PERSONAL RELEASE FORM

In consideration of your permitting me to appear in the motion picture now entitled . (the Picture) And for other good and valuable consideration, the receipt and sufficiency of which I hereby acknowledge, I hereby grant to you the following: I hereby grant to you the irrevocable, universal and perpetual right to use, and to grant others the right to use, my actual or simulated likeness, photograph, voice, personal characteristics and other personal identification in all manner and media whatsoever whether now known or hereafter created or devised, in and in connection with, the Picture and any other related productions including sequels, prequels, remakes, serials, adaptations and a making of film or documentary. Nothing contained herein obliges you to exercise any of the rights, licenses or privileges granted to you by the letter agreement. I waive and relinquish all rights and remedies at law or in equity, including moral rights, and release and forever discharge you and all others dealing with you from, and agree not to sue you or any of them, with respect to any claim, cause of action, liability, loss or damage of any nature whatsoever arising out of your exercise of the rights granted herein, including claims relating to defamation or invasion of any right of copyright, privacy, personality or publicity. In no event shall I be entitled for any reason whatsoever to enjoin, restrain or interfere with the distribution and/or exhibition and/or exploitation of the Production or its advertising or publicity I hereby indemnify you against all claims, liability and expense rrespecting this Release. I agree that I shall be entitled to no additional consideration as a result of the exercise of the rights granted herein and that you may rely upon this letter in preparing and exploiting the Picture and any other production.

I acknowledge that the Producer owns and shall own all rights, title and interest (including copyright) in the Recording. I warrant that I have full power to enter into this Release and that the terms of this Release do not in any way conflict with any existing commitment on my part. I acknowledge that I am not a member of SAG, AFTRA, ACTRA or any other similar union or guild. The word `you`as used herein shall mean .. and it`s successors, assigns and licensees.

(IF THE ABOVE SIGNATORY IS UNDER THE AGE OF 18 YEARS, THE PARENT OR LEGAL GUARDIAN OF SUCH PERSON SHOULD SIGN BELOW) I hereby warrant that I am the parent and/or legal guardian of the person who signed the foregoing agreement, that I have allowed said person to execute said agreement, that I will indemnify you against all claims, liability and expense respecting said agreement, and that, knowing of your reliance thereon, I agree to cause said person to adhere to all of the provisions of said agreement.
