Contact with Anchored Vessel

• One vessel was recently involved in a contact


incident with another vessel at anchor.

• Own vessel was in ballast condition, bound for


North Channel (East of Johor) outer port limit
area to carry out tank cleaning operation.

• The Bridge Team was comprised of Master,


2/Off and a Helmsman.
• While progressing on passage, Own Vessel
course was adjusted to clear anchored vessel
which was in close proximity to the planned
track.

• While the course alteration was in progress,


the Master noticed a Fishing Vessel which was
acquired that very time and found close south
of the Opponent Vessel.

• Bridge Team assessed that Fishing Vessel was
crossing from Starboard to Port basis data
read out from ARPA.

• All 3 Radar on board was set to speed over


water mode, Bridge Team failed to verify
targets actual status using speed over ground.
• During this critical period where alteration of
course was in progress, OOW completed their
handing over. Both OOW did not appreciated
developing close quarter situation and set &
drift due to prevailing tidal condition.
• Master decided to head towards the stern of the
Fishing Vessel and reduced the rate of the port
turn and speed.

• Action to avoid close quarter situation with


Fishing Vessel resulted in another close-quarters
situation.

• Due to prevailing current, own vessel resulted


setting towards opponent vessel and her
Starboard side tank 4S and 5S WBT came in
contact with the port bow of the opponent
vessel.

Learning points
• Vessel did not proceed at a safe speed so that she
can take proper and effective action to avoid
collision.

• Vessel did not maintain a proper look-out


appropriate in the prevailing circumstances and
conditions so as to make a full appraisal of the
situation and of the risk of collision

• Target Data on Radar/ARPA was misinterpreted,


stationary vessels were assessed as moving
through water.
Learning points
• Vessel did not made proper use of radar equipment,
particularly when approaching a vessel at close range,
vessel had 3 Radars, none of them were used in ground
stabilized mode thus enhancing the observer’s
situational awareness

• Handover between OOW took place when a critical


maneuver was imminent.

• Information of set & drift (COG & SOG) was apparently


not appreciated while vessel was carrying out critical
maneuver
