Why the patient came?

 23rd July 2002


 A 45 years old female was brought from Juhu Galli
slums in the night with severe pain in abdomen since
 The patient was brought DAMA from Nanavati
hospital as the patient was advised ICU admission
and the patient could not afford it.
What do I do?
 The patient was told that the condition is fatal
 Requires ICU management
 Patient was asked to shift to municipal
Hospital
Who brought the patient?
 The neighbours brought the patient
 Patient was already diagnosed acute
pancreatitis
• Pt. had similar attacks twice before on
14th & 21st July 2002 both of which
were Sundays
• On Sundays she cooks and eats
mutton which is very spicy and oily
• She had unconsciousness because of
pain twice. ( P/H of unconsciousness
after fights at home)
• Breathlessness < after walking
Chief complaint
• Severe pain in abdomen comes in
waves
• The attacks are
• Cravings—mutton3, oily things3,
sweets2, chicken
• Aversion—mirchi (chiilies)
• Thirst—4 glasses /day, 1 glass at a
time
• < by eating hard things like Bhutta—
abd. Pain
• Has not passed stool since 2 days,
• Used to pass once a day
• Urine—N
• Sweat—all over the body, more on
face, offensive, stains delible, white
Acute appendicitis
 Youthful exuberance
 Confirming therapeutics
Intestinal Obstruction
 Converting a non-believer
Pyogenic Meningitis
Barter’s syndrome
 Last ditch effort
