ACCIDENT WITH INJURY IN AHTS VESSEL

O QUE ACONTECEU:
WHAT HAPPENED:
Leso na perna de um marinheiro ao ser atingido por um cabo de retinida que estava tensionado no
convs, quando o mesmo se soltou de uma estrutura tubular ao qual estava preso.
Injury on a seamans leg who was hit by a heaving line which was tensioned on deck, when it came off from a
tubular structure to which it was tied.

de orientao no
procedimento, pois
no
mencionava a utilizao do
guincho auxiliar na atividade de
transferncia de cabo.

because it didnt mention the use of the
tugger winch on the wire rope transfer
activity.

Retinida presa na estrutura

Heaving line tied to the structure

Falha na gesto de riscos, pois no

foi levado em considerao o peso
do cabo durante a transferncia.
Risks management failure, because it
was not considered the weight of the
wire rope during the transfer.

Falha na comunicao, pois no

houve aviso que a retinida estava
amarrada antes do acionamento do
guincho auxiliar.

Retinida soltou da estrutura

was no warning saying that the wire
hope was tied before the auxiliary winch
was started

Falha na gesto de mudanas,

pois no foi avaliada a utilizao do
guincho auxiliar na atividade.
Failure on Changes management,
because the use of the tugger winch
was not evaluated for the activity.

O QUE DEVEMOS FAZER PARA EVITAR:

WHAT WE SHOULD DO TO AVOID:

Revisar e treinar a fora de trabalho no procedimento de transferncia de cabos, incluindo a orientao

de utilizao do guincho auxiliar nas operaes de transferncia de cabo.
Revise and train the workforce on the wires transfer procedure, including the orientation regarding the use of the
tugger winch on wire transfer operations.

Revisar as anlises de risco da operao considerando o peso do cabo durante sua

transferncia.
Revise the operation risk assessment considering the weight of the wire hope during its transfer.

Considerar os riscos das tarefas onde haja possibilidade de tensionamento de cabos, advertindo
os trabalhadores a manterem-se protegidos de eventuais chicoteamentos dos cabos.

Consider the task risks where there is a possibility of wire hope tension, warning the workers to keep sheltered
from possible heaving line snapback.

CERTIFIQUE-SE AO RECOLHER CABOS, DE QUE NO HAJA POSSIBILIDADE DE

OCORRER TENSIONAMENTOS!!
MAKE SURE THAT THERE IS NO POSSIBILITY OF TENSION WHEN HEAVING UP WIRE ROPES!!
