usually as
important
as physical
examination

The purpose of History


Taking
Soon after pregnancy is confirmed,
a full history is taken from the woman in order
to assess her present state of health and
to detect any factors which may affect the
course or outcome of pregnancy.

So
It will identify any physical, psychological or
social needs and from the basis of her care plan.

History taking may be

stressful for the

women.
It necessitates a number of personal and
searching
questions which should include an exploration of
why
the women is pregnant, her attitudes to childbearing

Mrs. Anthony : Good Morning, maam!


Midwife Anne : Good Morning, Mrs.Anthony. How are you doing today?
Mrs. Anthony : Im not feeling well, actually. I have caught flu for about
three days.
Midwife Anne : Yes, I can see your face is pale. Have you had any
complanits ?
Mrs. Anthony : Yes, of course. I always sneeze and cough all the time.
Ive also headache and they all make me suffering.
Midwife Anne : Have you taken any medicine, Mrs. Anthony ?
Mrs. Anthony:No. I havent. I dont want to take any risk for my baby.
Thats why I come here to see you.
Midwife Anne: Have you worked hard all day long every day ?
Mrs. Anthony: Yes, I have. Actually, I was very busy last week. I hardly
could have rest and I thought I was very exhausted and my meal
schedule was irregular as I still vomit whenever I finish having meal.

Midwife Anne : I suggest you not to work too hard, Mrs.


Anthony. Im afraid there will have something wrong with your
baby. By the way, is there anyone in your family who had ever
got miscarriage?
Mrs. Anthony : Well, my mother had twice miscarriage if Im not
mistaken.
Midwife Anne : I see. And you had swollen ankle or hand or
high fever ?
Mrs. Anthony : I think I do not have such complaints, maam.
Midwife Anne : Alright, Mrs. Anthony. Ill check your abdomen,
blood pressure and weight and then we are going to see your
fetus development through USG.
Mrs. Anthony : I hope there is nothing serious that happens to
me, maam.
Midwife Anne : Dont worry, maam. We are going to find out
what bothering you.
