PATIENT MEDICAL RECORD SYSTEM FOR LYCEUM
OF THE PHILIPPHINES UNIVERSITY-CAVITE
HEALTH AND SERVICES DEPARTMENT

 Importance and Significance

The Health and Services Department of Lyceum of the

Philippines University- Cavite is a health-care facility dedicated to
the provision of holistic, comprehensive, and quality care to all
students and teaching and non-teaching personnel. It supports
the university's mission in providing equity access to learning
through health education and counseling provided during
consultation and admission.
 Importance and Significance

Currently, this department operates a manual records

management system. This capstone project aims to introduce an
automated system, in which computers will be used to achieve
more accurate and easy record management. Having this system
will provide a time saving and ease retrieval of patients’
information. This will be a windows-based program that will
ensure adequate security to patients records in which only
authorize users can have access to the system with username
and password. The program will also provide auto-sort records to
easy retrieval of patients’ information.

This study will benefit the following:

• Lyceum of the Philippines University-Cavite Health and

Services Department

The school nurses and doctors will have an easy access to review
and evaluate personal information, health records and general
health conditions of their patients through this system. The auto-
sort records feature of the system will save them time to retrieve
all the information they need immediately. Therefore, this
system is believed to further improve their data management for
a better-quality service.
 Importance and Significance

This study will benefit the following:

All students and teaching and non-teaching personnel as
patients of the Health and Services Department will experience a
better-quality service since this system will provide the
department an efficient method for faster accommodation.

This study will benefit the following:

• Parents/Guardian
Parents will be easily notified about their child’s condition if
diagnosed with any sickness inside the school premises. Persons
in the department can call or send a message directly to
parents/guardian since their contact numbers can be easily
found through this system. Noting that the information provided
will be treated with confidentiality.

This study will benefit the following:

This capstone project will contribute to the academe for its
newly constructed system that will help one of its department to
ensure a better-quality service to increase its productivity.
 Scope and Limitation

The patient medical record system will be constructed by the

proponents only for the use of the Lyceum of the Philippines
University-Cavite Health and Services Department. This capstone
project will be limited to securing students’ personal
information, health records and general health conditions such
as certain immunizations, vision, hearing and scoliosis screening,
dental, health action plans and routine clinic visits and providing
auto-sort records for an easy retrieval of patients’ information

To develop a patient medical record system for

Lyceum of the Philippines University-Cavite
Health and Services Department. Specifically,
the project consists of the following features:
 Expected Outcome

• An easy access and review of all the patient’s files available

in the clinic.
• Provides a back-up of patients’ health card which contains
the name, age, status, sex, date of birth, status, address,
religion. As for students, their student number, course,
parent/guardians name, address of guardian and telephone
number will also be provide
• Contains the medical examination checklist. This comprises
of Past Medical History, Personal History, Family History,
Ob/Gyne History (for female students). All general health
conditions of individual students will be consolidated.
 Expected Outcome

• Includes the data of dental examination as written in their

dental card.
• An auto-sort button that delineates diagnosis and remarks for
each patient as per review of the medical examination.
• Provides list for requirements that were not yet submitted
upon the request of a personnel in the clinic.
• Easy printing and exporting of files. All files containing
relevant information needed by the Health and Services
Department for any purposes can be readily printed directly
from the system. Moreover, files can also be readily exported
to the computer or via email.
 Expected Outcome

• Access by only authorized users. Only the authorized

personnel from the Health and Services Department can have
an access to the system.

• Microsoft Visual Studio C# and VB.Net

• MySQL for the database.
