Physical Environment:
-The community is well-kept and clean. There is a beautiful pathway of flowers at the entrance that
captures visitors attention

-Different sections of housing are available including independent living, assisted living, and a
medicaid approved nursing care unit

-There are bird cages and bird baths on the top floor that residents can view

-There is an arts and craft station

-Auditorium for entertainment and activities

-Handmade gifts created by residents are for sale in the gift shop

Health & Social Services: Most residents take scheduled medications for their chronic illnesses which
is given by nurses, med techs and in-house providers. Transportation is available for outside provider
appointments, podiatrists, dental services, etc.
Upstream Factors
Economy: Residents have a need for long term care and are of low income.

Transportation & Safety: There are sidewalks, rails, and emergency buttons to help
residents with safety. The residents use walkers, wheelchairs, and other assistive
devices to help with fall prevention. The building is easily accessible for wheelchairs
both indoors and outdoors and is equipped with multiple ramps.

Education: There is a library on site and a computer for residents to use. Books are
donated to the facility for residential use as well. These are great resources for
residents to exercise their cognitive function.

Reflect on Community
General Appearance: This facility is such a beautiful community that is well maintained
and very clean!

Strengths include the undefining love the caregivers have for the residents. Staff
members are constantly engaging with residents and encouraging them to interact and
keep them going!

Weaknesses include the nuns having to slowly close down different facilities because
there are not enough oncoming nuns to “take over” and continue to run the facility
following the mission of the Sisters.
-Another weakness is that many patients seemed “bored” or uninterested in participating
in activities. This can be challenging for staff but they continue to work on offering
various activities to try to keep it fun!

➢ Recommendations:
○ Improve communication with the staff; there were occasions where the
staff did not communicate with each other and they thought a
resident was lost.
○ Encourage residents to do more for themselves; keep them mobile and
active.
○ Encourage residents to interact with one another.
○ Provide weekly or daily walks for those who are still able to ambulate
○ Provide more healthy food options at all meal times
○ Have appropriate snack times and offer healthy snacks
