Chronic aspecific LBP
It has been shown that pain can alter the amplitude of muscle activity during
movement (Arendt-Nielsen et al., 1996; Madeleine et al., 1999; Zedka et al., 1999).
However, our group has also shown that pain alters the strategy by which the brain
prepares for movement: experimental back pain elicited by intramuscular (i.m.)
injection of hypertonic saline selectively delays anticipatory postural activation of the
deep muscles (Hodges et al., 2003b). The same effect has been observed in people
with chronic back trouble, even though they were pain free at the time of testing
(Hodges and Richardson, 1996). Extensive theoretical findings suggest that the new
postural strategy serves to stiffen the spine and is probably protective. However,
those findings also suggest that two
aspects of the protective strategy are problematic if maintained long term. First, the
primary role of the deep trunk muscles lies in fine-tuning of spinal control, which
means the new postural strategy is probably associated with a reduction in spinal
control. Secondly, the superficial trunk muscles impart stiffness to the spine via
increased compression, and a sustained
increase in loading of the spine is thought to stimulate nociceptors in spinal
structures and predispose to injury (Gardner-Morse and Stokes, 1998; van Dieen
et al., 2003).

Dolore e cronicizzazione
Sappiamo che ci sono diversi tipi di
risposte allo stimolo doloroso: una
risposta immediata, che spesso scatena
un riflesso midollare, monosinaptico,
tipico della fase acuta, quale pu
manifestarsi dopo un trauma, ed una
risposta mediata a vari livelli del SNC,
polisinaptica, (Zedka 1999) che certo
mostra le caratteristiche adattative,
economiche della cronicizzazione.
Dolore e Cronicizzazione
I movimenti risultano ridotti di
escursione, impacciati, ben presto si
manifestano quelle alterazioni
strutturali che portano a fenomeni
distrofici, perdita di qualit dei
tessuti, rigidit che a loro volta si
avvolgono in un meccanismo a
spirale, autoproducentesi fino
allanchilosi.
Cronicizzazione ed Entropia

cambiamenti patologici si strutturano
verso lirreversibilit, il paziente scivola

Il paziente attraverso il circuito virtuoso della
terapia, per mezzo del contesto terapeutico, non
solo vede ridursi gli elementi della sua specificit
patologica, ma pu cominciare a ricomporre la
sua IMMAGINE CORPOREA, alterata dal disturbo,
dalla deafferentazione, dalla semplificazione delle
strategie motorie difensive messe in opera nella
maggior parte delle patologie di interesse
riabilitativo. Per mezzo della relazione, del

limite, e con esso ritrova l' ambiente esterno, lo
spazio con le sue coordinate, i suoi vincoli e le

Abstract
Patients with chronic low back pain have a worse posture, probably related to poor control of
the back muscles and altered perception of the trunk midline.
The aim of this study was to evaluate the efficacy of a perceptive rehabilitation in terms of
stability and pain relief in patients with chronic nonspecific low back pain.
Thirty patients were enrolled and randomized into two groups: 15 patients received
rehabilitation, on the basis of a specific tool to perform perceptive exercises [perceptive
group (PG)], and 15 patients received a back school programme [back school group (BG)].
Both groups were assessed using stabilometry and the McGill Pain Questionnaire
before and at the end of treatment.
For the reference values of stabilometric parameters, 15 healthy individuals were enrolled.
Significant reductions in sway length (P=0.019) and laterolateral sway velocity (P=0.038)
were observed in the PG. The anteroposterior sway velocity was reduced in both the groups,
but significantly only for BG (P=0.048). The percentage of sway length reduction was
inversely and significantly correlated with the initial sway length value for PG (R=-0.708,
P=0.003), but not for BG (R=-0.321, P=0.243). In the PG, the sagittal arrows and bi-acromial
and bi-spinoiliac lines' angles were all significantly reduced. General pain relief was reported
after treatment, without a significant difference (P=0.436).
Our results suggest that a perceptive rehabilitation can improve the postural
stability for the realignment of the trunk, controlling the back pain. The use of
cognitive exercises may strengthen the usual rehabilitation of low back pain,
avoiding the recurrence of symptoms.
