with the NORFIL Foundation.

assistance for the treatment
of the mentally handicapped.

LGU-Santa Assistance
i. If the patient is a non-PHIC member, the LGU-Santa shall endorse
the patient to be enrolled with the LGU-Santa PHIC Point-of-Care
program; and
ii. Maximum of two (2) Chemotherapy sessions fully paid by the
LGU-Santa.
i. If the patient is a non-PHIC member, the LGU-Santa shall endorse
the patient to be enrolled with the LGU-Santa PHIC Point-of-Care
program; or
ii. If the patient exceeded hemodialysis or peritoneal dialysis
sessions to be covered by the PHIC insurance, the LGU-Santa shall
pay for a maximum of four (4) dialysis sessions.
Annual financial assistance of five thousand pesos (PhP5,000.00)
If the patient is a non-PHIC member, the LGU-Santa shall endorse
the patient to be enrolled with the LGU-Santa PHIC Point-of-Care
program; and
ii. A medicine voucher assistance or medical diagnostic or hospital
guaranty not to exceed five thousand pesos (PhP5,000.00) per
annum.
