TITLE & ACTION Contract Amendment No. 3 Registry of Vacant/Abandoned Properties Neighborhoods Department/Housing and Community Development Division
MOTION
That the contract between the City and the Federal Property Registration Corporation for Registry of Vacant/Abandoned Properties, be amended to exercise the second (2 nd) of three renewal options, extending the period of service from October 1, 2012 to September 30, 2013. All other terms and conditions, as previously amended, shall remain the same. Nothing contained herein shall be amended, modified, or otherwise revised, without prior approval from the PSEC and the Mayor. It is the consensus of the committee that both of the two proposals responding to the Request for Proposal (RFP) were found to be responsive, interested, qualified, and available, to provide the required services. The ranking of first and second designates the order of qualification of these companies to perform the required services and alphabetically they are: 2) Southern Aire Contracting, Inc. 1) Sun City Builders, LLC We recommend the above list is forwarded to the Mayor for final selection so that fee and contract negotiations may begin with Sun City Builders, LLC, the number one ranked firm. That Contract No. 7846-01 between the city of Jacksonville and VT Griffin, for the Management and Maintenance/Caretaker Support Services for Cecil Commerce Center, be amended by: extending the period of service to August 31, 2012. The maximum indebtedness remains the same at $2,711,689.00. All other terms and conditions, as previously amended, shall remain the same. Nothing contained herein shall be amended, modified, or otherwise revised, without prior approval from the PSEC and the Mayor. Fee and Contract Negotiations Traffic Engineering Services Annual Contract Department of Public Works
That Contract No. 8396-03, originally executed March 10, 2010, between the city and Aerostar Environmental Services, Inc., for Contamination Assessment & Remediation Services for Various Projects, be amended by changing the consultants name from Aerostar Environmental Services, Inc., to Aerostar SES, LLC. All other terms and conditions, as previously amended, shall remain the same. Nothing contained herein shall be amended, modified, or otherwise revised, without prior approval from the PSEC and the Mayor. That the City of Jacksonville enter into a contract with Waitz & Moye, Inc., for Traffic Engineering Services-Annual Contract by: (i) incorporating the attached Scope of Services identified as Exhibit A; (ii) incorporating the attached Contract Fee Summary identified as Exhibit B; (iii) providing each project performed under this agreement shall be authorized by an individual Purchase Order with a not-to-exceed cost applicable thereto; (iv) providing the maximum indebtedness to the City pursuant to this agreement shall be a not-to-exceed amount of $250,000.00; and (v) providing the initial period of service shall be for one year, upon satisfactory performance by the Consultant, from execution of contract to October 31, 2013, with two one-year renewal options at terms mutually agreeable. All other terms and conditions are per the RFP and the Citys standard contract language. Nothing contained herein shall be amended, modified, or otherwise revised, without prior approval from the PSEC and the Mayor.
