GENERALIDADES
The interaction between amphotericin B (A) and ergosterol (E) in the fungal cell membrane is consistent
with the formation of pores with a diameter of approximately 0.8 nm. Amphotericin molecules would line
the inner surface of the pore.These pores permit leakage of intracellular contents. The interaction
between the echinocandins and glucan synthase leads to reduced synthesis of -(1,3)-glucan, a crucial
component of the interwoven glucan polymers that form the fungal cell wall. Azoles act within the cell to
inhibit 14--demethylase and to reduce synthesis of ergosterol, a sterol that is crucial to membrane
function.

action of ERG3 on 14 a
-methylfecosterol.

tratamiento de induccin durante 2 semanas
con anfotericina B flucitosina.

Alternativa vlida a fluconazol en el tratamiento de la
infeccin orofarngea por Candida (muguet).

Eficacia en tratamiento de candidiasis sistmica no se
ha estudiado de forma sistemtica.
Aspergillus
No existen estudios comparativos vs. anfotericina B en
el tratamiento de aspergilosis invasiva. Los estudios no
comparativos ms importantes obtuvieron una
respuesta favorable (completa o parcial) en el 39 y
63% de los pacientes, respectivamente.
Itraconazol IV seguido de itraconazol VO en
aspergilosis invasiva, con respuesta favorable en 48%.
Consolidacin" de los pacientes con aspergilosis
invasiva tras el tratamiento inicial con anfotericina B.

Alternativa a fluconazol en el tratamiento de
"consolidacin" de meningitis criptoccica, tras
induccin con anfotericina B.
Prevencin de micosis invasiva en inmunodeprimido
Transplante alognico de mdula sea, administracin
secuencial de itraconazol intravenoso seguido de la
solucin oral del frmaco fue ms eficaz en la
prevencin de la IFI que fluconazol.
Transplante de hgado: itraconazol no fue ms eficaz
que fluconazol en la prevencin de la IFI y produjo,
adems, un mayor nmero de efectos secundarios.
La experiencia de administracin profilctica de
itraconazol en otro tipo de trasplantes es anecdtica.
ITRACONAZOL
Tratamiento emprico de la neutropenia febril
ABD e itraconazol por va intravenosa seguido de solucin
oral en neutropenia y fiebre que no responde a un
tratamiento antibitico de amplio espectro: eficiacia similar.
Itraconazol y anfotericina B en la prevencin de la IFI en
estos pacientes, la mortalidad fue similar en ambos grupos
y la eficacia global de ambos frmacos no fue
significativamente diferente. Itraconazol se asoci con
menos efectos secundarios (19% de abandonos por
toxicidad entre los tratados con itraconazol frente al 38%
en el grupo de anfotericina B).

Tratamiento de eleccin esporotricosis, la blastomicosis,
cuando no existe afectacin del SNC,
paracoccidioidomicosis, faeohifomicosis e histoplasmosis en
pacientes inmunocompetentes en ausencia de afectacin
