Program Model The program is based on B.W. Tuckmans (1965) Five (5) Step to Team Development. Anyhow, the program will only covers up to Step 4 only.

Program Administration a) Target Group and Participants Open; a maximum of 40 participants per group b) Program Duration 3 Days 2 Nights (Residential) c) Training Investment A training fee of RM 12,000.00 (Ringgit Malaysia Twelve Thousand only) per group. Fees quoted in this proposal does not include training facilities and equipment d) Invoice Training invoice will be submitted to client on the last day of the training session. Payment should be made payable to ACTION DRIVEN SDN BHD not later than 30 days after the issuance of the invoice. e) Accommodation If the program is conducted outstation or on a residential basis, then the accommodation and meals for two (2) of our Consultant(s) should be arranged and borne by the client. f) Transportation If required, return air ticket for the Consultant for training session will be arranged and borne by the client.

g) Training Facilities and Equipment The following training facilities and equipment should be arranged and borne by the client : i. LCD projector ii. Screen iii. P.A System iv. Collar microphone (if applicable) v. Flipchart Papers (30 pieces) and Flipchart Stand vi. Stationery (pen/pencils/colour pencils) and A4 paper vii. Marker Pens (4 colours Black, Blue, Red and Green)

h) Intellectual Property Action Driven Sdn Bhd claims all the rights for program contents, modules and notes. No part of this program may be reproduced, in any form or by any means, without permission in writing from Action Driven Sdn Bhd. Consultants Profile Mohamad Zaini Bin Hj Badron Zaini is a Bachelor of Science in Human Development graduate from Universiti Putra Malaysia. Began his career in the training field, hes involved in training management and coordination for various corporate training programs. His passion in studying the mind and learning technologies have earned him the confidence in delivering training and helped him improve his skills in developing programs. Zaini managed to work closely with international speakers such as Roy Ashworth of SCQuARE International, Valentina Stolar of Aspire International and Steven Lesser of InfoWorks International. With this exposure he continues to focus on improving himself in developing and delivering training modules on Personal Development, Service Excellence, Creativity & Innovation, Leadership and Teambuilding. With his experience of more than 7 years, Zaini has been delivering training for Personal & Professional Development, Leadership & Supervisory, Creativity & Innovation, Service Excellence and Teambuilding for various levels of staff. Zainis impactful, dynamic and lively delivery won the heart of clients such as AIROD, Securities Commission, Malaysia Airports, Petronas Management Training Academy (PERMATA), Selangor Medical Centre, Institut Jantung Negara (IJN), Bank Pertanian Malaysia, Saujana Consolidated, International Islamic University of Malaysia, Syarikat Air Terengganu (SATU) and Radicare. In the international arena, he had conducted training sessions with PT Bank Buana Indonesia Tbk (BBI) and Telekom Brunei Berhad (TelBru) which have benefited from his vast knowledge in Service Excellence Program. With his sound people and management exposure, he managed to blend the two disciplines effectively and relate it to the actual business practice. A professional member of Malaysian Association of Professional Speakers and a certified trainer with Pembangunan Sumber Manusia Berhad (PSMB), he was also appointed as Subject Matter Expert (SME) for the Service Excellence Program during his career with Focus Learning Corporation Sdn Bhd. (2002-2008).

Consultants Profile Zuraifah Zolkepley Zuraifah or prefered to be called June, holds a double degree in Bachelor on Information Systems (Honours) and Bachelor of Applied Computing from University of Tasmania, Australia. She also holds a Master in Information Technology from University Technology Malaysia and wrote a thesis on knowledge management. June spent years of her career attached to public university, focusing on areas such as ICT Trainings, Project Management and ICT Integration. Her love for teaching and training backed by her five (5) years of experience in training, moved her to pursue a career as a trainer or learning facilitator. She able to facilitate various programs especially in ICT, Customer Service and the seven (7) soft skills areas introduced by Ministry of Higher Education for university students and shes not stopping there. As a trainer, she managed to capture her audiences hearts with her animated mannerism and innovative teaching styles. She introduced a lot of modern teaching methods into her classroom, bringing her participants imagination and involvement in the classroom to a new height. Her training session is always fun, interactive and engaging; packed with funfilled games, role plays and activities Still very much a Drama Queen, she introduced the magic of drama, acting and videos into her teaching sessions. Her engaging and charismatic delivery earns high score in training feedbacks from participants. June has experienced in conducting trainings for various faculties and entities at International Islamic University Malaysia (IIUM) which benefited from her knowledge in Service Excellence Program. Her involvement in facilitating also includes Institut Jantung Negara (IJN) and Miyazu (M) Sdn Bhd.
