des strilisations.

b) Avant chaque utilisation du LT; il faut le soumettre au test de performance suivant: - Examiner lintrieur du LT et vrifier labsence de particules libres, lintgrit et la libert des orifices rtrcies. - Pour examiner ltanchit des ballonnets, il faut les gonfler. Pour l'utilisateur moins expriment, il est recommand de choisir une profondeur d'anesthsie plus importante. g) Positionner la tte La position idale de la tte pour l'insertion du LT est lgrement sur leve. Cependant le tube court et sa forme en S permettent une insertion dans une position neutre de la tte. A cause de lanatomie des enfants il est recommand de positionner la tte en lgre extension, ainsi que le menton. h) Insertion du Tube Laryng LT Pour positionner le LT il faut utiliser une technique adapte l'anatomie cervicofaciale du patient. C'est pourquoi il est ncessaire de tenir compte du mode d'emploi suivant: On recommande de tenir le LT au niveau des marques noires (marques repres) comme un stylo.

Avec la main libre dgager le maxillaire infrieur vers le haut - et s'assurer que la langue ne tombe pas en arrire pendant le mouvement du LT vers le bas. N'insrer pas le LT avec force! Pousser la partie infrieure du LT avec l'extrmit douce contre le palais dur du patient. Faire glisser le LT doucement en suivant le voile du palais et le mur postrieur centralement dans l'hypopharynx. Le marque repre au milieu doit maintenant tre au niveau des dents. En cas de problmes insrer le LT en latral en le tournant un peu. Gonfler les ballonnets au volume marqu sur la seringue. Le ballonnet proximal qui stabilise le LT est remplit d'abord. Aprs qu'il soit adapt l'anatomie du patient, le ballonnet distal est gonfl automatiquement et ceci grce une communication des canaux de gonflage. Pour des indications lectives, surtout pour la ventilation spontane, on recommande d'insrer le LT jusqu'au raccord. Puis, de gonfler les ballonnets avec l'inflateur de pression 60cm d'H2O et de brancher le tube au circuit de ventilation. En manipulant le ballon d'anesthsie, retirer le tube en mme temps jusqu ce que la ventilation soit facile et sans rsistance (grand volume tidale et pression de ventilation minimale). Ajuster la pression dans les ballonnets encore une fois et la vrifier en continu afin d'viter une pression excessive.

Le LT est en place et le patient peut tre ventil. Vrifier la ventilation pulmonaire par capnographie, par auscultation du thorax et de l'estomac avec un stthoscope. Vrifier labsence de bruits hydroariques en palpant et auscultant le creux pigastrique. Si la ventilation nest pas suffisante, possibilit de repositionner le LT entre le marque repre suprieure et infrieure. Si la ventilation est suffisante, possibilit de fixer le LT par le cale-dents.
Attention:
Les ballonnets doivent tre compltement vides, sinon il y a possibilit de dtrioration la dpose. Enlever le LT seulement quand on est prt aspirer et intuber.

Manual de uso
Para poder utilizar el Tubo Larngeo LT de modo seguro, leer atentamente las siguientes pginas. El fabricante garantza el uso del Tubo Larngeo LT hasta 50 veces, respetando cuidadosamente las instrucciones de limpieza, desinfeccin y esterilizacin.
Advertencias
El usario debe estar familiarizado con las siguientes advertencias al momento de considerar el uso del Tubo Larngeo LT: La presin de las vas respiratorias superiores puede desviar gases hacia el estmago o hacia la atmsfera. Despues de colocar el LT controlar el movimiento del trax y hacer el control y el monitoreo del Co2. Los tubos que no se puedan limpiar, lavar y secar correctamente ponen en riesgo de resudios peligrosos o de una esterilizacin inadecuada.
Libre de ltex
El Tubo Larngeo LT es totalmente libre de ltex y su uso es seguro en pacientes que tienen sensibilidad a este material.
a) Limpieza y desinfeccin
Lavar completamente el LT y los balones con agua y jabn o con agentes desinfectantes no agresivos. Ideal son agentes desinfectantes de un pH neutro. Nunca utilizar material agresivo, como, p. ej. cepillos, que puedan daar los balones y la superficie del LT. Los productos de silicona tampoco pueden tener contacto con aceites o grasas de silicona. Eliminar todo el residuo de los detergentes usados enjuagando bien el LT con suficiente agua desionizada. La desinfeccin debe hacerse con desinfectantes comerciales aprobados para la desinfeccin de instrumentos medicos, la silicona y material termoplstico. No son adecuados los agentes o desinfectantes con componentes similar al fenol o alcalinos. Estos materiales pueden endurecer el material. Usar rigorosamente el tiempo de imersin y la concentracin consejado por el fabricante. Enjuagar bien el LT en agua corriente para asegurar que ningn tipo o resto de desinfectante quede en el producto. Atencin: La utilizacin de agentes y desinfectantes no adecuados causa defectos en el material del LT. Los balones pueden daarse y la estabilidad del LT se puede perder. Si el LT posee de daos irreversibles, el LT no puede ser utilizado. Para la limpieza o desinfeccin se recomienda utilizar el procedimiento mecnico: - trmico alcalino 93C (neutralizacin a base de acido citrico despues de la limpieza) - termico neutral 93C

b) Esterilizacin
Para la esterilizacin consejamos el procedimiento en autoclave segn la norma EN 554 a 134C. Antes de cada esterilizacin limpiar los productos, enjuagar con agua y secarlos bien. Los resudios de los agentes de limpieza o desinfeccin pueden provocar daos a los productos durante la esterilizacinn a vapor. Seguir atentamente el procedimiento indicado a continuacin para garantizar una esterilizacin sin daar el producto: Durante la ventilacin el abridor de vlvula permite el pase de gases desde el exterior de los balones hacia fuera. La vlvula permanecer abierta conectandola al abridor de vlvula. (Atenccin: No doblar el tubo). Advertencia: Conectar el abridor de vlvula a la vlvula y solo usar el abridor durante la esterilizacin.
(Si el abridor de vlvula no es utilizado, rogamos seguir las instrucciones a continuacin: Inmediatamente antes de la esterilizacin evacuar completamente los balones con la jeringa. El Tubo tiene que estar seco. Restos de aire o humedad que se encuentren en los balones provocar por la temperatura y la presin en el autoclave daos irreparables. Atencin: Si los balones desinflados se vuelven a inflar, recomendamos no esterilizar o reusar el tubo. En estos casos tanto los balones como la vlvula pueden estar daados. Es normal que los balones se reinflen paulatinamente durante un periodo de horas, ya que el material de silicona es permeable.)
Antes del procedimiento en autoclave colocar el LT en un envase adecuado para autoclave. Mantener los parmetros espezificados de 134C (10 min.). El LT perder con el tiempo un poco el color. Despues de cada esterilizacin rellenar la Record Card.

f) Conseguir una anestesia adecuada: Antes de introducir el LT es necesario tener una anestesia profunda. Para conseguir una anestesia adecuada es necesario usar aparatos estndar. En general la anestesia es un poco mayor a la del uso con una cnula de Guedel. g) Posicin de la cabeza: La posicin ideal para la introduccin del tubo es la posicin elevada de la cabeza. El LT tambien permite la introduccin en posicin neutral. En pediatria recomendamos, por la anatomia, elevar la mandibula (chin lift) y una extensin de la cabeza ligera. h) Introduccin del Tubo Larngeo LT: Para posicionar el LT es necesario usar una tcnica que tenga en consideracin la anatomia del paciente. Seguir atentamente las instrucciones de uso a continuacin:
Recomendamos sujetar el LT como una pluma por encima del baln proximal.

Abrir la boca del paciente. Introducir el tubo en posicin central hasta llegar la marcacin central al nivel de los dientes del paciente. Durante la introduccin mantener la punta del LT a lo largo del paladar y prestar atencin de no empujar la lengua hacia dentro. Inflar los balones con el manmetro de control a una presin de 60cmH2O. Gracias al nuevo sistema de inflado en primer lugar se infla el baln proximal, estabilizando as el LT y despus el baln distal. En uso electivo y en especial durante la ventilacin espontanea, recomendamos introducir el LT hasta el conector. Inflar los balones mediante el manmetro de Inflado Universal a una presin de 60cmH2O y conectar el LT al circuito de ventilacin. Durante el uso con la bolsa de respiracin, tirar lentamente del tubo hasta conseguir una ventilacin fcil y con poca resistencia (volumen tidal alto con presin ventilatoria baja). Reajustar nuevamente la presin en los balones y controlar de manera contnua para evitar presiones altas.

Iniciar la ventilacin a travs del conector de 15mm. Controlar la ventilacin por auscultacin del pulmn, capnografa y el movimiento del trax. Si la ventilacin no es suficiente, recomendamos reposicionar el LT entre las marcaciones de los dientes. El protector antimordisco protege el LT y permite una fijacin segura.

Istruzioni per luso
Prima di utilizzare il tubo LT, leggere attentamente le istruzioni d'uso. Il produttore garantisce luso del prodotto fino a 50 volte se le procedure di pulizia, disinfezione e sterilizzazione vengono correttamente eseguite dallutilizzatore.
Avvertimenti
Per poter utilizzare il tubo laringeo LT in modo sicuro, leggere attentamente le seguenti avvertenze: Pressioni elevate delle vie aeree possono deviare i gas all'interno dello stomaco o nell'ambiente. Dopo il posizionamento, procedere con i normali controlli respiratori ed utilizzare un apparecchio adatto per il monitoraggio dell'ossido di carbonio, come indicato dal protocollo ospedaliero. La non corretta pulizia o sterilizzazione pu essere causa di problemi durante lutilizzo.
Privo di lattice
LT privo di lattice e quindi sicuro per luso con pazienti che sono sensitivi al lattice.

I prodotti succitati devono essere applicati conforme all'istruzioni d'uso della ditta ECOLAB (ECOLAB Germania Anwendungstechnik- Tel.: +49 (211) 98 93-0 - www.ecolab.com). b) Sterilizzazione Per la sterilizzazione consigliamo il procedimento in autoclave secondo EN 554 a 134C. Prima di ogni sterilizzazione, gli strumenti devono essere lavati, risciacquati con acqua ed asciugati. Residui di detergenti ed agenti disinfettanti possono provocare danni al materiale durante la sterilizzazione a vapore. Attenersi scupolosamente alle procedure indicate qui di seguito affinch sia garantita una perfetta sterilizzazione senza provocare danni al LT: L aprivalvola permette il passaggio dei gas dalle cuffie e alle cuffie. La valvola si apre avvitand laprivalvola (Evitare che il tubo si pieghi). Attenzione: Avvitare laprivalvola saldamente e utilizzarlo soltanto per la sterilizzazione.
(Se laprivalvola non viene utilizzato, seguire le seguenti istruzioni: Prima di procedere con la sterilizzazione in autoclave, far fuoriuscire tutta laria da entrambe le cuffie, mediante la siringa inclusa nel kit, in modo tale che risultino perfettamente sgonfie. Assicurarsi inoltre che la valvola del LT e la siringa siano ben asciutte. Eventuali residui di aria o di umidit rimasti allinterno delle cuffie, a contatto con le elevate temperature in autoclave, provocherebbero unespansione tale da arrecare danni irreparabili sia alle cuffie sia al palloncino spia. Attenzione: Se la cuffia si gonfiasse immediatamente non procedere con la sterilizzazione in autoclave o riutilizzare il tubo. Ci potrebbe indicare il malfunzionamento della valvola oppure una perdita nella cuffia. E tuttavia normale che, trascorse alcune ore, LT si gonfi ancora lentamente, poich il silicone utilizzato per la realizzazione del tubo leggermente permeabile ai gas.)
Prima di procedere con la sterilizzazione in autoclave, il tubo deve essere imbustato in un sacchetto cartapolietilene. Considerare che la temperatura massima regolata sulle autoclavi non supera i 134C per 10 minuti. Il tubo laringeo perder gradualmente colore con linvecchiamento e dopo i vari utilizzi. Compilare il Record Card dopi ogni sterilizzazione del dispositivo.

f) Eseguire un adeguata anestesia Prima di inserire il LT, necessario portare il paziente ad un adeguato livello di anestesia durante il quale bene seguire le tecniche di monitoraggio standard. Solitamente la profondit dellanestesia necessaria poco pi di quella richiesta per linserimento di una cannula di Guedel. Per un utilizzatore poco esperto, si consiglia di scegliere un livello di anestesia pi profondo. g) Posizione della testa La posizione ideale della testa quella della testa elevata. Tuttavia, grazie alla lunghezza ridotta del tubo ed alla sua angolazione, possibile procedere con linserimento del LT qualunque sia la posizione della testa del paziente. Non quindi indispensabile lestensione. Sui pazienti pedictrici, proprio per la loro particolare confirmazione anatomica, si consiglia di tenere la testa leggermente iperdistesa sollerando il mentro. h) Inserimento del tubo laringeo LT Per posizionare il LT necessario utilizzare una tecnica che prenda in considerazione l'anatomia. Seguire attentamente le istruzioni riportate qui di seguito: Si consiglia di tenere il tubo tra le dita come se fosse una penna, a livello del marker nero di riferimento.

Con la mano libera, tenere aperta la bocca del paziente ed assicurarsi che la lingua non venga spinta all'indietro durante la manovra di inserimento. Mai esercitare forza! Posizionare la parte piatta della punta del tubo contro il palato e farla scivolare all'interno della cavit orale fino a posizionare la linea di marker mediana a livello dell'arcata dentaria superiore. In caso di difficolt di intubazione, potrebbe essere utile un'intubazione laterale. Gonfiare entrambe le cuffie con il volume indicato sulla siringa. Grazie alle speciale linea sequenziale, la cuffia prossimale sar la prima a gonfiarsi per stabilizzare il tubo. Una volta che la cuffia prossimale si ben adattata alla conformazione anatomica del paziente, la cuffia distale si gonfier automaticamente. Nell'anestesia di elezione si consiglia di inserire il LT pi in profondit. Gonfiare le cuffie a 60 cmH2O con il manometro Universal. Durante la ventilazione, ritrarre lentamente il tubo con le cuffie gonfie fino a che la ventilazione avvenga liberamente (un ampio volume tidalico con una minima pressione sulle vie aeree). Misurare la pressione continuamente per evitare pressione eccessiva nelle cuffie.

A questo punto il tubo laringeo posizionato ed il paziente pu essere ventilato. Controllare la ventilazione polmonare mediante auscultazione, capnografia e seguendo il movimeto del torace. Nel caso in cui la ventilazione non fosse sufficiente, spostare il tubo in direzione distale o prossimale in base alla linea di marker mediana. Utilizzando il biteblock incluso, il tubo laringeo pu essere protetto e fissato in modo sicuro.
