Summary
Etiologia
O citomegalovrus (CMV), tambm conhecido como HHV-5, um
herpesvrus humano (HHV) e pertencente famlia Herpesviridae,
assim como o vrus varicela-zoster,
o vrus Epstein-Barr o HHV-8 (vrus
associado ao Sarcoma de Kaposi),
e subfamlia -Herpesvirida. Seu

no eliminado do organismo e,
como os outros herpesvrus, permanece ali de forma latente, e sua
viremia se mantm em nveis reduzidos. Em diferentes circunstncias,
ele pode ser reativado como, por
exemplo, em casos de gestao,
uso de drogas imunossupressoras,
AIDS ou qualquer outro fator que
altere o sistema imunitrio, causando
diversas doenas como pneumonia,
esofagite, encefalite, hepatite, pancreatite, gastrite, enterite, colite e
retinite (1, 4-7, 9-11).
Apesar de possuir replicao inteiramente intracelular, o que impede
a ao de anticorpos neutralizantes,
o CMV pode ser facilmente inativado
por fatores fsico-qumicos. Sua vida
mdia a 37C de 45min. O nico
reservatrio para a transmisso dos
CMV em humanos o prprio homem.
Para haver contaminao necessrio o contato ntimo j que secrees
biolgicas como smen, secrees
vaginais, saliva e urina atuam como
vetores. Alm disso, pode haver contaminao horizontal por transfuso
de rgos e sangue. Uma ltima forma
de contaminao ocorre por transmisso vertical durante a gestao
(via transplacentria), no momento
do parto ou no perodo ps-natal (via
leite materno) (3-5, 12, 13).

sugerem que a imunossupresso
ps-queimadura (36), a transfuso
de sangue (28) e o transplante de
pele de doadores sorologicamente
positivos para CMV (37) podem ser
considerados fatores de risco para
o desenvolvimento de infeco por
CMV (38).
Embora em indivduos imunocompetentes a infeco pelo CMV
no seja capaz de expressar nenhum efeito patognico, procedendo-se de forma assintomtica, em
indivduos imunocomprometidos
pode resultar em srias complicaes, devido sua disseminao
pelo organismo, com possvel
envolvimento de rgos, gerando
distrbios do trato gastrointestinal,
do sistema nervoso central, corioretinites, pneumonites, adrenalite,
alm de tornar o paciente mais
suscetvel a outras infeces, como
aspergilose, criptococose, candidase e pneumocistose (5). Alm disso,
pacientes com sorologia positiva
para infeco por CMV apresentam
um maior tempo de estadia hospitalar, quando comparados aqueles
sem evidncia sorolgica (31), o
que acaba por aumentar os gastos
com o tratamento e, principalmente, gerar uma maior exposio destes pacientes a outras infeces.

Infeco Congnita
De 10 a 25% dos recm-nascidos
infectados congenitamente apresentam inmeros sintomas, tais como,

congeladas e armazenadas. muito
utilizada apesar do seu alto custo e de
sua difcil execuo. Pode ser utilizada
tanto qualitativamente (diagnstico
por PCR), quanto quantitativamente na medio da carga viral, que
proporcional ao nvel de DNA do
CMV (44).
As deteces de IgM e IgG atravs
dos diversos mtodos sorolgicos
(imunofluorescncia indireta, ELISA,
raioimunoensaio), so rotineiramente solicitadas para o diagnstico da
infeco congnita por CMV, porm
tm papel limitado, no permitindo
afastar ou confirmar esta infeco na
ausncia de deteco viral (4).
O mtodo de ELISA (EnzimeLinked Immunosorbent Assay), que
apresenta sensibilidade de 100% e
especificidade de 86%, detecta anticorpos no sangue. Existe a possibilidade de resultados falsos-positivos
devido a reaes cruzadas com alguns vrus da famlia Herpesviridae,
fatores reumatides e anticorpos
antinucleares (45).
O esquema de interaes que
ocorre numa placa de ELISA demonstrado na Figura 4.
Pelo fato dos anticorpos IgG,
diferentemente dos IgM, conseguirem passar passivamente atravs
da placenta, sua pesquisa se torna
sem aplicao para o diagnstico de
infeco congnita. Assim, no mtodo
de ELISA, deve-se haver uma prvia
