Participant Information and Project Details
Please answer the questions in whatever way meets your needs for full expression and clarity. If you find your response to a particular question is already covered in another question, simply refer us to the relevant answer. Likewise, if you struggle to connect with a particular question, feel free to express that and move on to the next question. We are not expecting detailed responses to all the questions. Section I of this Participant Information is designed both to give us information about you, and to give you an opportunity to engage in a process with yourself. Some people have expressed excitement about the questions and saw them as the beginning of their program and an opportunity for clarity and growth; others have expressed frustration at the level of detail. It has taken people between 1-10 hours to complete, depending on how much they wanted to explore the various questions. This is your choice; we are not expecting more than a few lines for narrative questions.
I. INSTRUCTIONS
Our preference is to receive this form itself by email (preferably as an attachment, so formatting is not lost), and the $150 retreat fee sent via snail mail or at the retreat. If you have access to email: Please send the completed (preferably as an attachment, so formatting is not lost) to occupymanifest@gmail.com If emailing your information will not work for you, please snail mail both your application form and your application fee to the address, 606 Sterling PL ap 1 Brooklyn NY 11238 Attn: Learning in Action Information. NOTE: processing handwritten applications is very challenging for us, so we ask that you make every effort to type your responses. If payment by check will not work for you, please plan to bring cash to the retreat. Note: all of our communications about the program are via email, so it will definitely be a challenge for you not to have access to email during the program. We do not have the staffing capacity to send all communications to individuals via snail mail or by phone, so if you do not have email, you could make individual arrangements with a friend to receive and print the emails for you or find another alternative strategy for staying connected and informed. Thank you so much for taking the time to fill out and submit. After we ve had a chance to review your information, we will send out instructions for setting up a phone discussion the next step in the process. We are looking forward to being in touch with you soon. Warmly, Occupy Manifest (Note: Please make a copy of this application for yourself before sending it in to us.)

III. ACCOMMODATIONS
To help you choose the type of accommodations you d like, please see the section on accommodations in the program description. 1. Please choose for each of the 3 retreats your rooming options. Please RANK the options in the row just below the amounts (see table below). Please use the following ranking system: 1 = first choice! 2 = also a happy choice 3 = not optimal, but it will work (we will only put you in this choice if we can t figure out how else to make things work) 4 = no way y Feel free to use one rank for multiple options (for example, if you are equally happy with all three choices, mark all of them as #1). y We will do our best to accommodate everyone s needs and preferences, and will let everyone know which accommodations they ll be staying in. Notes: (1) We are asking you to pick your choice now for ease and simplicity. However, we will make it possible for you to negotiate different rooming options later if you desire. (2) Availability of single rooms is minimal. Unless you have special circumstances, you can assume that you will be staying in a double/triple room. (3) Please note that February is in the middle of the rainy season in CA, Please choose the camping option for those retreats only if you are an experienced camper and ready to sleep in a tent, or if you would be willing to bedroll if it rains (bedroll means sleeping in a sleeping bag on a carpet in a common room that will likely be used during the day for small group practice).

The level of detail of this section of the document reflects our desire to contribute to as much clarity, flexibility, understanding and connection as possible. We also recognize that the level of detail might be overwhelming for some and may not meet needs for clarity and connection. If you find that you re struggling with this section, please complete as much of it as you can, note any questions you have, send it with your application, and we will contact you to complete the process by phone. The amount we request for participation in the Occupy Manifest in 2012 is the total expenses for the year which are $3500-$4500 per person. (Participants are requested to contribute $150-$300) As we have shared, we have a commitment to including all people, regardless of financial resources. At the same time, we want a fuller sense of partnership and shared responsibility. Therefore, we are asking you to engage with a process with us around the decision of how much to contribute, before making a decision about it. We see this as an opportunity to invite everyone into engagement with their process, awareness of their options, and full choice about the needs they want to attend to. We are excited to hold this information with you and curious to discover what this process will lead to.
2. What considerations are going into your choice about how much to contribute?
4. Would you be able to contribute more if you used a payment plan? What needs may this attend to for you and for us?
5. Have you considered raising funds to cover the program costs or to increase what you are able to contribute? If yes, what are your plans?
6. If you are choosing not to raise funds and are not able to cover the program costs, what needs are you attending to by making this choice? Would your choice change if we could offer you more support with strategies for fundraising?
