Deve ser notado, porém, que este trabalho foi realizado em 1997, quando o ensaio
TRAP ainda estava nos primeiros anos de pesquisa da sua utilidade.
Yang et al. (26) apresentaram provas mais convincentes da utilidade clínica da
telomerase em efusões pleurais em três grupos: (a) efusões pleurais malignas, de acordo
com diagnóstico citológico; (b) efusões pleurais não-malignas; e (c) efusões pleurais
suspeitas de malignidade, mas citologicamente inconclusas. Das 144 amostras, foi
detectada atividade telomerase em 91% (64 em 70) das amostras malignas, 91% (20 em
22) amostras suspeitas, e apenas 5,8% (3 em 52) das amostras não-malignas. Em geral,
a atividade telomerase tinha uma sensibilidade de 91%, uma especificidade de 94% um
valor de previsão positiva de 96%, e um valor de previsão negativo de 89%.
Curiosamente, os três resultados falso-positivos neste estudo foram de amostras de
pacientes com tuberculose. Os autores comentaram que a prevalência da tuberculose é
maior entre pacientes de câncer e concluíram que a medição da atividade telomerase é
um auxiliar útil da citologia na avaliação de efusões pleurais malignas (26).

CÂNCER DE PRÓSTATA
Meid et al. (34) investigaram o uso da análises TRAP em urina vertida após massagem
da próstata, para detecção de câncer da próstata. Os autores observaram que o exame
citológico de células esfoliadas em sedimentos de urina, mesmo após massagem da
próstata, apresenta uma sensibilidade relativamente fraca para detecção de câncer da
próstata. A atividade telomerase foi detectada em 14 de 24 amostras de pacientes com
câncer da próstata e em nenhuma dos 12 controles sem evidência histológica de
neoplasia (sensibilidade, 58%; especificidade, 100%). O teste de telomerase apresentou
mais sensibilidade em 9 pacientes com câncer fracamente diferenciado (8 em 9
positivos) do que em 15 pacientes com tumores bem ou moderadamente diferenciados

com a atividade telomerase naquela célula. Este estudo, diferente dos estudos
anteriormente discutidos, não estava medindo a atividade telomerase.
