Conclusões: Nossos testes sugerem que amostras de SIPF nos métodos A e B são
boas alternativas para a monitorização dos níveis de Phe em pacientes com PKU,
sendo que o método A apresentou maior concordância em relação ao padrão ouro.
Introduction: Inborn Errors of Metabolism (IEM) are genetic diseases caused by the
absense of activity of one or more specific enzymes or by defects in the transport of
proteins. Phenylketonuria (PKU) is an IEM caused by the deficiency of the
phenylalanine hydroxylase (PAH), enzyme that is responsible for the conversion of
Phenylalanine (Phe) into Tyrosine (Tyr). This disease can be detected early through
Neonatal Screening and its diagnosis is confirmed through the measurement of
amino acids by High Performance Liquid Chromatography (HPLC). The excess of
Phe and the consequent amino acid imbalance cause toxic effect on the Central
Nervous System (CNS) and consequently loss of cognitive function and cognitive
deficit. The current treatment is mainly made through restricted diet in Phe,
supplementation of essential amino acids and laboratory and clinical follow-up. It is
important to remember that early diagnosis and treatment can lead to excellent
results in the quality of life of these patients.
Objective: The objective of the present study was to evaluate two different methods:
a fluorimetric method developed In House (method A) and the commercial Neonatal
Phenylalanine Kit (Perkin Elmer®) (method B) for the detection of Phe levels in dried
blood spots samples (DBS) in patients with PKU who are being followed up at the
ambulatory of the Medical Genetics Service of the Hospital de Clínicas of Porto
Alegre (MGS-HCPA) and compare these results with the results obtained by HPLC,
which is considered the gold standard for the diagnosis and monitoring of patients
with PKU.
Conclusions: Our tests suggest that DBS samples in methods A and B are good
alternatives for the monitoring of Phe levels in patients with PKU, and method A
showed greater agreement with the gold standard.

1. INTRODUÇÃO
A fenilcetonúria (PKU) é uma aminoacidopatia causada pela deficiência da
enzima fenilalanina-hidroxilase (PAH), a qual conduz um acúmulo de fenilalanina
(Phe) No Brasil, a PKU pode ser detectada através do Programa Nacional de
Triagem Neonatal (PNTN) ou Teste do Pezinho (TP) que deve ser feito entre o 3º e o
5º dia de vida. Se o resultado do TP for alterado ele deve ser repetido em nova
amostra de sangue pelo método de Cromatografia Líquida de Alta Performance
(HPLC) que é considerado o Padrão Ouro no diagnóstico de PKU.

Segundo Marsden e Levy (2006) a HPA por deficiência de PAH pode ser
dividida em PKU Clássica, PKU Atípica e HPA Não-PKU, sendo que ainda pode ser
acrescentada a HPA transitória (MARSDEN; LEVY, 2006).
PKU Clássica: nível de Phe sanguínea > 1200 µmol/L (> 20mg/dL) em dieta
normal.
PKU Leve: nível de Phe sanguínea entre 360 – 1200 µmol/L (6 – 20mg/dL)
em dieta normal.
HPA Não-PKU: nível de Phe sanguínea entre 120 – 360µmol/L (2 – 6mg/dL)
em dieta normal.
HPA transitória: devido à prematuridade, iatrogenia, doença neonatal do
fígado ou hiperfenilalaninemia materna.

de Phe dietética ao dia, para manter um nível sanguíneo de Phe seguro de
não mais de 300 µmol/L (5mg/dL).
PKU Moderada: os indivíduos afetados toleram 350 – 400mg de Phe
dietética ao dia, para manter um nível sanguíneo de Phe seguro de não mais
de 300 µmol/L (5mg/dL).
PKU Leve: os indivíduos afetados toleram 400 – 600mg de Phe dietética ao
dia, para manter um nível sanguíneo de Phe seguro de não mais de 300
µmol/L (5mg/dL).
HPA Leve: os indivíduos afetados tem concentração plasmática de Phe
menor que 600 µmol/L(10mg/dL) em dieta normal (GULBERG et al., 1998).

O nível de Phe sanguínea está diretamente relacionado ao grau de retardo e
lesão neurológica, que também se correlacionam com a idade que o tratamento foi
iniciado. Contudo, pacientes que iniciam o tratamento precoce e adequado podem
sofrer algum déficit, especialmente relacionado à atenção e percepção. Através
disso percebe-se a necessidade da continuidade do tratamento, durante a vida
inteira, de maneira rigorosa (MARTINS et al., 2006).
Mesmo pacientes adultos que não seguem a dieta restrita em Phe podem
apresentar sintomas como, depressão, sintomas de ansiedade, agorafobia e
dificuldades psicossociais. Até mesmo aumentos transitórios dos níveis de Phe
sanguínea podem provocar déficit cognitivo transitório (MARTINS et al., 2006).

sugerem que seja realizado o teste de tolerância de Phe da dieta com os pacientes
com HPA (NYHAN; OZAND, 1998; SUMAILY; MUJAMAMMI, 2017).
2.5 TRATAMENTO
Em 1953, Bickel foi o primeiro a divulgar para o mundo que através de uma
dieta restrita em Phe, os níveis sanguíneos desse aminoácido diminuem e evita-se o
dano neurológico naqueles indivíduos que tem diagnóstico precoce (PLASENCIA;
TORRES; TAMAYO, 2009). Desde então, as possibilidades de terapia vêm
crescendo e ainda hoje os trabalhos pioneiros de Bickel continuam sendo à base do
tratamento da mesma (PLANA et al., 2006). O diagnóstico conjuntamente com início
do tratamento precoce leva a excelentes resultados no prognóstico da PKU
(HOEKS; HEIJER; JANSSEN, 2009).
Mesmo que o tratamento com dieta restrita em Phe e o uso da FM seja bem
sucedido, a pobre palatabilidade da mesma gera uma baixa adesão ao tratamento,

principalmente por adolescentes e adultos (MITCHELL; SCRIVER, 2011). Devido a
isso, algumas terapias estão sendo investigadas, tais como:
O tratamento dos pacientes com PKU deve ser mantido durante o resto da
vida, uma vez que HPA na adolescência tem sido associada com distúrbios de
atenção, instabilidade de humor, degeneração da substância branca que leva a
convulsões e distúrbios de marcha (BÉLANGER-QUINTANA et al., 2011).
