MANEJO PRE ENDOSCÓPICO EN PACIENTES CIRRÓTICOS
Hemorragia variceal
Fármacos vasoactivos
Terlipresina
Somatostatina
Octréotide
Endoscopia
Vapreotide
Recomendaciones:
Menor sangrado a la - Iniciar con sospecha de etiología
endoscopia variceal
Mejor control del - Terlipresina 2 mg c/4 h
sangrado a las 12 horas - Somatostatina 250 ug bolo + 250-
500 ug/h
- Octreotide 50 mcg bolo + 50
mcg/h
3-5 días

Dos ámpulas (80mg) en 100 ml de solución salina
0.9% o glucosada 5% pasar a 10 ml/hora (8mg/hora)
80 mg - 100 ml solución
8mg - 10 ml
Octreótide ámpulas 1 mg
Una ámpula (1 mg) en 100 ml de solución glucosada
5% pasar bolo de 5 ml y posteriormente 5 ml/hora

Manejo endoscópico
Endoscopia temprana = primeras 24 horas
• De elección
• Alta sensibilidad y especificidad para
localización de lesiones
hemorrágicas
• No existe un tiempo específico
• Segunda endoscopia 16-24 hrs
posterior a inicial no se recomienda
de rutina
• Hemorragia variceal = 12 h

management of acute peptic ulcer bleeding. Lancet 2013; 381: 2033–43
Manejo
endoscopico
En 10-20% los pacientes no responden a tratamiento endoscopico
ni farmacologico.
Terapia de rescate
Terapia de rescate
• Sonda Sengstaken-Blakemore logra detener 40-90% sangrado
• Balón esofágico: 30-40 mmHg 50-100 ml
• Balón gástrico: 200-300 cm de aire
• Propanolol 20 mg VO cada
12 horas
• Nadolol 40 mg VO cada 24
horas
• EL riesgo del resangrado variceal
en los primeros 2 años es del 60%,
con una mortalidad del 35%
• Betabloqueador selectivo +
mononitrato de isosorbide.
