The risks have to be assessed on the three risk factors: effect, frequency and probability and a Risk Score has to
be calculated according to formulae R= E * F * P (derived from Fine & Kinney method, see REFERENCES):

Score E = Effect of the event/ exposure


20 Fatality or multiple fatalities
15 Serious accident with permanent disabilities
10 Minor or serious accidents without permanent disabilities
5 Incidents, minor effects like small cuts, minor burns, bruises
0.1 Negligible effects or no injury at all

Score F = Frequency of exposure


5 Exposure continuous or multiple times per shift
4 Exposure 1 - 3 times per day
3 Exposure 1 - 3 times per week
2 Exposure 1 - 3 times per month
1 Exposure 1 - 5 times per year

Score P = Probability of event

This classification and scores are mandatory to be used for the sake of comparability between operations.

4.5. Operational risk prioritization

The next step is to review the Risk Scores and look into risk reduction measures. The values and actions in the
table below have been derived from the Fine & Kinney method (see REFERENCES). The countermeasures should

Please note that the Risk Score seems an accurate or absolute number. This is not the case as the ingredients for
the calculation have been estimated, based on what we know today. It is important to realise this and to redo risk
assessments periodically based on new insights (accidents that happened etc.).
