Objetivos
Grau de Recomendao
Baseada nesta anlise est sendo proposta uma conduta para avaliao dos pacientes
com suspeita clnica de apendicite aguda que chegam ao Pronto Atendimento do HSL a ser
discutida pela equipe de Plantonistas do PA, pelas diversas Equipes de Cirurgia e pelo Setor
de Diagnsticos por Imagem que atuam no HSL.

Tratamento antibitico emprico:
Diarria do viajante Fluoroquinolona ou Sulfmetoxazol-Trimetoprim diminui a durao de 3 a 5 dias para 1 a
2 dias.
Diarria com durao > 10 a 14 dias Tratar giardase, especialmente se houver histria de exposio gua
ou viagem.
Infeces por Campylobacter resistente a quinolona (10%) pioram com o tratamento. A opo eritromicina e
derivados.

Doena intestinal inflamatria
Uso de vacina antitifide para viajantes que vo para rea endmica (Vi parenteral, Ty21a oral) e
para clera.
Frutos do mar e contato com o litoral cultura para vibrio em poca de epidemia
Diarria do viajante que no respondeu a Sulfametoxazol-Trimetoprin ou a Quinolona cultura
para
vibrio
Dor abdominal persistente e febre cultura para Yersinia enterocoltica
Dor abdominal direita, sem febre alta, mas com diarria sanguinolenta ou no sanguinolenta: solicitar
cultura para E.coli produtora de Shiga-Toxina
Sndrome Hemoltico-Urmica est associada E.coli enterohemorrgica produtora de Shiga-Toxina
(O157:H7) ou STEC
Guillain Barre pode ocorrer aps infeco por Campylobcter jejuni
Desnutrio com ou sem diarria pode se seguir infeco por E coli enteroagregativa ou
Cryptosporidium
Proctite em homem homossexual pode ser diagnosticada por retosigmoidoscopia. Envolvimento
somente dos 15 cm distais sugere herpesvirus, gonococos, chlamydia, ou sfilis. Acometimento mais proximal
sugere Campylobacter, Shigella, C. difficile ou Chlamydia. Diarria no inflamatria sugere giardase.
Teste de lactoferrina fecal ou pesquisa de leuccitos ajudam a demonstrar componente inflamatrio,
que freqente em colite invasiva por Salmonella, Shigella e Campylobacter jejuni, em colites intensas por C
difficile e nas doenas inflamatrias intestinais.
Testes para parasitas so: Fluorescncia e EIA para Giardia e Cryptosporidium; Coloraes acid-fast
para Cryptosporydium, Cyclospora, Isospora ou Micobactrias, bem como culturas para M. avium complex.
Regra dos trs dias para pacientes hospitalizados: Diarria com incio aps o 3 dia de internao em
pac. < 65 anos, no deve ter, como rotina, coprocultura colhida pela baixa relao custo-efetividade.
Rehidratao: Sempre tentar via oral, 800 a 1000 ml/hora. Vit A e Zinco em possveis portadores de
dficits. Glutamina e derivados esto em estudo como auxiliares na recuperao da mucosa.
