We request you to undertake the Investigation / Verification / Document Pick Up of this claim and submit your confidential reports to us within a period of 15/10/5 days respectively. Please submit your reports after investigating the claim from all possible angles with specific information made available to us on the following aspects:

Any illness/treatment prior to 19-nov-2011 with Prescriptions/ OPD sheets /case sheets, if any for consultation/ hospitalizations. Check health, habits, income and occupation of Life Assured and Collect evidences for adverse findings. Collect and Verify proof of age/identity/address like copies of ration card, voter identity card, and driving license, Pan Card, Voters List etc. of deceased Life Assured, nominee and her family members. Ascertain age and collect age proof of family members. Check and collect insurance details of deceased Life Assured and her family with us and other insurance companies. Verify the genuineness of age proof submitted at the time of proposal. To verify the genuineness of Death Certificate. Check for treatment details from family doctor Case sheets and complete treatment details for any treatment/ailment taken during the lifetime of Life Assured. Collect photo identity and age proof of deceased Life Assured Translation of documents to English, if collected in vernacular. Any other information/leads, which help in getting further information Check the possibility of suicide.

Please mention this investigation number 60976-15-1-114938 in all reports and bills related to investigation. Record of any reported crime or complaint in the Police Station; whether the life insured had been convicted at any time prior to the date of proposal for any crime; inquire with the local police authorities for reputation of the Life Assured.
