Stepwise progression:
Anti-inflammatory meds (steroids) > pulsed
radiofrequency> ablative procedures
“Intravenous rescue” = lidocaine infusion,
magnesium infusion, metoclopramine infusion
Regional blocks
Physical therapy, acupuncture
