Reason for Safety Rule Change?

The following are some of the reasons behind a change to a common set of Safety Rules;
IPR/GDF Suez main board has ownership of the Safety Rules (recourse),
Ensure standards identified in the Corporate procedure O&E/H&S PROC/018 Safety Rules & Lock Out Tag Out Procedure are complied with this document (O&E/H&S PROC/018)
is compliant with the GDF Group Rule GR05 Permit to Work System,

Provides opportunity for alignment of all company sites onto one set of Safety Rules (legal), Provides the company with a set of Safety Rules that has a wider application across the global business, Facilitates easier company audit & peer group review.

P018 Company Safety Rules - Conversion training for staff Safety Rules Training

Review process
Technical Review

- The company brought together a number of Safety Rule, Health and Safety practitioners from within the business who along with some Health and Safety Consultants reviewed the technical aspects of the safety rules to bring them up to date.
Safety Distances for testing dead Verbal instructions ROSH procedure All documents now called Permits Approved Procedures Removal of local legislation

- Legal Review - The technically reviewed rules were then placed with lawyers who play a major lead safety protocol processes, their review led to
Chronologically Logical Structure Format Drawings

P018 Company Safety Rules - Conversion training for staff

Why has all this been done?

To update the rules. To provide a technically correct set of rules. To provide a legally defendable set of rules do
not forget it will probably initially be a Safety Controller or Designated Person standing against the legal system!

Safety Rules Training

2.1

PHILOSOPHY OF THE SAFETY RULES

2.1.3

These electrical and mechanical Safety Rules are based on a philosophy that the Safety Rules should briefly and clearly specify those actions which must be implemented and identify those practices which should be followed, to establish conditions in which persons who have to carry out work on or adjacent to the Plant and Apparatus will be safeguarded from the inherent Dangers and provided with Safety from the System. Every person when doing work should expect that a Safety Document is required unless determined otherwise by an appropriate person nominated in writing by the Location Manager.

2.1.4

P018 Company Safety Rules - Conversion training for staff Safety Rules Training

2.1

PHILOSOPHY OF THE SAFETY RULES

2.1.5

Whenever work is carried out affecting Plant and Apparatus which is part of the System, two types of Danger may arise:
Danger inherent in the System arising from the design function of the Plant and Apparatus. The Safety Rules, when implemented, will achieve the safety of persons at work from these inherent Dangers at the commencement of and during the course of the work;

I.

II.

Danger arising from the environment at and in the vicinity of the work point and not associated with the System. These Safety Rules are not designed to specify the means of establishing General Safety from this second type of Danger, which may arise whenever work is done.

P018 Company Safety Rules - Conversion training for staff Safety Rules Training

2.1
2.1.6

PHILOSOPHY
To carry out work affecting Plant and Apparatus within a System, the procedure to be observed may be divided into the following stages:
making available the Plant and Apparatus concerned for the work required; establish conditions to safeguard persons from the inherent Dangers of the System; execution of the work required; clearance of the Plant and Apparatus on completion or termination of the work; restoration of the Plant and Apparatus to its normal conditions within the System.

(i) (ii) (iii) (iv) (v)

PHILOSOPHY
To carry out work affecting Plant and Apparatus within a System, the procedure to be observed may be divided into the following stages:

P018 Company Safety Rules - Conversion training for staff Safety Rules Training

2.1
2.1.7

PHILOSOPHY OF THE SAFETY RULES

To achieve safety within the stages specified above, these Safety Rules require that defined persons be given responsibilities for:

I. II.

III. IV.
V.

VI.

Identifying and assessing all foreseeable risks from System derived hazards. Implementing control measures to reduce all risks to a level which as a minimum satisfies all current legislative requirements. Establishing safe conditions for persons to work on the Plant and Apparatus; Either checking that safe conditions have been established for work on Plant and Apparatus which has been Isolated from the System, or Identifying the appropriate specialised procedures which will be applied when work has to be done on Plant and Apparatus which remains energised, and then to authorise, in writing, the commencement of work, and finally to cancel the written authority on termination of the work; Receiving the written authority to commence work and executing the work in a safe manner by suitably nominated competent personnel, thereafter to supervise General Safety during the course of the work and to clear the written authority when the work is terminated.

P018 Company Safety Rules - Conversion training for staff Safety Rules Training

2.1
2.1.8

PHILOSOPHY OF THE SAFETY RULES

The Safety Rules for achieving the safety of persons at work from the inherent Dangers of the System are limited therefore to specifying:

(i) the actions necessary to ensure safety during each of the stages above, in which Dangers may arise from the design function of the Plant and Apparatus; (ii) the responsibilities of persons for ensuring safety during each of the stages above from Dangers which may arise from the design function of the Plant and Apparatus, and, in relation to the general Dangers arising whenever work is performed, the Safety Rules are limited to: (iii) identifying the person responsible for achieving safety from these general Dangers.
P018 Company Safety Rules - Conversion training for staff Safety Rules Training

2.1

PHILOSOPHY OF THE SAFETY RULES

2.1.9 The Safety Rules will be supported by Codes of Practice and Local Procedures to specify instructions for implementing the Safety Rules effectively and efficiently and to ensure that the Safety Rules are applied in a consistent manner at identified locations throughout the Company. 2.1.10 The Safety Rules procedure will be enacted by persons who are authorised in writing for the appropriate functions that they are undertaking. Any person carrying out a function under the Safety Rules must be able to discuss associated Safety Rules issues with his line manager. This does not preclude communication on any other issues related to safety that may arise. All such issues should initially be communicated to a Safety Controller. 2.1.11 The application of the Safety Rules and their associated procedures shall be monitored and formally audited. This will be undertaken at regular intervals both by staff internal to and external from the asset in order to assess compliance with and identify any failures in the application of the Safety Rules.

P018 Company Safety Rules - Conversion training for staff Safety Rules Training

2.2

PRINCIPLES OF THE SAFETY RULES

2.2.1 To fulfil the requirements of the philosophy, the following principles have been adopted in formulating the Safety Rules:

(i) the Safety Rules are concerned only with achieving safety for persons.
(ii) when work is to be carried out on High Voltage Apparatus, the primary means of achieving safety is by isolation from the System(s) followed by earthing. No work shall be carried out near Live HV Apparatus unless it is unreasonable for it to be Dead. If this work involves the testing of Live HV Apparatus, the means of achieving safety shall be by the application of specialised Approved procedures, which shall include suitable precautions to prevent injury. (iii) in the case of Low Voltage Apparatus, the primary means of achieving safety is, if reasonably practicable, by isolation from the System(s). If isolation is not reasonably practicable, safety is achieved by the application of specialised Approved procedures. No work shall be carried out on or adjacent to Live LV Apparatus unless it is unreasonable for it to be Dead, reasonable for a person to be working while it is Live and suitable precautions are taken to prevent injury;

P018 Company Safety Rules - Conversion training for staff Safety Rules Training

2.2.1
(iv)

PRINCIPLES OF THE SAFETY RULES

when work is to be carried out on Plant, the primary means of achieving safety is by isolation from the System(s) followed by draining, venting, purging and control of stored energy as appropriate, except when the work requires the Plant to be energised. For these exceptions the means of achieving safety is by the application of specialised Approved procedures; the fundamental means of protecting persons at work is the application and maintenance of the primary means of achieving safety specified in 2.2.1 (ii), (iii) and (iv) supported by appropriate actions to maintain the effectiveness of the primary means, e.g. locking off Isolating Devices; the nomination of persons to carry out defined requirements under the Safety Rules is formal, although part of their normal responsibilities; the application of the Safety Rules shall ensure that a safe situation exists across all control area boundaries and operational interfaces, be they totally or partially within the jurisdiction of the Company,

(v)

(vi)

(vii)

P018 Company Safety Rules - Conversion training for staff Safety Rules Training

2.2.1
(viii)

PRINCIPLES OF THE SAFETY RULES

to achieve Safety from the System, that is, from Dangers which may arise by design of the Plant and Apparatus, each of the five stages referred to in the philosophy 2.1.6 will involve one or more of the following functions: (a) Consent which includes: instructing actions to implement precautions before work commences, and consenting to the cancellation of the Safety Document and instructing actions to restore Plant and/or Apparatus to service; (b) Making Safe/Restoration of Plant and/or Apparatus which includes: before work commences, taking actions to make Plant and Apparatus safe for work and issuing a Safety Document; after completion of work and the cancellation of the Safety Document, taking actions to restore the Plant and Apparatus to service and/or; (c) Work which includes: receipt of safety document, execution of the required work to its completion or termination and, after the work area has been cleared, clearance of the safety document.

P018 Company Safety Rules - Conversion training for staff Safety Rules Training

2.2
2.2.2

PRINCIPLES OF THE SAFETY RULES

The above three functions cover separate responsibilities which are distinct from each other and are treated distinctively in the Safety Rules. The Safety Rules do not state the number of persons necessary to discharge the three functions. However, in order to implement the Safety Rules efficiently, it will frequently be necessary for two or more persons to perform the three separate functions because of technical and geographical complexities in the Companys system. The Safety Rules do not preclude one individual from personally performing all three functions, to his own instructions, as identified in Principles 2.2.1 (viii) above.

2.2.3

2.2.4

P018 Company Safety Rules - Conversion training for staff Safety Rules Training

Summary - Principles
To fulfil the requirements of the Philosophy, the following principles have been adopted in formulating these Safety Rules;
1 2 3 4 5 6 7 8

Safety Rules only concerned with Safety of Persons, For work on HV - Isolation required, with confirmation followed by Earthing, For work on LV - Isolation required where reasonably practicable, if isolation not possible then justify live working For work on Plant - Isolation required, draining, venting, and if needed, purging and/or control of stored energy, Primary Precautions to be maintained (Locked & Tagged), The nomination of authorising persons is Formal, Application shall ensure a safe situation exists across boundaries, Safety from the system will require the functions of consent, making safe/restoration and, work.

GENERAL PROVISIONS
Suitable and sufficient risk assessments shall be carried out prior to the commencement of work in order to identify and assess any potential hazards. These Safety Rules are provided as a control measure to ensure that work can be undertaken on the System without Danger from the Plant and/or Apparatus.

4.2

Compliance with the Safety Rules ensures that working on Plant and/or Apparatus to which these Safety Rules apply can be undertaken without hazard from the System. If the hazards cannot be controlled by exclusion then suitable control measures must be put in place to minimise the risks from the hazards to an acceptable level.

4.3

Every person when doing work should expect that a Safety Document is required unless they are told otherwise by an appropriate person nominated in writing under the Safety Rules.

4.4 Shall

INTERPRETATIONS
When shall is used in these Safety Rules with no qualification, this indicates a mandatory requirement with no discretion permitted and no judgement to be made. (You will do it) When shall, where practicable is used to qualify a requirement, a slightly less strict standard is imposed. It means that where it is possible to achieve in the light of current knowledge and invention, then the requirement must be met. One is not allowed to avoid the requirement on the grounds of cost (money, time, trouble and physical difficulty). (If you can do it, do it) When shall, where reasonably practicable' is used to qualify a requirement then a judgement must be made as to what is reasonable, taking into account a balance of the magnitude of the risk (likelihood and severity) against the cost (money, time, trouble and physical difficulty) of avoiding the risk. (A balanced decision to be made) When used in these Safety Rules, the term work includes testing. Where any reference to the male gender is made within the rules the female gender also applies equally.

Safety Rule Technical Update

An issue raised had been identified, some voltage measuring devices are less than 800mm in length, such test sticks are commercially available. An investigation identified that such test sticks were acceptable to international standards (ISO) and the company may in the future, not be able to procure HV test equipment of a longer length. Hence and additional table has been included in the Safety Rules with caveat about usage only when testing dead following application of isolation.
P018 Company Safety Rules - Conversion training for staff Safety Rules Training

6.1

APPROACH TO EXPOSED HIGH VOLTAGE CONDUCTORS / INSULATORS

6.1 APPROACH TO EXPOSED HIGH VOLTAGE CONDUCTOR/INSULATOR 6.1.1 Persons shall not allow any part of their body to approach exposed conductors designed for and operated at, High Voltage, or insulators supporting such conductors, within the specified Safety Distances, specified in table 6.1.4 (a) unless the conductors have been Isolated and Danger has been excluded. The only exception is when it is necessary to carry out testing (see 6.1.3) to prove a circuit Dead prior to the application of Primary Earths, where following completion of the isolation and prior to the application of the Primary Earths the Safety Distances specified in table 6.1.4 (b) shall apply.
6.1.2

When exposed High Voltage conductors are not Isolated, the only objects permitted to approach them, or insulators supporting them, within the Safety Distances specified in table 6.1.4 (a), shall be Approved insulated devices. When exposed conductors are Isolated but could be subject to High Voltage, the only objects permitted to approach them, or insulators supporting them within the Safety Distances specified in table 6.1.4 (a), shall be Approved Voltage Measuring Devices or Earthing Devices. When using Approved Voltage Measuring Devices, persons shall not allow any part of their body to approach exposed conductors within the Safety Distances stated in table 6.1.4 (b).

Safety Rule Technical Updates

1. As in life circumstances associated with Safety Rules have moved on. It is important in these days of litigation that;
anybody instructing persons to carry out work records the requirements of the required actions and gets the instructed person to sign his acknowledgement of the precautions required.

2. All documents are now called Permits, because even by issuing an Access document you are permitting persons to carry out work.
The Permit (Access) allows for the recording of limitations imposed to achieve safety from the system not requiring the application of an isolation.

P018 Company Safety Rules - Conversion training for staff Safety Rules Training

6.2

SAFETY PRECAUTIONS FOR WORK ON OR ADJACENT TO PLANT & HV APPARATUS

6.2.1

Where Safety from the System is required and this can be achieved by limiting the work or the work area then instructions defining the limits shall be confirmed in writing and a Permit (ACCESS) issued.

Safety Rule Technical Update

Some locations have tried previously tried to circumvent the need for Permit ROS/ROMP and associated procedures, by arguing that as they were not using the installed supply to move some item it was not a restoration of supply. The ROSH process looks at the process from the other end - the restoration of the hazard - rather than what type of power supply is being used i.e. the restoration of, normally, a movement such as rotation/linear movement THE HAZARD.

P018 Company Safety Rules - Conversion training for staff Safety Rules Training

6.2.3 RESTORATION OF SYSTEM HAZARD

When work is to be carried out on Plant and/or HV Apparatus and it is essential to restore a System hazard to facilitate that work, Safety from the System shall be achieved and maintained as follows: i. The work shall be detailed in an Approved written Restoration of System Hazard (ROSH) procedure specifying the requirements necessary to maintain Safety from the System and prevent Danger whilst the System hazard is restored. A Permit (ROSH) shall be prepared, taking into account the requirements outlined in section 6.2.2 (i) to 6.2.2 (viii) above, along with the associated ROSH procedure and only those keys identified within the ROSH procedure.

ii.

iii. No other Safety Document shall be in force on the same item of Plant and/or HV Apparatus whilst the Permit (ROSH) is in force.

P018 Company Safety Rules - Conversion training for staff Safety Rules Training

6.2.3 RESTORATION OF SYSTEM HAZARD

Some sites change the Hands Off tag to a specialised R.O.S.H. tag. Clear to staff and contractors which Precautions which may be varied by the Acceptor under the Permit ROSH. Identifies to other staff and contractors why a point of isolation may not be locked in the isolated position.

P018 Company Safety Rules - Conversion training for staff Safety Rules Training

Safety Rule Technical Update

All procedures that are used directly with Safety Documents shall be APPROVED. An Approved procedure shall be issued with a Permit (ROSH), (TEST) or(LIVE). Approved means sanctioned and signed by the Location Manager. Provides the Safety Controller with support provided the Safety Controller carries out what is identified within the Approved Procedure. It means that the process has been sanctioned by Location Manager.

P018 Company Safety Rules - Conversion training for staff Safety Rules Training

6.2.4 TESTING
When testing is to be carried out on Plant and/or HV Apparatus and it is essential to introduce testing supplies and/or remove isolation and/or the removal of Primary Earths to facilitate that testing, Safety from the System shall be achieved and maintained as follows:

i.
ii.

The scope of testing shall be detailed on an Approved test instruction.

A Permit (TEST) shall be prepared, taking into account the requirements outlined in section 6.2.2 (i) to 6.2.2 (viii) above, along with only those keys necessary to facilitate introduction of a testing supply and/or remove isolation and/or the removal of Primary Earths. Only testing, and associated work of a minor nature, that is essential to the completion of testing, may be carried out under a Permit (TEST). No other Safety Document shall be in force on the same item of Plant and/or HV Apparatus whilst the Permit (TEST) is in force.

SAFETY CODES OF PRACTICE

A corporate suite of documents that will amplify the requirements of the rules for specific items of plant and apparatus and also for general application of the Safety Rules. The appropriate documents are then used to generate local procedures for use with the Safety Rules. Locally these are often called: Procedure Health & Safety Local Safety Instructions Safety Rule Instructions Company Safety Instructions etc. The intent of the COP must not be diluted as it passes into the local documentation, the local procedure can only confirm or enhance the requirement of the COP.

SAFETY CODES OF PRACTICE

An incomplete suite and not all of these will be issued at any specific asset, there are 34 in total (including the Definitions document), but some are irrelevant to certain plants e.g. PF Code of Practice. The Codes can be localised but this must not dilute the corporate document, if no localised version exists then the corporate document must be complied with.

These documents can be found on the company Connect web site or via the Safety Rules and Lock Out Tag Out webpage in the H&S portal

SAFETY CODES OF PRACTICE Guidance Notes

Presently a set of four documents that will give locations guidance associated with the administrative assistance of applying the Safety Rules. The Guidance Notes need not be complied with at a site but a similar, or better, system must be in place to manage these supporting requirements polite way of saying each site must have something in place that covers each area, but it may be as part of another document.

These documents can be found on the company Connect web site or via the Safety Rules and Lock Out Tag Out webpage in the H&S portal.

O&E/H&S CoP/018/040 Appointment of Persons. O&E/Company Safety Rules/GN Guidance Notes for Company Safety Rules. Both provide overarching guidance which is carried forward locally into local document relating to the Appointment of Persons.

Conversion authorisation format shall be decided locally, due to differences in breadth of changes required being different at each location. The agreed format should be risk assessed and following successful completion of the agreed conversion route a new authorisation certificate must be prepared and issued identifying the limitations of the authorisation.
For ex-GDFS sites it has been decided that due to the differences between current site safety protocols and the Company Safety Rules a full authorisation interview in line with H&S CoP/040 will be required.

Safety Rule Authorisation

When things go wrong it will initiate some form of incident report. This is considered and if necessary the findings are shared though either a Safety Communications Note (SCN) or a Safety Information Memoranda (SIM), similar to META REX.

These will allow a sharing of the failure of plant, failure of process, failure of person, it will identify the cause and suggest a methodology of preventing a re-occurrence. For the Rye House incident even before the video we had issued a SIM 063
