INITIAL CLIENT CONSULTATION INTERVIEW FORM

The purpose of an initial consultation is for the Registered Importer to advise you, the prospective
client, of the measures and costs required to import your vehicle. Our position is to render a
knowledgeable and defined opinion for your importation based on legal compliance.
Due to the nature of our business and existing clientele, we do require a consultation fee of $69.95 for a
scheduled consultation with our executive, either over the phone or in person or by written Response Via
Email.
We know your time is valuable, as is ours, so we intend to get back with you quickly all fees charged
will be deducted in the event we are hired. We have been working with Federal Agencies such as: CBP
and Homeland Security, and also the following Importations Laws and Regulating entities: CFR,
NHTSA and the EPA.

One of three outcomes is possible following your consultation:

A. You and the Registered Importer mutually agree to the terms outlined between each
party and a we are hired,
B. The Registered Importer informs you of how and why you cannot import your vehicle,
C. You decide not to use the services of our company, and you may be referred to another entity.

NOTE: Any documents you supply at importation will be photocopied and supplied back
to you. With your permission, we will use certain originals that will be beneficial in the
importation process. Any originals not used will be returned to you at the conclusion of
the importation.

PLEASE READ CAREFULLY & Sign Below Following your initial interview, if you agree to retain
the Registered Importer, and the Registered Importer agrees to represent you, you will both sign an
Importation Agreement for your vehicle. The Importation Agreement for your vehicle will set forth the
terms and conditions of the importation costs and duties of each party.
If the Registered Importer is willing to import your vehicle and you decide not to retain our company to
represent you, we strongly urge you to schedule a second appointment if you have additional questions. If
your vehicle is on the port, the Registered Importer recommends that you immediately consult with other
legal counsel to protect your vehicle. Procrastination in this matter can cost you more then your cars value
and CBP will send you a bill for disposal.

NOTICE: If our office does represent you with regards to the matters set forth by you
herein on this information sheet, or from your consultation, we must receive a written and
signed Importation Agreement between you and the Registered Importer for there to be
any additional costs other than what was paid for the consultation.
If the Registered Importer does not represent you, (this includes not representing you with regards
to the initial importation process set forth by you on this information sheet, or any matters
discussed during consultation), then no additional fees apply.

If your importation problem(s) involve a potential customs violation (CBP), it is important that you
are aware that your vehicle can be seized by customs, and additional costs will occur. Therefore, the
Registered Importer may strongly urge you to immediately retain him/her to protect your rights or for
you to seek an Import Attorney.
The Registered Importers decision to not represent you should be taken as a knowledgeable answer
regarding the inability to import your vehicle.
Your signature only acknowledges that you have reviewed this information and does not
mean that you have retained the Registered Importer.
