ABSTRACT
Abstract
Population ageing leads to an increased prevalence of neurodegenerative
diseases and age-related cognitive decline. The epsilon 4 (4) alelle of the
apolipoprotein E (ApoE) gene is known to be a key genetic risk factor for Alzheimers
disease (AD) and possibly for other neurological disorders. Some evidence in the
literature indicates that allele 4 interferes with human cognition independently of
chronological age. The present study investigated the correlation of allelic variants of
ApoE with the cognitive performance of elderly individuals without apparent cognitive
impairment. Regardless of a statistical power compatible with the purpose, the present
study did not identify differential performance across ApoE genotypes when a sample
of 213 cognitively preserved older adults were submitted to a sequence of
neuropsychological tests encompassing several cognitive domains, including short- and
long-term episodic memory, processing speed, attention and executive functions. There
were no effects of age, gender, marital status, schooling, depressive symptoms and use
of central nervous system depressants when the analyses were controlled for such
factors. Our findings support the hypothesis that the allele 4 does not contribute to
detectable cognitive decline within the context of non-dementia.

1. INTRODUO
O envelhecimento populacional ocasiona um aumento da prevalncia de doenas
neurodegenerativas, sobretudo demncias. A prevalncia mdia de demncia, acima dos
65 anos, varia entre 2,2% na frica, 5,5% na sia, 6,4% na Amrica do Norte, 9,4% na
Europa e7,1% na Amrica do Sul (Lopes e Bottino, 2002). Estudos brasileiros
descrevem taxa de prevalncia de demncia intermediria entre os resultados dos
estudos internacionais: 7,1% (Scazufca, Cerqueira et al., 2002). Entre as diferentes
formas, a Doena de Alzheimer (DA) consiste na mais frequente, com aumento
significativo em sua prevalncia em dcadas recentes (Castellani, Rolston et al., 2010).
Marcadores ou aspectos clnicos que permitam um diagnstico precoce da DA
tem sido objeto de estudos e nesse campo destacam-se os marcadores genticos
(Bertram, Lange et al., 2008). Dentre eles, o alelo psilon 4 (4) do gene para a
apolipoproteina E (ApoE) constitui o principal fator gentico de risco para DA
(Bookheimer e Burggren, 2009).
No contexto clnico, questiona-se quando e como esta variante genotpica
manifesta-se fenotipicamente na cognio humana. Neste sentido, h evidncias de que
o alelo 4 interfira no desempenho cognitivo independentemente da idade cronolgica
(Tuminello e Han, 2011) e de sinais ou sintomas compatveis com quadro demencial
(Caselli, Reiman et al., 2007). Outra vertente entende a presena do alelo como fator de
risco para DA a partir do diagnstico, em uma contribuio alelo-especfica ao advento
e evoluo da doena, conferindo uma maior velocidade de perda cognitiva (Bunce,
Anstey et al., 2011).
Diante do aumento na expectativa de vida e de diferentes hipteses existentes
associadas s possveis expresses fenotpicas vinculadas ao gentipo de ApoE,
pesquisas em idosos no dementes contribuem na elucidao da relao entre este
gentipo e desempenho cognitivo.

ltimo caso com protagonismo dos determinantes genticos, tendo os genes ora um
papel principal, ora coadjuvante para uma expresso fenotpica (Greenwood e
Parasuraman, 2003). Considerando o envelhecimento humano e as alteraes cerebrais
secundrias a este processo, o fator gentico pode agir como determinante ou
contribuinte na expresso de algumas patologias. Dentre inmeros casos, podemos citar
o gene da presenilina, presente no cromossomo 21, que est relacionado DA pr-senil
de forma autossmica dominante (Korenberg, Chen et al., 1994).
No entanto, essa descoberta no abarca os casos de DA espordicos e de incio
tardio que representam 85-90% dos casos. No incio da dcada de 90, identificou-se
uma associao entre a regio do cromossomo 19 que determina a produo de
apolipoprotena E (ApoE) e a DA (Saunders, Strittmatter et al., 1993).
ApoE uma glipoprotena presente no plasma humano, sintetizada no fgado e
responsvel pelo transporte de lipdeos atravs do plasma e outros fluidos corporais
(Moraes, Lins et al., 2012). a apolipoprotena mais presente no sistema nervoso
central, localizada no cromossomo 19, e das suas trs isoformas principais 2,
( 3 e 4)
h seis diferentes combinaes genotpicas: ApoE
2/2, ApoE 3/3, ApoE 4/4,
ApoE 2/3, ApoE 3/4, e ApoE 2/4.
Nos seres humanos, o crebro o segundo local em que a sntese de ApoE mais
ocorre. Embora existam evidncias sobre o seu papel especfico na formao da placa
amilide e dos emaranhados neurofibrilares, bem como da associao destas formaes
histopatolgicas etiologia da DA, os mecanismos subjacentes ainda so pouco claros
na literatura (Moraes, Lins et al., 2012).
Atualmente,dentre os fatores de risco para DA, lista-se a idade(Raber, Huang et
al., 2004) e a presena do alelo 4 do gene para a ApoE como importantes fatores de
risco para DA de incio tardio. O alelo 4 considerado o maior fator gentico (Blom,
Giedraitis et al., 2009), aumentando em aproximadamente 3 vezeso risco para cada
adio de alelo 4 (Corder, Saunders et al., 1993). E, quando comparados a sujeitos com
2/3 ou 3/3, sujeitos com o gentipo 4/4 seriam 8 vezes mais susceptveis a
desenvolver DA (Bookheimer e Burggren, 2009).
Observa-se, tambm, relao entre idade de incio da doena e gentipo de
ApoE em pacientes diagnosticados com DA: 50% dos carreadores de 4 tem em mdia
66 anos de idade; 50% dos 3/4 tem mdia de 73 anos de idade; e metade dos 3/3
tem 86 anos. Indivduos no carreadores de 4 e carreadores de 2 tem uma idade de
inicio de doena ainda mais tardia que os demais grupos (Raber, Huang et al., 2004).

2. OBJETIVOS
2.1 Principal
Investigar a associao de variantes allicas do gene da ApoE com o perfil
cognitivo de idosos sem comprometimento cognitivo aparente.
2.2 Secundrio
Identificar fatores que interferem no desempenho dos testes cognitivos em
idosos sem comprometimento cognitivo aparente.

1. Introduo
O envelhecimento populacional proporciona um aumento na prevalncia de
doenas neurodegenerativas, sobretudo demncias. Entre as diferentes formas, a Doena
de Alzheimer (DA) consiste na mais frequente, com aumento significativo em sua
prevalncia em dcadas recentes. DA consiste em um comprometimento cognitivo e
comportamental com intensidade suficiente para interferir nas atividades de vida diria
e qualidade de vida, caracterizada pelo dficit da memria associado ao dficit em ao
menos outra funo cognitiva, tais como funes executivas, linguagem e ateno,
excludas outras etiologias demenciais [3].
Marcadores ou aspectos que permitam um diagnstico precoce da DA tem sido
objeto de estudos e nesse campo destacam-se os marcadores genticos [4]. Sabe-se que
o alelo psilon 4 (4) do gene para a apolipoproteina E (ApoE) constitui o principal
fator gentico de risco para DA [5] e tem possvel participao tambm em outras
desordens neurolgicas[28].
ApoE consiste na apolipoprotena presente em maior quantidade no sistema
nervoso central (SNC), sendo o crebro o segundo local em que sua sntese mais ocorre
no organismo humano. Suas variantes allicas no cromossomo 19 apresentam-se em trs
isoformas principais (2, 3 e 4) e seis combinaes genotpicas diferentes: trs
homozigticas (2/2, 3/3, 4/4) e trs heterozigticas (2/3, 3/4, 2/4).
Embora existam evidncias sobre o papel especfico da ApoE na formao da
placa amilide e dos emaranhados neurofibrilares, bem como da associao destas
formaes histopatolgicas etiologia da DA, os mecanismos subjacentes ainda so
pouco claros na literatura [18]. No contexto clnico, questiona-se quando e como uma
variante genotpica manifesta-se fenotipicamente na cognio humana. Neste sentido,
h evidncias de que o alelo 4 interfere no desempenho da cognio humana
independentemente da idade cronolgica [24] e de sinais ou sintomas compatveis com
quadro demencial [7]. Outra vertente entende a presena do alelo como fator de risco
para DA e maior velocidade de perda cognitiva a partir do diagnstico, em uma
contribuio alelo-especfica ao advento e evoluo da doena [6].
Pesquisas em idosos no dementes buscam identificar declnio no desempenho
cognitivo que possam ser associados a uma contribuio pelo gentipo existente. Neste
sentido, este trabalho almeja investigar a associao de variantes allicas do gene da

4. DISCUSSO COMPLEMENTAR
O principal objetivo deste estudo consistiu em investigar a associao de
variantes allicas clssicas do gene para ApoE com o perfil cognitivo de idosos sem
comprometimento cognitivo aparente residentes na comunidade atendidos em servios
especializados em geriatria no Distrito Federal. Este estudo, por sua vez, no identificou
efeito dos gentipos de ApoE sobre a performance nos testes cognitivos aplicados
(MEEM, Escala Wechsler-Pares Associados, Dgitos diretos e inversos, Torre de Hani
e TEVA) na amostra, apesar do rigor metodolgico na aplicao dos testes, dos critrios
para seleo dos pacientes e do controle de variveis confundidoras, como escolaridade,
idade, sintomas depressivos, uso de frmacos psicotrpicos e outros. Frente a essas
consideraes e diante dos resultados encontrados, alguns aspectos metodolgicos e de
resultado sero comentados a seguir.
4.1 Seleo da Amostra
A amostra selecionada formada por pacientes provenientes de dois centros
mdicos localizados em diferentes regies do Distrito Federal: o Centro de Medicina do
Idoso (CMI) e o Hospital da Universidade Catlica de Braslia (HUCB). O CMI um
centro de atendimento geritrico, integrante da estrutura organizativa do Hospital
Universitrio de Braslia (HUB/UnB), localizado regio central da capital federal. O
HUCB um hospital universitrio organizado em torno de determinadas especialidades
clnicas, incluindo geriatria, e localiza-se na regio administrativa de Taguatinga,
Distrito Federal.
4.2 Caracterizao da amostra
A amostra foi de 213 indivduos, com mdia de idade de 72,9 anos, maior
frequncia de mulheres (n = 187; 81,2%), maioria de casados (n = 86; 40,4%) e vivos
(n = 82; 38,5%) e mdia de anos de estudo de 5,7 anos.
O percentual de mulheres observado neste estudo est acima do observado na
populao idosa geral. Porm, coerente com outros estudos que observam maior
frequncia de mulheres nos servio de atendimento a pessoas idosas. H autores que
associam este elevado percentual a uma tendncia maior das mulheres em participar de
aes coletivas referentes sade, confirmando que a participao dos homens em
grupos de idosos consideravelmente menor que das mulheres (Trentini, Silva et al.,

5. CONCLUSO
Nosso estudo no identificou associao entre o gentipo de ApoE e
desempenho cognitivo em idosos sem comprometimento cognitivo aparente. Estes
achados corroboram a hiptese de que o alelo 4 no contribui com um declnio
cognitivo detectvel em um contexto de no demncia.
Em relao ao objetivo secundrio, observou-se influncia da idade e
escolaridade na performance em testes cognitivos que avaliam desempenho global,
memria de curto e longo prazos, ateno, flexibilidade cognitiva e velocidade de
processamento. Estes resultados tambm so vastamente corroborados pela literatura a
respeito de envelhecimento cerebral em idosos no-dementes.

Abstract
Population ageing leads to an increased prevalence of neurodegenerative
diseases and age-related cognitive decline. The epsilon 4 (4) alelle of the
apolipoprotein E (ApoE) gene is known to be a key genetic risk factor for Alzheimers
disease (AD) and possibly for other neurological disorders. Some evidence in the
literature indicates that allele 4 interferes with human cognition independently of
chronological age. The present study investigated the correlation of allelic variants of
ApoE with the cognitive performance of elderly individuals without apparent cognitive
impairment. Regardless of a statistical power compatible with the purpose, the present
study did not identify differential performance across ApoE genotypes when a sample
of 213 cognitively preserved older adults were submitted to a sequence of
neuropsychological tests encompassing several cognitive domains, including short- and
long-term episodic memory, processing speed, attention and executive functions. There
were no effects of age, gender, marital status, schooling, depressive symptoms and use
of central nervous system depressants when the analyses were controlled for such
factors. Our findings support the hypothesis that the allele 4 does not contribute to
detectable cognitive decline within the context of non-dementia.

1. Introduction
Population ageing results in an increased prevalence of neurodegenerative
diseases, especially dementia. Among the different forms of dementia, Alzheimers
disease (AD) is the most common, and its prevalence has increased in recent decades
[12]. AD causes a level of cognitive and behavioural impairment that interferes with
everyday life activities and the quality of life. It is characterised by memory impairment
associated with deficit in at least one other cognitive function, such as executive
functions, language, and attention, with the exclusion of other causes of dementia [4].
The markers and other features that allow for the early diagnosis of AD,
especially genetic markers, are subject of attention [5]. Allele epsilon 4 (4) of the
apolipoprotein E (ApoE) gene is the main genetic risk factor for AD [6], and it is
possibly involved in other neurological disorders [30].
ApoE is the lipoprotein present in the greatest amount in the central nervous
system (CNS), and the brain represents the second most important site for its synthesis
in the entire human body. The allelic variants of ApoE in chromosome 19 exhibit three
main isoforms (2, 3, and 4) and six different genotypic combinations: three
homozygous (2/2, 3/3, and 4/4) and three heterozygous (2/3, 3/4, and 2/4).
Although there is evidence regarding the specific role that ApoE plays in the
formation of amyloid plaque and neurofibrillary tangles and regarding the association of
these histopathological formations with the aetiology of AD, the underlying
mechanisms are still poorly explained in the literature [20]. From the clinical
perspective, there are doubts about when and how a given genotypic variant manifests
phenotypically in human cognition. In this regard, some evidence indicates that allele 4
interferes with human cognition independently of chronological age [26] by determining
signs and symptoms compatible with cognitive decline in a context devoid of dementia
[8]. Another line of research shows that the presence of this allele is a risk factor for AD
and an accelerator of cognitive loss once the diagnosis is set, thus accounting to an
allele-specific contribution to the appearance and progression of the disease [7].
Studies conducted with non-demented elderly seek to identify impairment in
cognitive ability that might be associated with the genotype. Therefore, the aim of the
present study was to investigate the association of the allelic variants of ApoE with the
cognitive performance of elderly individuals without apparent cognitive impairment.

as well as the use of benzodiazepines, antipsychotics, antidepressants, tricyclic

drugs

anti-hypertensive,

depressive symptoms, and the different classes of CNS depressants in use on the model
when the analyses were controlled for such factors.
4. Discussion
The present study did not identify a correlation between classic genotypes of
ApoE and differential performances in a battery of neuropsychological tests (MMSE,
STM, LTM, DSF, DSB, DSTotal, Tower of Hanoi, and SRT) in elderly individuals
clinically identified as having preserved cognitive functions. The investigation of
performance in cognitive tests among the elderly requires judicious procedures that are
able to establish the cognitive integrity of these patients to exclude those with mild
cognitive impairment and preclinical stages of AD. In this regard, our study represents a
contribution to the scientific literature because it was performed with a sample enrolled
in a prospective research project where clinical follow-up was performed on a regular
basis, thus ensuring the exclusion of diseases and other variables that might interfere
with cognition, such as subclinical and early cases of AD. Despite the rigorous
exclusion criteria used, the methodological rigor in the application of tests and the
control of confounding variables, such as schooling, age, comorbidities, and use of
medications, no adjustment made significant the observed differences between carriers
and non-carriers of a given ApoE allelic form. The multiple linear regression model
indicated that the lack of significant differences could not be attributed to a limited
power of the test, which varied between 0.86 and 1.00 for all of the investigated
variables.
There is no consensus in the literature on the possible correlation between ApoE
genotypes and cognitive performance in non-demented individuals. Notwithstanding the
contribution of 4 to AD, some individuals develop the disease in the absence of this
genotype, whereas some carriers do not develop dementia. This fact points to other
genomic elements or a wide allelic architecture as the centre of the genetic aetiology of
this disorder. In addition, one observation that is not fully supported by Mendelian
concepts suggests that half of the first-degree relatives of patients with AD will develop
the disease by their ninth decade of life[23].
From the histopathological point of view, the current notions on the role that
ApoE plays in the human brain assert that it favours the deposition of beta-amyloid
plaques [22], increases the phosphorylation of protein tau [16], and impairs neuronal
repair
among
carriers
compared
to
non-carriers
[19].
However,
these

form an exclusive case series of this genotype. For this reason, it was not possible to test
the possibility of allele e4 expressing a dose-dependent effect.
5. Conclusions
To summarise, our study did not find a relationship between ApoE genotypes
and cognitive performance in non-demented elderly individuals. Our findings
corroborate the hypothesis that the allele 4 does not promote detectable cognitive
disorder within the context of non-dementia.
