SUBMISSION LETTER AND AUTHOR DECLARATION Please complete this letter and declaration, and send it as an email attachment

with your paper or item of correspondence. The submission letter should be sent in either .doc, .pdf or .jpg format. It does not need to be signed. Please enter any free text comments in the field below each statement. Submissions received without this letter will not be processed for review. Before submitting your manuscript or item of correspondence, please use the check boxes below to confirm that you have:
