Introduction
In the past there was a dogma suggested that the human child does not feel pain, and that it is dangerous to give the child powerful analgesia because of the risks and complications.
Pain assessment
One such standard approach of assessment of pain is QUESTT which is as follows:
Q Question the child U Use pain rating scales E Evaluate childs behavior S Secure parents involvement T Take cause of pain into account T Take earliest action
Drug therapy
Non-narcotic analgesics
Non-narcotic analgesics
Ketamine
There are evidences about the efficacy of low dose ketamine (of less than 2 mgkg-1 intramuscularly or less than 1 mgkg-1 intravenously or epidurally) in the management of acute postoperative pain.
Narcotic analgesics
Opioids are the mainstay in the management of post-operative pain and they provide increased tolerance to pain. In newborns, clearance is diminished and elimination half lives are prolonged as compared to the older children.
Conclusion
It is now widely accepted that infants and children require appropriate method of pain relief in the post-operative period for a smoother and rapid outcome. Relatively simpler nerve block techniques, along with analgesic drugs can be employed safely so as to relieve the post-operative pain.
