HIGH POTENTIAL INCIDENT IN AHTS O QUE ACONTECEU: WHAT HAPPENED: Parada de propulsores. Durante a operao de montagem de mangotes na linha principal prximo a um FPSO, ocorreu pane eltrica que levou a parada alternada dos motores principais de bombordo e de boreste. A embarcao, aps investigar as causas, identificou falhas em dois disjuntores que alimentavam o banco de baterias. Thrusters stop. During the hose assembly operation in the main line near a FPSO, an electrical malfunction occurred which caused the alternated stop of port side and starboard main engines. The vessel, after investigating the causes, identified failures in two circuit breakers that fed the battery bank.

Desarme dos disjuntores do painel de
distribuio 24VDC e, consequente, perda no simultnea dos motores de BB e BE; Disconnection of circuit breakers of the 24VDC distribution panel and, consequently, non simultaneous loss of PS and SB engines;
Ausncia de sinal sonoro/luminoso que
pudesse indicar o desligamento do disjuntor do passadio e na CCM. Absence of sound/light signal that could indicate the circuit breaker disconnection of the bridge and in the ECR.

Instalao de sinalizao sonora/luminosa nos quadros de distribuio de 24V do Passadio e
CCM, quando na ocorrncia de desarme dos disjuntores; Installation of sound/light signaling in 24V distribution panels of the Bridge and ECR, when on occurrence of disconnection of circuit breakers;
Familiarizar toda a liderana de bordo e os tripulantes de Mquina no novo Sistema de Segurana
de Motores instalado aps o incidente. Familiarize the entire leadership on board and the engine officers on the new Engine Safety System installed after the incident.
