INTRODUO
A integridade da parede abdominal tem sido tema de grande interesse
cirrgico, dada sua importncia no cumprimento de funes digestivas,
respiratrias, obsttricas, vasculares e, inclusive, ortopdicas (BUENO &
LAZARO DA SILVA,1985; GRAC0VETSKI et alii, 1985; ERLICH et alii, 1936
VASCONCELOS, 1986). A ausncia dessa integridade, pode dificultar ou mesmo
impossibilitar o cumprimento dessas funes fisiolgicas, sendo necessrio
recorrer a manobras cirrgicas para resolver ou melhorar a situao em
questo.
A motivao para realizar este trabalho decorreu portanto, da
necessidade de se pesquisar um material destinado a substituir ou
reforar a parede abdominal. A alternativa de utilizar um segmento do
intestino delgado nos pareceu oportuno. pelo fato de ser um material
autlogo de ampla disponibilidade. Deste rgo, preferimos o intestino
delgado por sua melhor tolerabilidade remoo. A remoo do clon
oferece risco adicional (GAMBEE et alii, 1956;
WISE
et alii 1975;
DUN et alii, 1978; SHIKATA & SHIDA 1986). No intestino delgado
dispensamos o ileon e optamos pelo jejuno cuja remoo melhor tolerada
(DOWLING & BOTH, 1967; BRISTOL et alii, 1985) possui criptas menos
profundas que poderiam provocar uma indesejvel permanncia da mucosa
(EASTWOOD, 1977; WEISNER et alii, 1986)
REVISAO DA LITERATURA
MATERIAL E MTODOS
' Sabendo, desde ento, que a mucosa intestinal pode persistir quando
exposta na superfcie corporal, e que a presena do pedculo nem sempre

abscesso, que foi necessrio drenar; nos outros casos a secreo drenou
livremente atravs do defeito cutneo. Nenhum desses ces apresentou
sinais de toxemia ou disseminao do quadro infeccioso.
A distribuio das complicaes no mortais segundo os grupos
experimentais mostrada na Tabela 4.2.
As alteraes cutneas, caracterizaram-se por aberturas progressiva da
ferida cirrgica,, produzindo desde fstulas at deiscncias totais
(Figura 4.1) achados j assinalados, comentados organizados de acordo
com sua distribuio no item anterior
A mucosa jejunal estava presente em todos os ces sacrificados, sem se
poder observar diferenas macroscpicas marcantes entre as diferentes
tcnicas operatrias empregadas.
O aspecto da parede do remendo jejunal, observado por meio de um corte
transversal, mostrava graus variados de deiscncia cutnea e convexidade
intestinal, com maior espessura na periferia que na parte central. (Figura
4.3).
Em 27 ces que evoluram sem aderncias, o remendo apresentava-se livre
apenas com elo mesentrico de aspecto fibroso e enxge cujo revestimento
peritoneal apresentava continuidade com o revestimento peritoneal da
parede abdominal (Figura 4.4).
Na presena de aderncias (oito casos),uma fcil liberao atravs de
manobras digitais, permitia conferir as caractersticas acima
mencionadas.
Dos quatro ces que tiveram pedculo vasculomesentrico preservado, em
dois (16 e 22 dias de ps-operatrio), a seco do pedculo
vasculomesentrico evidenciava textura fibrosa, e caracterstica enxge;
nos outros dois ces com menor tempo de cirurgia ( 6 e 7 dias de psoperatrio) a seco do pedculo vasculomesentrico evidenciou leve
sangramento, seguido de cianose e contrao muscular do remendo jejunal.
Nos ces que tiverem seu pedculo vasculomesentrico secionado ( Grupo B,
ligadura ) ,conferiu-se, que mesmo sem elo vsculo-mesentrico ,
preservavam caractersticas
macroscpicas normais (Figura 4,4).
Cabe destacar que durante a
obteno
da
pea anatomopatolgica,
nos ces de outros grupos, em alguns casos, o corte do pedculo
vasculomesentrico mostrava pouca ou nenhuma hemorragia, sugerindo uma
diminuio ou ausncia de permeabilidade vascular.

Comentrios sobre a tcnica e evoluo operatria das abdominojejunoplastias.
1967; LIAVAG & VAAGE, 1972 ;HASLUNB, 1973? HAGLUND et alii, 1975;
W1LLIAMS0N & CHIE, 1978; D0WLING, 19S2; FALK et alii, 1985; HAGLIND et
alii, 1985; SIRONTIN, 1986) porque 2/3 a 3/4 do fluxo vascular da parede
intestinal correspondem ao fluxo mucoso (PERBECK et alii, 1985 b)
fazendo-la a camada mais susceptvel isquemia {LILLEHEY, 1957; CHIU et
alii, 1985) motivo pelo qual, acreditamos tm contribudo para a atrofia
mucosa encontrada em nosso trabalho; tanto na forma aguda como na forma
crnica.

A isquemia aguda produzida por pinamento do pedculo, para ser
eficiente, deveria ser por um perodo maior a uma hora, conforme a
recomendao de CHIL e cols. (1970). Um nico co que atingiu este tempo
de isquemia faleceu, sem nos proporcionar a oportunidade de conferir esta
hiptese; os outros ces, sofreram perodos menores de isquemia que
possivelmente possibilitaram a regenerao mucosa a partir das clulas
criptais persistentes (LEBLON & STEVENS, 1948; CARNIE et alii, 1965;
WAGNER & GABBERT, 1979 c) que graas ao seu maior ndice citocintico
(CARO, et alii 1962; JOFTES, 1963; GALLAND et alii, 1968.

As isquemias crnicas, certamente estiveram presentes no Grupo B
(ligadura e seco do pedculo), e possivelmente nos outros grupos,
contribuindo com seu efeito trfico negativo para os achados histolgicos
da mucosa, pois as agresses agudas so reversveis enquanto as crnicas
deixam suas seqelas (LILLEHEY et alii, 1959; LOBE et alii, 1986).
Todas, essas hipteses relativas aos influxos dos fatores trficos
fisiolgicos precisam de melhor entendimento baseados em futuros trabalhos
experimentais.
O fator trfico patolgico modificador da mucosa intestinal o "Trauma
mucoso" de natureza inespecfica e reversvel (DOWLING, 1982; EASTWODD,
1977; WILLIAMSON, 1978}. Trauma que produz uma imagem histolgica tpica,
caracterizada por hipertrofia criptal compensatria (LOEHRY & GREACE,
1974) da atrofia vilosa decorrente da perda traumtica ou da hipertrofia
das pontes intervilosas (LOEHRY & CREAMER, 1969), imagem histolgica
tambm presente em nosso modelo, provavelmente produto das agresses
mecnicas (JOSEPH & THOMAS, 1958; JOSEPH, 1960; MILTON & FINGK, 1960;
STHEPENS et alii,1964; ZETTEELUND, 1962; TOWLEY et alii, 1964; LOEHRY &
GREACE, 1974; SENIOR et alii, I985; MATTOS & LAZARO DA SILVA, 1986), e
possivelmente, tambm das agresses infecciosas (SYM0NS,1965; ZUFAROV et
alii, 1973) que sendo diferentes em cada um dos ces inclusive dentro de
um mesmo remendo jejunal, justificam nossos heterogneos achados
histo1gicos.
Cabe destacar que nosso modelo experimental no mostrou sinais de
displasia, fato controvertido, baseado em outros trabalhos experimentais,
que guardam alguma relao com o nosso (WILLIAMSON et alii, 1980).
Sobre a hipertrofia da camada submucosa e muscular do remendo jejunal.
A hipertrofia destas duas camadas constatada aos exames histolgico e
histomtrico, poderia ter sido decorrente de processos adaptativos do
remendo jejunal s diferentes condies baromtricas da parede abdominal,
j que a presso intra-abdominal a ser suportado pelo jejuno, uma vez
transposto para parede abdominal, maior que a presso intraluminal
intestinal fisiolgica, possivelmente provocando uma hipertrofia
adaptativa ou seguindo um outro mecanismo fisiopatolgico de desconhecido.
Sobre a fibrose da camada muscular serosa.
A fibrose deve ter sido causada por um processo reparativo decorrente da
isquemia crnica nos segmentos jejunais transpostos, esses tipos de
